About this file
This is Standard Form 30 (Amendment of Solicitation/Modification of Contract) issued by the Defense Health Agency on July 1, 2026, in response to questions received regarding solicitation HT941026QE013. The amendment addresses six questions and answers clarifying requirements for phlebotomy personnel under Performance Work Statement (PWS) section 5.1.3.
The key clarifications include: (1) candidates holding Korean university degrees in Biomedical Laboratory Science/Clinical Pathology, Korean Ministry of Health and Welfare Medical Technologist Certificates, and documented Core Lab/Phlebotomy work experience are acceptable for education requirements, despite the PWS requiring 120 didactic hours of phlebotomy training; (2) ASCP MLS certification alone is not acceptable for licensure/certification requirements—only American Heart Association BLS (Course C) or American Red Cross CPR/BLS certification meets the requirement; (3) experience documentation may consist of a candidate's curriculum vitae, employment documentation describing Core Lab/Phlebotomy duties and medical information system experience, plus verification contact information, without requiring government-prescribed forms; (4) BLS certification must be current at the time of quote submission; (5) the incumbent contractor for this requirement is W81K02-21-P-0173; and (6) the quote due date will not be extended from July 9, 2026. The amendment is not extended beyond the original deadline, and offerors must acknowledge receipt of this amendment prior to offer submission.

QUESTIONS AND ANSWERS
Question 1: Education Requirement - PWS 5.1.3.1 PWS paragraph 5.1.3.1 requires graduation from an academic course of phlebotomy that includes 120 didactic hours. In the Republic of Korea, phlebotomy training is generally incorporated into Biomedical Laboratory Science / Clinical Pathology education, hospital on-site training, and clinical practice, rather than offered as a separate stand-alone phlebotomy course. Would a candidate who holds a Korean university degree in Biomedical Laboratory Science / Clinical Pathology, a Korean Ministry of Health and Welfare Medical Technologist Certificate, and documented Core Lab/Phlebotomy work experience be considered acceptable for the education requirement, or is a separate stand-alone phlebotomy course certificate specifically required?
Answer 1: Yes, a candidate who holds a Korean university degree in Biomedical Laboratory Science / Clinical Pathology, a Korean Ministry of Health and Welfare Medical Technologist Certificate, and documented Core Lab/Phlebotomy work experience is considered acceptable for the education requirement.
Question 2: Licensure / Certification / Registration - PWS 5.1.3.2 Please clarify whether a candidate holding an active ASCP MLS certification and a Korean Ministry of Health and Welfare Medical Technologist Certificate would be considered acceptable under the licensure/certification/registration requirement in PWS paragraph 5.1.3.2.
Answer 2: No, an ASCP MLS certification is not considered acceptable for this requirement. Per PWS paragraph 5.1.3.2, The contract personnel shall have completed a phlebotomy course that includes 120 didactic hours. Contract personnel shall have and maintain current certification in either the American Heart Association Basic Life Support (BLS) (Course C) or the American Red Cross CPR/BLS Course.
Question 3: Experience Documentation - PWS 5.1.3.3 Please confirm what type of documentation is acceptable to demonstrate the experience requirements in PWS paragraph 5.1.3.3. Clinical laboratory technologists at Korean hospitals perform hundreds of successful venipunctures using a straight and a butterfly needle and hundreds of successful skin punctures every day.
Would a candidate's CV, together with employment documentation describing Core Lab/Phlebotomy duties and medical information system experience, be sufficient, or is a separate Government-prescribed verification form required?
Answer 3: Yes, a candidate's CV, together with employment documentation describing Core Lab/Phlebotomy duties and medical information system experience is sufficient, along with a point of contact for verification. As stated in PWS paragraph 5.1.3.3:
The offeror shall provide evidence of prior experience in the following format:
The title and number of the contract/project Description of work completed Length of contract/project (year/month/date - year/month/date) Complete point of contact at Client Organization (including name, phone number, email address)
Question 4: Basic Life Support Certification - PWS 5.1.3.2 and 5.2.5 Please confirm whether the BLS certification must be current at the time of quote submission, or whether it may be obtained prior to commencement of performance.
Answer 4: BLS certification must be current at the time of quote submission.
Question 5: Incumbent Contract Is there a current incumbent contractor for this requirement? If so, please provide the incumbent contract number.
Answer 5: Yes, the current contract number is W81K02-21-P-0173.
Question 6: Due Date Extension Would the Government consider extending the quote due date from July 9, 2026 to July 16, 2026?
Answer 6: No, the quote due date will not be extended.
