1 RFQ 377MDG What all do I need to fill out? Follow the instructions in 52.212-1 and Addendum to 52.212-1, Instructions to Offerors - Commercial Products & Commercial Services.
To view the full text of FAR and DFARS provisions and clauses, go to https://www.acquisition.gov/browse/index/far and https://www.acq.osd.mil/dpap/dars/dfarspgi/current/index.html, as identified in FAR provisions 52.252-1 and 52.252-2 contained in the solicitation.
For page-by-page hands-on help preparing your quote, contact APEX Accelerators (https://www.sba.gov/local-assistance/federal-contracting-assistance) or the Small Business Development Center (https://www.sba.gov/local-assistance/resource-partners/small-business-development-centers-sbdc) to schedule an appointment.
2 Performance Work Statement
Page 7 As the contractor in SAM, does my company need laundry work experience? As indicated by Questions 7 and 8 in Attachment 3, Technical Capability Form, and the evaluation method described in 52.212-2, Evaluation - Commercial Products & Commercial Services, the Government will consider experience during the evaluation of technical capability. If your company does not have experience performing medical laundry services like those required by the PWS, you are encouraged to use Question 11 to describe your company's capability to meet the PWS requirements in sufficient detail to enable the Government to determine if your company is technically
3 Technical Capability form
Page2 question 7-9 Can I list the place where the laundry will be cleaned at? All questions on Attachment 3, Technical Capability Form, must be answered.
Question 9 specifically requires vendors to "identify the physical address of the facility you will use to provide the required laundry services ." As indicated in paragraph (c) of 52.212-2, the Government reserves the right to conduct an onsite visit of the facilities proposed to assess adequacy.
4 SF1449, Technical Form, Pricing Schedule also the RFQ
Everthing that is required What else is required besides the 3 forms and RFQ? Per paragraphs (b)(1) through (b)(5) of the Addendum to 52.212-1, the signed and dated quote must include the SF1449, Technical Capability Form, Pricing Schedule, and Representations & Certifications.
5 What is deadline for all quotes to be submitted? See Block 8 on page 1 of the Standard Form (SF) 1449, titled Offer Due Date/Local Time. FAR 52.212-1 paragraph (b) has been updated to specify that "local time" is Pacific Time.
6 Do we have to right to call or was this form used for questions only To ensure all potential vendors have the same information on which to base their quote, all questions are submitted via email using the Question Submission Form and the Questions & Answers are posted at SAM.gov via an amendment.
7 What does pre-soliciation mean? Your question appears to be asking about the presoliciation notice. The purpose of the presolicitation notice is to advise suppliers on the general scope and purpose of an upcoming soliciation, to stimulate interest of prospective vendors that may be intereted in and qualified to perform the requirement. The presolicitation notice does not contain sufficient information for quote submissions, therefore, any responses to the presolicitation notice are considering informational only.
8 SF1449, Technical Form, Pricing Schedule
What does it mean to submit 3 electronic files? The Addendum to 52.212-1 requirement to "submit the quote in three separate electronic files, as follows: i. SF1449 and Representations & Certifications; ii.
Technical Capability Form; iii. Pricing Schedule " means that each of these quote submission requirements must be a separate file. The SF1449 and Representations & Certification must be one file (typically a PDF). The Technical Capability Form must be one file, the completed RFQ Attachment 3. The Pricing Schedule must be one file, the completed RFQ Attachment 4.
