About this file
This document appears to be a Frequently Asked Questions (FAQ) guide related to the price list appendix for a Department of Defense (DoD) Uniform Formulary Blanket Purchase Agreement (UF BPA) and Uniform Formulary Additional Discount Program (UF ADP) solicitation.
The FAQ provides instructions for completing the price list spreadsheet, including information on formulary notes, entering package sizes and prices, highlighting cells for non-submitted quotes, and auto-calculating certain fields. It also outlines the process for submitting more than 10 NDCs, and provides contact information for questions.
The related federal contract opportunity is a solicitation (HT9402-24-Q-9401) for the DoD Pharmacy Uniform Formulary BPA and ADP. The solicitation covers pharmaceutical agents in several drug classes, including migraine agents and targeted immunomodulatory biologics. Responses to clinical questions are due by a specified date, and a pre-quote teleconference is scheduled. The Defense Health Agency is the issuing agency for this opportunity.
