About this file

This is a Human Trafficking Affidavit required by the City of Lakeland, Florida, in compliance with Section 787.06(14) of the Florida Statutes for nongovernmental entities executing, renewing, or extending contracts with the municipality. The affidavit must be completed and notarized by an authorized officer or representative of the contracting entity and serves as a legal attestation that the nongovernmental entity and its subsidiaries or affiliates do not use coercion for labor or services as defined in Florida law. The document requires the executing party to confirm personal knowledge of the statements, acknowledge the definitions set forth in statute, and commit to immediate notification to the City should any future violations occur. The affidavit includes signature blocks for the authorized company representative and notary public, with space for company name, printed name, title, date, and notary commission expiration.

This affidavit is a mandatory compliance requirement for the Taxiway A Holding Bay construction project at Lakeland Linder International Airport. All bidders and awarded contractors must submit a completed and notarized human trafficking affidavit as part of their bid package to demonstrate compliance with Florida statutory requirements and City of Lakeland policies. The affidavit does not involve pricing, set-asides, or funding considerations but rather serves as a legal certification mechanism to ensure contractor compliance with anti-trafficking statutes and City procurement standards.
