I. INSTRUCTIONS TO OFFERORS
This solicitation is being prepared in accordance with FAR part 13 Simplified Acquisition Procedures. This solicitation is for commercially services and thus fall under FAR Part 12, Acquisition of Commercial Items. Quotations will be evaluated in accordance with 52.212-2 Evaluation – Commercial Items.
Submit all questions in writing to Kim Yen Tu at kimyen.n.tu@usda.gov. Questions should be submitted no later than three (3) days prior to the response deadline to ensure adequate time is available to respond and to amend the solicitation if necessary. Submit quotes to Kim Yen Tu at kimyen.n.tu@usda.gov by 4/04/2022 at 12am Central time.
In order for an offeror to have a complete quotation package, the following items must be submitted to the Contracting Officer to be considered acceptable:
· A signed copy of the SF 1449 Form. First page only is fine.
· Price schedule. Delivery orders will be issued on an as-needed basis. Pricing should take into consideration shipping costs for positive samples only to NVSL.
· An active Federal Contractors Registration at http://www.sam.gov. Contractors shall include a printed copy of the first page of the SAM listing, or a DUNS number if the SAM application is in progress. The online Certification and Representations for business size determination should use NAICS code 541380. Please note that registering in the SAM database is free of charge. Free SAM registration assistance can be obtained by calling the help desk toll free at 866-606-8220. Free assistance can also be found at your local PTAC office, http://www.aptac-us.org/new/Govt_Contracting/find.php.
· Fill out 52.204-24 (d) representation highlighted portion on page 13 of the Statement of Work.
· A copy of the completed provision 52.212-3, Offeror Representations and Certifications—Commercial Items. Contractors shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at http://www.sam.gov for NAICS code 541380. If an offeror has not completed the annual representations and certifications electronically at the SAM website, the offeror shall include a copy of the completed provision with the quotation.
· Technical Capabilities Statement and Past Performance References. Contractors shall submit up to three customer references from the last 12 months including contact information and/or previous contract numbers with the USDA. Capabilities statement must show that the laboratory meets the approval requirement of the USDA, APHIS (VS Memo 567.2) to conduct official diagnostics tests for ISAV.
· The contractor understands that this solicitation is subject to the Service Contract Labor Standards statute unless contractor is exempt per FAR provision 52.222-53, Exemption from Application of the Service Contract Labor Standards to Contracts for Certain Services-Requirements. Wage Determinations based on the contractors location will be enforced at contract award. Contractors can request wage determinations based on location from the Contracting Specialist before solicitation close date. Wage Determinations can also be found at www.sam.gov
