About this file
SDWIS/State Water Sample Schedule Report Summary
These are State Drinking Water Information System (SDWIS) water sample schedule reports issued by New York State county health departments (Monroe County Health Department, Oneida County Health Department, and Tompkins County Health Department) for three public water systems: OCFS Industry Residential Center (PWS ID: NY2701993), Taberg/Limited Secure (PWS ID: NY3210747), and MacCormick Secure Center (PWS ID: NY5406797). The reports outline mandatory water quality sampling requirements and compliance obligations for each facility across multiple contaminant categories and testing frequencies. Sampling locations include distribution systems, treatment plants, and entry points, with specific sample sites designated at residential buildings, bathrooms, and maintenance areas. The reports specify sampling frequencies ranging from monthly to every nine years depending on the contaminant type, with sample collection windows typically between June 1 and September 30 for lead and copper testing and other prescribed periods for disinfection byproducts, radiological testing, and organic chemical analysis. Due dates for sample collection extend through 2029, with some sampling requirements already overdue or completed by health department personnel.
The sample requirements address total coliform, nitrate, lead and copper, disinfection byproducts, primary and secondary inorganic chemicals, organic chemicals, per- and polyfluoroalkyl substances, and radiological contaminants in accordance with New York State Department of Health regulations and EPA standards. Sampling protocols require collection of treated water at entry points, first-draw samples from residential locations, and samples at points of maximum residence in distribution systems. Several facilities have received partial waivers from sampling requirements based on vulnerability assessments and absence of hazardous waste sites in watershed areas. Health departments have assumed responsibility for collecting certain samples at multiple facilities. All sampling results require submission to the appropriate county health department, with consumer notification mandated for lead and copper results collected at individual residences per federal regulations.

Please collect a TREATED water sample at the ENTRY POINT in the TREATMENT PLANT (or nearest available TREATED WATER SAMPLE LOCATION - such as the First Customer). DO NOT USE RAW / UNTREATED WATER for any sample to be collected from the ENTRY POINT (unless treatment is not required). Please Indicate on all submission paperwork, the chlorine residual measured at the time of the sample. Please note, all Nitrate samples without chlorine residuals noted or samples collected from a RAW / UNTREATED water sample location must be analyzed within 48 hours of collection. Samples with a chlorine residual have a much longer holding time.

Sample Point: DIST SYSTEM GENERAL Sample Point No.: GENERAL Sample Point Type: DS-DlsIribulion System
System must collect 5 first draw samples every 3 years between June 1 and September 30. The sample locations should meet the requirements established in Subpart 5-1.42. Due to increased system population and facility locations In 2013. system must collect 5 samples. The samples must be first drawwater (after 6 hours molioniess) from previously used locations used for drinking water only. Do not use locations used for purposes other than drinking water (e.g. mop sinks, process water, etc...).
Federal Lead and Copper Rule regulations require consumer notification regarding samples collected at individual residences. Therefore, In addition to the standarri rnerwork submission, all customers must be nolified of results of samples collected at their residence. OCHD will assist with appropriate notification letter pursuant to sample results.

Stage 2 DBP Rule is effective beginning 2014. 2014 results Indicated low TTHM and HAAS concentrations.
Therefore, sampling has been reduced to a 3-year frequency. The system is a small system (<500), therefore a single sample set may be collected for TTHM/HAA5. The samples must be collected at the LRAA1 sample location - the point of highest result during the lime of highest result. At this time, the system must collect samples from the Point erf Maximum Retention on the First Monday of August every three years (e.g. 2017,2020.2023.2028. etc...). Should sample results Increase above a specified level, increased sampling frequency may be required.

PARTIAL WAIVER ISSUED - Pursuant to NYSDOH Techrrfcal Reference Manual Item PWS 73 (Determining Vulnerability to Contamination by Pesticides and other Synthetic Chemicals), sampling for bis- (2-elhyl-hexyl)-adipate, bis-(2-elhyl-hexly1)-phthalale, and benzo-a-pyrene, may be waived as long as certain criteria are met. namely that no mura'cipal or hazardous waste sites, SARA Title III locations or military locations are present on the watershed area. Since no related contaminants have been found in previous samples and since this water system meets the locational criteria, the system is granted a waiver from sampling for the noted contaminants.
