About this file
This notice of availability (NoA) solicits solutions for the Defense Health Agency's Fiscal Year 2018 National Defense Authorization Act Section 731 Patient Navigator Pilot Program. Solutions are sought to develop and demonstrate a health care assistance system that connects beneficiaries with patient navigators to help coordinate care, reduce administrative burdens, and improve health outcomes. The NoA describes a two-phase evaluation process to select an awardee for a firm-fixed-price or fixed-price incentive contract. Phase 1 involves submitting a draft performance work statement, value-based incentive structure, price proposal, past performance, and pitch materials by July 26, 2019. Phase 2 consists of negotiations with the selected applicant to refine the solution and finalize contract terms. The selected applicant must address all objectives in the statement of work, propose fair and reasonable pricing within available funds, and demonstrate satisfactory past performance.

Innovative is defined as any technology, process or method that is new as of the date of submission of a proposal. It also includes any new application of an existing technology, process, or method.

Funds are not presently available for this acquisition. No contract award will be made until appropriated funds are made available.
Organizational Conflicts of Interest (OCI)
The applicant is hereby notified that DHA has contractual relationships with multiple companies that due to the nature of the performance with DHA, may create an actual or potential organizational conflict of interest, which must be avoided, neutralized or mitigated. It is the responsibility of each applicant to identify if any such actual or potential organizational conflict of interest exist with regard to their solution (see FAR 9.5, Organizational and Consultant Conflicts of Interest).
As a part of the solution, the applicant shall provide the Contracting Officer with information of previous or ongoing work that is in any way associated with this NoA. In the event an actual or potential organizational conflict of interest exists, the applicant shall, in addition to the solution, submit a plan to the Contracting Officer (CO), demonstrating how the applicant will avoid, mitigate or neutralize any actual or potential organizational conflict of interest while supporting this contract and any other DHA contract.
The solution selected to move to Phase 2 must not present an actual or potential organizational conflict of interest, or it must be accompanied by a plan that the CO determines effectively avoids, neutralizes or mitigates a potential or actual organizational conflict of interest.
The above restrictions shall be included in all subcontracts, teaming arrangements and other agreements calling for performance of work which is subject to the organizational conflict of interest restrictions identified in these provisions.
The above provisions may be modified or deleted at the discretion of the Government. The Government reserves the right, in case of a breach, misrepresentation or nondisclosure, to terminate the resultant contract, disqualify the applicant from subsequent related contractual efforts, or pursues any remedy permitted by law, regulation or the terms and conditions of this solicitation.

Solution Evaluation
Solutions received by the submission deadline will be evaluated in accordance with the evaluation and selection process listed below. All solutions are treated as procurement sensitive and are disclosed only for the purpose of evaluation. Award will be made to the applicant proposing the best solution to address the objectives outlined in the SOO for a fair and reasonable price and within available funds. A two phase process will be used to select a single applicant for award.
Applicants who submit a solution will receive an invitation to make a live pitch (i.e. oral presentation) of their solution to the DHA team of evaluators during a Pitch Day event to be held on or about the week of 5 August 2019 in San Antonio, TX. A live pitch is not mandatory but is encouraged. All applicants shall submit Pitch Day slides, whether or not the pitch is presented live. Pitch Day details will be provided via e-mail invitation the week following receipt of solutions.
The pitch shall not merely repeat the Statement of Objectives or contain the contents of the PWS pasted into slides, but rather provide convincing evidence the proposed solution will meet the Government’s objectives.
The following examples of convincing evidence are encouraged –
(a) Authentic company URL or web address.
Note: The Government may elect to use the information provided as part of its continuous market research. However, the government is not obligated to use the URL or web address as part of its evaluation process to determine the Selectee or Awardee.
(b) Summary of services currently provided in the open market and how the current solution differs.
(c) Diagrams, graphs, or charts to depict the essence of the proposed solution.
The Government may ask questions/interview applicants during the live pitch. No communication is anticipated with applicants following Pitch Day prior to notification of selection for participation in Phase 2 or exclusion from consideration.
Phase 1 Evaluation
Phase 1 submissions will be evaluated as follows:
ELEMENT 1 - A panel of Subject Matter Experts (SMEs) will assess the draft Performance Work Statement (PWS) and Pitch Day slide deck. The draft in conjunction with the Pitch Day slide deck shall address all objectives in the SOO in a manner that allows the evaluators to fully understand the approach and intended outcomes, but does not require a high degree of detail. The assessment will take place to determine:
SUB-ELEMENT A - Importance to Agency Programs demonstrated through the potential contributions of the solution(s) to improve patient outcomes, patient experience, and reduce costs
SUB-ELEMENT B - The technical merits of the proposed solution
ELEMENT 2 - The proposed value-based incentive structure will be evaluated to determine how effective the proposed incentives will be in motivating the contractor to achieve the pilot objectives.
ELEMENT 3 - Cost/pricing specialists and the Contracting Officer will evaluate the draft price Solution and total price to determine if it is both:
SUB-ELEMENT A - Fair and reasonable
The Government will evaluate applicant’s proposed prices by adding the total price for all incentives and options to the total price for the basic requirement. Evaluation of options does not obligate the Government to exercise the option(s).
Price will not be scored or ranked; price will be evaluated to determine it is fair and reasonable. Price will be evaluated in accordance with DFARS 212.209. Applicants are required to submit other than cost and pricing data to support a fair and reasonable price determination, to include all price information required by the Notice of Availability (NoA). Because price will be evaluated for reasonableness before Phase 2, it is possible an applicant may not be selected because of an unreasonable price. The price submitted in Phase 1 is binding as a ceiling price in Phase 2 and in a subsequent award. Applicants should submit their best price with their original Solution submission.
SUB-ELEMENT B - Within available funds
Each applicant’s total evaluated price, to include all potential incentives and option periods, will be compared to available funds. If the proposed price exceeds available funds it is unawardable and may not be selected for Phase 2.
ELEMENT 4 - The past performance team and/or Contracting Officer will assess the relevant past performance summary, may interview references and may use all other means available to fully vet the past performance history of the applicant. Applicants must have a satisfactory or better past performance record to be considered for Phase 2. This element is pass/fail.
Phase 1 Selection Criteria
With the exception of the pass/fail Past Performance element, elements are listed in order of importance. The solution selection will be based on the individual merits of an applicant’s proposed solution. The applicant whose submittal best satisfies all objectives of the SOO in consideration of the SOO constraints evaluated under the evaluation elements will be selected for negotiation at Phase 2.
Applicants not selected for Phase 2 will be notified of their exclusion from consideration and provided feedback on their submittal, which will contain the basis for exclusion. In the event negotiations with the selected applicant are unsuccessful, the results of the evaluations of the applications received for Phase 1 will be reviewed to select another applicant for negotiations in Phase 2.
Phase 2 Negotiation & Award
The Government will conduct a solution feedback session with the selected applicant after notice of selection. The solution feedback session will address any areas of refinement the Government recommends for the solution approach. The selected applicant will be required to submit the following within 30 days of the session:
ELEMENT 1 - Complete Performance Work Statement (PWS), ELEMENT 2 - Finalized value based incentive structure linking payment to performance, cost savings, beneficiary satisfaction, and improved health outcomes, and
ELEMENT 3 - Final price proposal not to exceed the maximum total price proposed at Phase 1.
All elements are available for negotiation at this juncture. In no circumstance will the maximum price proposed at Phase 1 for Element 3 be exceeded. The Government will present all other terms and conditions including mandatory terms and conditions associated with FAR Part 12 commercial contracts. The selected applicant must be considered a responsible party prior to award IAW FAR 9.104
