About this file
This is a Price Appendix Frequently Asked Questions document that provides instructions for completing pricing spreadsheets across three distinct pricing models for pharmaceutical submissions.
The document addresses three pricing methodologies. For BPA submissions, vendors must complete yellow cells within the spreadsheet while all other cells remain locked. Vendors enter the Package Size (column G) for each National Drug Code (NDC), which auto-populates remaining cells. Price per NDC (column F) is required; submitting no price will trigger a red highlight indicating non-participation in that Formulary Category. Price per Unit (column H) auto-calculates using the formula: Price per NDC divided by Package Size. The spreadsheet accommodates ten NDCs, with vendors completing only tables for their submitted NDCs. For additional NDCs beyond ten, vendors must contact the Points of Contact listed on the RFQ document. For ADP non-FAMP Discount submissions, vendors input the first non-FAMP price (column F) and Current Annual Federal Ceiling Price (column H), which auto-populate across subsequent rows. A 0.0000% Additional offered refund (column G) triggers a red highlight indicating non-participation. Total Calculated Refund (columns I and J) auto-calculates using: ((non-FAMP-FCP) + (non-FAMP×Y%)). For ADP WAC Discount submissions, applicable to products without FCP such as diabetic supplies, vendors input WAC price (column F) with auto-population across rows. A 0.0000% WAC Discount (column G) highlights red to indicate non-participation. Total Calculated Retail Refund per Package (column H) auto-calculates. All three sections require vendors to review Formulary Notes at the spreadsheet top to understand P&T Committee decision-making processes and permit contact with POCs for questions or concerns regarding table completion.
