About this file
This is an Indian Economic Enterprise (IEE) Representation Form issued by the Indian Health Service's Division of Acquisition Policy, which serves as a self-certification document for vendors responding to solicitations under the Buy Indian Act (25 U.S.C. 47).
The form requires vendors to certify their status as an Indian Economic Enterprise with Indian Ownership, maintaining eligibility throughout three key periods: at offer submission, contract award, and full contract performance. Vendors must be registered in the System of Award Management (SAM) and may need to provide additional eligibility documentation to Contracting Officers. The form requires specific details including the name of the 51% owner of the IEE firm, business name, federally recognized tribal entity, certifying signature, and Unique Entity Identifier (UEI) number. This form is specifically associated with solicitation SS-25-PHX-006 for proficiency testing services at the Whiteriver Service Unit and Cibecue Health Center.
