Fairfax County Health Department - ID Physician Consultant for Antimicrobial Stewardship
Background
Fairfax County Health Department’s (FCHD) Division of Epidemiology and Population Health has received funding from the Virginia Department of Health (VDH) to implement a program to strengthen antimicrobial stewardship at skilled nursing facilities (SASS) in Fairfax County. The program period is through July 2027, based on availability of funding.
Purpose
The objectives of the SASS program are 1) to strengthen facilities’ stewardship programs, supporting each facility to address gaps and achieve their program goals; and 2) to implement a Quality
Improvement Collaborative to improve assessment, communication, diagnostic testing, and antimicrobial use, initially targeting residents with potential urinary tract infections and then addressing other common infections.
Scope of Work
1. This work may be completed by a single ID physician or by up to 2 ID physicians who each contribute to the program.
2. Apply expert knowledge in medicine, infectious diseases, and antimicrobial stewardship to contribute to the development of processes and materials to improve assessment, communication, diagnostic testing, antimicrobial use, and post-prescriptive review for SNF residents. Disease foci likely will include urinary tract infections, respiratory tract infections, and skin/wound infections.
3. Lead or assist in the development, review, and presentation of educational sessions for physicians and nurse practitioners who prescribe antibiotics to SNF residents.
4. Lead or assist in the development, review, and presentation of educational sessions and training-of-trainers for Directors/Assistant Directors of Nursing and Infection Preventionists regarding resident assessment and diagnostic testing, communications with prescribers, post-prescriptive review, and measurement of antimicrobial use rates and trends.
5. Participate with facility and FCHD program staff in Quality Improvement Collaborative meetings
(virtual every 2-3 months; in-person twice yearly).
6. Provide other expert input and advice to the FCHD program team to strengthen program activities.
Minimum Qualifications
• MD or DO degree with subspecialty of Infectious Diseases (Board Certified).
• Experience in antimicrobial stewardship and working in long-term care facilities.
Time Commitment
• The total expected time to complete the scope of work is an average of 12 hours per week.
• Most activities can be performed virtually. In-person meetings will generally be held at FCHD (10777
Main Street, Fairfax). Travel to participating SNFs generally will not be required but may occur opportunistically (e.g., if the ID physician is going to the facility for another purpose).
• Consultant(s) must provide availability. Most time commitments will be scheduled in advance based on consultant(s) indicated availability. Ad hoc requests may be made with the understanding that the timeliness of response will depend on the physicians’ availability.
Travel
Travel expenses will be reimbursed up to a total of $1,666 per 12-month period in accordance with
Fairfax County Travel Policies and Procedures.
