UNITED STATES TRANSPORTATION COMMAND

508 SCOTT DRIVE

SCOTT AIR FORCE BASE, ILLINOIS 62225-5357

30 June 2017

MEMORANDUM FOR ALL PROSPECTIVE OFFERORS

FROM: USTRANSCOM/TCAQ-R

508 Scott Drive

Scott AFB, IL 62225-5357

SUBJECT: Request for Quotation (RFQ) HTC711-17-Q-R002, Air Ambulance Services

1. The United States Transportation Command (USTRANSCOM), TCAQ-R, intends to establish Blanket

Purchase Agreements (BPAs) for air ambulance service with a five year period of performance. The aircraft utilized shall be contractor-furnished aircraft, configured to air ambulance standards. The BPAs will be for service from Outside the Continental United States (OCONUS) to the Continental United

States (CONUS) and within CONUS for the movement of Department of Defense (DoD) eligible patients. Special requirements may be offered on a case-by-case basis. In the future, service areas may be expanded to include within OCONUS Geographic Combatant Commands (GCCs). The AORs are defined at the following website: http://www.defense.gov/ucc/.

2. The BPAs will be used to supplement military capability. There is no guarantee any service will be purchased by the Government. A best value decision, explained further in Attachment 3, will be made prior to issuing a call for service.

3. Medical crews must be accredited by the Commission on Accreditation of Medical Transport Systems

(CAMTS) and/or the National Accreditation Alliance of Medical Transport Applications (NAAMTA).

The air carrier must also be DoD approved in accordance with 32 CFR Part 861. Only in the event a carrier with DoD approval is not available, may a CAMTS and/or NAAMTA accredited air carrier without DoD approval be utilized, and then only if needed to save “life, limb, or eyesight”. The DoD

Additional Standards for Contract Passenger Operations under FAR Part 135 apply (see Attachment 3, Paragraph 13).

4. Notwithstanding the requirements in paragraph 3, the following considerations apply:

a. Conditional BPA award notices may be initially sent to offerors who are DoD approved air carriers and have initiated the process to receive CAMTS and/or NAAMTA accreditation. In this case, the offeror will not be eligible for award of any missions until the accreditation standard is attained. If the medical accreditation is not achieved within twelve (12) months of the closing date specified in paragraph

6. below, the conditional award notice will be withdrawn, or

b. Offerors that are either CAMTS and/or NAAMTA accredited air carriers, but are not a DoD approved air carrier will receive a BPA with restrictions. In this case, the offerors are eligible to provide service only if needed to save “life, limb, or eyesight” as stated in paragraph 3. Additionally, upon notice of award the offeror shall immediately pursue DoD approval. However, if you are unable to achieve DoD approval within six (6) months of the closing date specified in paragraph 6. below, the BPA will be cancelled.

Upon receipt of the required credentials (DoD approval plus CAMTS and/or NAAMTA accreditation), the conditional award will be transitioned to a full award. Offerors not having either DoD Approval or a medical accreditation (by CAMTS or NAAMTA) are not eligible for award.

5. Medical staffing and retrofitting of medical equipment shall be commensurate with patient requirements. There are patients in stable condition who may require only one medical professional of at http://www.defense.gov/ucc/ least an Aeromedical Crew Member (ACM) Level-1. Additionally, there are specialized missions requiring ACM Level-4 staffing. A list of definitions is provided in Attachment 3.

6. In order to be included as a validated source under this BPA to provide air ambulance service beginning on 1 October 2017, the following information must be submitted no later than the closing date of the solicitation, 31 July 2017.

a. A price list (Attachment 1). All price lists must be signed, dated, and indicate the company name, address, and date through which the prices are valid. Round-trip missions and point-to-point missions are defined in Attachment 3. Additional potential reimbursable expenses may be added to the price list by an attachment. These additional reimbursable expenses, if applicable to a specific mission, are taken into consideration prior to a best value decision being made. All invoices for reimbursable expenses must be accompanied by paid receipts.

b. A list of aircraft (Attachment 2).

c. Offeror’s completed FAR 52.209-7, Information Regarding Responsibility Matters, and FAR

52.212-3, Alternate I (Attachment 5).

d. The information required in Attachment 3, Paragraph 9, entitled “Landing Permits”.

e. The information required in Attachment 4, FAR 52.237-7, Indemnification and Medical

Liability Insurance.

f. If you are a DoD approved air carrier without CAMTS and/or NAAMTA accreditation, documentation from the accrediting source must be submitted that verifies the process to become accredited has been initiated.

g. If you are a CAMTS and/or NAAMTA accredited air carrier without DoD approval, a DoD

Statement of Intent, AMC Form 207 must be submitted.

7. Attachment 4 includes FAR, Defense Federal Acquisition Regulation Supplement (DFARS), and

USTRANSCOM Federal Acquisition Regulation Supplement (TRANSFARS) clauses that will be incorporated into the resulting BPA.

8. Responses may be provided by mail to USTRANSCOM/TCAQ-R, ATTN: Mary Chapie, 508 Scott
