STATEMENT OF WORK

A. GENERAL REQUIREMENTS:

Contractor shall provide a clean and furnished one-bedroom apartment to the nursing students of USUHS Graduate School of Nursing (GSN), under a leasing agreement between the contractor and USUHS. The apartment will be a place of residence to serve as habitable temporary quarters while the students are on a temporary duty assignment to perform an official clinical rotation at Forrest Glen Hospital.

Lease Agreement: There shall be a leasing agreement provided by the Contractor specifying the terms of agreement between the contractor, USUHS and the occupant(s) of the apartment.

Additionally, the leasing agreement shall be created to allow for reasonable and practical written modifications, if needed, by USUHS. The agreement shall clearly specify the role and responsibilities of all parties involved.

Location: The apartment shall be within ten (10) miles of Forrest General Hospital located at 6051 US Highway 49, Hattiesburg, Mississippi 39402.

Performance: The Contractor shall be responsible for making the apartment available to the nursing student(s) of USUHS throughout the period of performance. The requirements of the contract shall be in agreement with the Contractor in such a manner that the Contractor will not disrupt the activities or operations of USUHS.

It is anticipated that not more than two students will occupy the apartment for 8-weeks or more at a time, however there may be times when the apartment is vacant. During these times the Contractor shall not lease the apartment without obtaining written consent from those representing the contract for USUHS.

B. CONTRACTOR RESPONSIBILITES:

1. Repairs and service of resident requested in writing by occupants

2. Utilities cost for water, sewage, garbage, pest control and electricity

3. Keep common areas clean

4. Maintain fixtures, furniture, hot water, heating and/or air conditioning equipment

5. Comply with federal, state, and local laws regarding safety and sanitation

6. Comply with leasing terms and conditions of the lease agreement and resulting purchase order

7. Facility must meet the standards required by the following laws and regulations:

i. Americans with Disabilities Act

ii. Hotel Fire & Safety Act of 1990

C. MINIMUM SPECIFICATIONS:

1. One(1) bedroom which must be furnished (at a minimum) with the following:

i. Two (2) twin beds (or larger) with mattress and box springs

ii. Three-drawer chest or dresser and closet

iii. Night stand/table with lamp

iv. Blinds/shades/drapes

2. The living/dining room(s) must be furnished (at a minimum) as follows:

v. Table and chair

vi. Sofa

vii. Adequate lighting

viii. Television with cable

ix. Internet Service

3. The bathroom must contain (at a minimum) the following:

x. Shower or bathtub

xi. Shower curtain or shower door

xii. Toilet

4. The kitchen must contain (at a minimum) the following:

xiii. Dishwasher

xiv. Refrigerator

xv. Microwave

xvi. Dishes and utensils

xvii. Pots/Pans and Coffeemaker

5. Other required items (at a minimum):

xviii. Washer and dryer on premises

xix. One (1) assigned parking space plus guest parking

D. INVOICE AND PAYMENT:

The Contractor must invoice on a monthly basis. The monthly rate and additional charges will be agreed upon and noted in the leasing terms and agreement by the Contractor and USUHS.

Base Year of Contract: 24 April 2012 through 23 April 2013

E. SPECIAL INSTRUCTIONS:

The occupant(s) of the apartment will be responsible for following facility rules and will be liable for any damages or loss of facility property, i.e. lost keys. The Government will not be responsible or liable for personal actions of individual occupants. The occupants of the apartment will be responsible for cleaning the unit.

F. CONTRACTOR SUPERVISION:

The contractor will coordinate will coordinate with the Dean and Primary Contract Officer Representative of the Graduate of Nursing, on services outlined in the agreement provided.
