Notice of Intent to Sole Source HT941026N0122
THIS IS A NOTICE OF INTENT TO AWARD A SOLE SOURCE SUPPLY CONTRACT
AND IS NOT A REQUEST FOR COMPETITIVE QUOTES.
The Defense Health Agency Contracting Activity (DHACA), Healthcare Contracting Division – West (HCD-W), intends to award a firm-fixed-price purchase order for Manufacturer: Melling Medical, LLC (Lumenis Digital Trio 2.0 All in One Laser System) to support an emergency requirement for the operations of 673rd Medical Group, JB Elmendorf AFB, Alaska, in accordance with Federal Acquisition Regulation (FAR) 12.102(a) under the Revolutionary FAR Overhaul (RFO).
Melling Medical, LLC 658 South Columbus St Alexandria, VA 22314
UEI: CA7CDKQND7U9
CAGE: 6XD62
The North American Industry Classification System (NAICS) code for this requirement is 334510 (Electromedical and Electrotherapuetic Apparatus Manufacturing), with a small business size standard of 1250 employees. The associated Product Service Code (PSC) is 6540 (Ophthalmic Instruments, Equipment and Supplies).
The 673rd Medical Group is requesting an emergency sole-source procurement of new Lumenis Digital Trio 2.0 laser from Melling Medical, LLC. The 673rd Medical Group has an emergency operational requirement to restore full ophthalmic laser treatment capability. Delay caused by competitive procedures would result in continued interruption of critical patient care (glaucoma, secondary cataract, and retinal procedures) at this remote Alaska location, adversely affecting mission readiness and beneficiary health.
Melling Medical, LLC is an authorized distributor of Lumenis systems and is the only source capable of delivering the required system, professional installation, and on-site training within the urgent timeframe needed by the 673rd Medical Group. Consequently, this acquisition of commercial products and services is being executed on a sole-source basis in accordance with Federal Acquisition Regulation (FAR) 12.102(a) and RFO 19.106-3(a)(1), as the contracting officer does not have a reasonable expectation that offers will be received from two or more Service-Disabled Veteran-Owned Small Business (SDVOSB) concerns.
Delivery Date: 90 Days ARO
Delivery Location:
5955 Zeamer Ave Ophthalmology Department
JBER, AK 99506
Melling Medical, LLC is the sole source capable of meeting the Government’s minimum requirements to provide the requested laser and certified training at JBER, AK.
This notice of intent is not a request for competitive proposals, and no formal solicitation document exists for this requirement. Sources interested in responding to this notice are required to submit a capability statement. Capability statements shall not exceed six (6) pages (8.5" x 11" format) using a font size no smaller than 10-point. All capability statements received by the due date of this notice will be considered by the Government.
Requests for documentation, inquiries for additional information, or submissions consisting solely of questions will not be considered affirmative responses. A determination by the Government not to compete this requirement based on responses to this notice is solely within the discretion of the Government.
