About this file
This document appears to be a Price Appendix FAQ related to a Request for Quotation (RFQ) for the Department of Defense (DoD) Pharmacy Uniform Formulary Blanket Purchase Agreement/Uniform Formulary Additional Discount Program.
The RFQ is seeking quotes from industry for pharmaceutical agents within drug classes identified for placement on the DoD Uniform Formulary. The Price Appendix FAQ provides detailed instructions on how to fill out the pricing tables for the BPA and ADP programs, including requirements to enter package size, non-FAMP prices, WAC discounts, and auto-calculated fields. The RFQ is for the May 2024 P&T Committee meeting, where two newly approved drugs will be reviewed - Eohilia and Zymfentra. The RFQ includes important deadlines, a pre-quotation teleconference, and contact information for the RFQ Point of Contact. Responding companies are instructed to submit clinical questions by the stated deadline.
