This is a combined synopsis/solicitation for commercial items in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a request for quotes (RFQ) for commercial items in accordance with the procedures of FAR Part 12 – Acquisition of Commercial Items and FAR Part 13 – Simplified Acquisition Procedures, to include FAR Subpart 13.5, Simplified Procedures for Certain Commercial Items. This RFQ incorporates all provisions and clauses in effect through Federal Acquisition Circular (FAC) 05-97 (24 January 2018) and Defense Federal Acquisition Regulation Supplement (DFARS) (4 May 2018).
Background:
The Uniformed Services University of the Health Sciences (USU) is the Nation's federal health sciences university and is committed to excellence in military medicine and public health during peace and war. We provide the Nation with health professionals dedicated to career service in the Department of Defense and the United States Public Health Service and with scientists who serve the common good. We serve the uniformed services and the Nation as an outstanding academic health sciences center with a worldwide perspective for education, research, service, and consultation; we are unique in relating these activities to military medicine, disaster medicine, and military medical readiness. USU is located in Bethesda, MD on the grounds of the Naval Support Activity Bethesda.
Additional information regarding USU can be found on our website: www.usuhs.edu.
Requirement:
USU has a requirement for trained teaching associates to provide initial training in the male genitourinary examination, the female breast and pelvic examination, and the ultrasound exam. These individuals will serve as both the subject of the examination and the instructor as to technique and overall approach. Full details for these teaching associates can be found in the attached Statement of Work (SOW). This will result in multiple five-year Blanket Purchase Agreements (BPA).
BPA Specifications:
This BPA will be used by USU to procure teacher associates for Gynecological Teaching Associate (GTA) and Male Urogenital Teaching Associate (MUTA) A/MUTA initial training for a period of performance of five (5) years. An awardee shall not consider this as a requirements contract. The Government reserves the right to utilize other sources if deemed in our best interest.
When a service need arises, a USU authorized caller will initiate a service call with the awardee directly. Caller will follow the call with an authorization email of the details of the conversation to the vendor. The awardee must not perform services prior to receiving this email from the authorized caller to the awardee. A list of authorized callers will consist of select USU representatives and will be provided to the awardee. Only a Contracting Officer will have the authority to modify the list and will be provided to the awardees on an as-needed basis. Once the awardee agrees to provide the service by scheduling classes, all BPA calls are binding and all terms and conditions of the agreement must be fulfilled.
The total BPA ceiling is $500,000.00.
The dollar limitation for each individual Task Order under the BPA shall not exceed $75,000.
The Government is only obligated for authorized purchases made under the BPA.
Statement of Work (SOW):
This RFQ includes two Statements of Work. Attachment #1 is the Base BPA Statement of Work. This SOW is a representative of all potential tasks that may be ordered under this BPA. Sample Task Order 1 is included as a representation of what could be ordered under this BPA. The Government is under no obligation to award Sample Task Order #1.
RapidGate (Base Access):
Commander, Navy Installations Command (CNIC), has established the Navy Commercial Access Control System (NCACS), a standardized process for granting unescorted access privileges to vendors, contractors, suppliers, and service providers not otherwise entitled to the issuance of a Common Access Card (CAC) who seek access to and can provide justification to enter Navy installations and facilities. Visiting vendors may obtain daily passes directly from the individual Navy installations by submitting identification credentials for verification and undergoing a criminal screening/background check. Alternatively, if the vendor so chooses, it may voluntarily elect to obtain long-term credentials through enrollment, registration, background vetting, screening, issuance of credentials, and electronic validation of credentials at the vendor’s own cost through a designated independent contractor NCACS service provider (RAPIDGate). Credentials will be issued every 5 years and access privileges will be reviewed and renewed on an annual basis. The costs incurred to obtain Navy installation access of any kind are not reimbursable, and the price(s) paid to obtain long term NCACS credentials will not be approved as a direct cost of this contract.
Further information regarding RAPIDGate can be found at: www.rapidgate.com
Questions regarding the RAPIDGate Program should be submitted to info@rapidgate.com with the subject line: RAPIDGate Program
DoD-wide Contractor Manpower Reporting Requirements:
The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for USU via a secure data collection site. The contractor is required to completely fill in all required data fields using the following web address: http://www.ecmra.mil/.
Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. Contractors may direct questions to the help desk at help desk at: http://www.ecmra.mil.”
Evaluation Criteria:
The government intends to award one or more firm-fixed priced (FFP) blanket purchase agreement to the offeror representing the best value to the government based on the evaluation factors listed below. Evaluation factors are listed in order of importance, with most important listed first. The government will conduct a trade-off between non-price and price factors. The Government is not required to award to the lowest priced offeror or the highest technically rated offeror.
The government reserves the right to award without discussions.
Evaluation Factors (listed in descending order of importance):
1. Technical Approach:
Offerors are required to submit their technical approach to meeting the requirements outlined in the Base statement of work (Attachment #1) and Sample Task Order #1 (Attachment #2). Offerors must display a clear understanding of the requirements. Offerors will be evaluated on the examination sequence proposed, as well as the degree to which the offeror guarantees availability to perform on the specified dates, as documented, e.g. by provision to provide back-up teaching associates in the event of a primary teaching associate cancellation or absence. Additionally, offeror’s will be evaluated to the degree to which offeror either accepts or takes exception to elements within the SOW, including exam sequence details, which have been included in the Base SOW.
Quotes submitted by offerors who fail to demonstrate a clear understanding of the requirement may not be considered further.
Quotes submitted by offerors that include the following statement, or similar, without any supporting information, may be determined to not have a clear understanding of the requirement and will not be considered for award:
“The offeror understands the requirements and the submitted quote has met all stated objectives.”
2. Past Performance:
The Government shall evaluate the offeror’s present and past performance on similar tasks completed within the past three years. Offerors must provide a minimum of two (2) but no more than four (4) customer references or CPARS reports for similar work performed. The Government reserves the right to select which reference(s) to contact. The following must be provided for each listed reference:
a. Name of client and address
b. Name of current point of contact including telephone number , e-mail address and title
c. Contract number
d. Contract value
e. Contract period of performance
f. A clear description of the supplies/services provided
USU may request additional information at any time.
Offerors will have an opportunity to address any adverse past performance history. Offerors who do not have any past performance history will not be rated favorably or unfavorably, but will instead receive a Neutral/Unknown rating for past performance. Offerors must clearly state in their quote if they do not have at least two (2) past performance references. Failure to provide this information may render an offeror’s quote non-responsive and may not be considered for award.
A strong history of relevant past performance may be more advantageous to the Government than a less relevant record of favorable performance. Offerors may provide past performance information for subcontractors and must provide the same level of detail as outlined above; however past performance information for the prime will be considered more important to the Government. The Government reserves the right to use data provided by the offeror and data provided by alternative sources in conducting its assessment of past performance history.
3. Price: Offerors shall propose prices for the Base SOW and Sample Task Order #1.
For Sample Task Order #1, offerors must include all applicable costs (Example: transportation, supplies, etc.) in their quote. The total quoted price for all applicable costs will be the evaluated price.
Offerors’ proposed prices shall be considered independently of the technical evaluation criteria and shall not be weighted or scored. Offerors who fail to provide quotes which include supplies, an on-site monitor, and required services will not be considered.
For price analysis purposes, offerors must provide a price breakout of the applicable costs (transportation, supplies, etc.). The resultant award will be a firm fixed price (FFP) blanket purchase agreement.
Additionally, The Offeror shall submit prices on a per student basis for the required work for years 2018, 2019, 2020, 2021, and 2022 for the work contained under the Base BPA SOW.
