About this file

This document is a Limited-Sources Justification for the Defense Logistics Agency (DLA) Troop Support, Medical Supplier Operations, Electronic Catalog (ECAT) Division. The justification is for acquiring 7 Olympus BF-UC190F Endobronchial Ultrasound (EBUS) Bronchoscopes with Ultrasound Cable for Walter Reed National Military Medical Center's Pulmonary Service, with a total quantity of 7 units at part number BF-UC190F-A.

The procurement is sole-sourced to Olympus due to unique product features deemed essential for government requirements, including a shorter 6.6mm distal tip, 160-degree flexion capability, a 2.2mm working channel, and superior needle orientation. The document emphasizes that Olympus is the only provider meeting specific medical equipment needs, with features not available from competitors, such as accommodating a 19g needle for histological samples and providing optimal maneuverability for lung biopsies. The acquisition will be conducted under the ECAT Multiple Award Schedule Program, with DLA Troop Support seeking to obtain competitive pricing through a Request for Quotation (RFQ) to medical/surgical contract holders.

Text version

This request is for the replacement of a limited portion of bronchoscopes that are currently in use in the Pulmonary Service at Walter Reed National Military Medical Center in Bethesda, MD.

Olympus provides the superior endoscopes compared to other endoscopes used in DoD because Olympus endoscopes are the only non-disposable endoscopes used in GI and Pulmonary throughout the DoD. The EBUS bronchoscope is a specific type of bronchoscope used to provide standard care services to patients in the diagnosis of many diseases with abnormalities in the lungs and is essential for appropriate staging and diagnosis in deadly diseases such as lung cancer. The absence of functional EBUS bronchoscopes would result in immediate decrement to patient care.

4. This action is being conducted under the authority of the ECAT Multiple Award Schedule Program. The circumstances justifying waiving the competition requirements is FAR 8.405- 6(a)(1)(i)(B) – Only one source is capable of providing the supplies required at the level of quality required because the supplies are unique or highly specialized, and FAR 8.405-6(b)(1) – Brand name specifications shall not be used unless the particular brand name, product, or feature is essential to the Government’s requirements, and market research indicates other companies’ similar products, or products lacking the particular feature, do not meet, or cannot be modified to meet, the agency’s needs. As explained below, this particular brand name product is essential to the Government’s requirements, and market research has established that other companies’ similar products cannot meet the Government’s needs. Minimum requirements for governmental use include: Flexible reusable endobronchial ultrasound bronchoscope with a minimum 2.2 mm working channel, color doppler function, compatible with existing endoscope processors and cleaning equipment.

Olympus is the market leader for Endoscopy equipment. Olympus EBUS bronchoscopes include features such as a shorter distal tip, thinner diameter (6.6mm – smallest available when compared with competitors) and unique flexion capability to 160 degrees (other brands limited to 120/130 degrees). These unique features provide improved maneuverability allowing for optimal angulation, an improved approach to difficult-to-reach target sampling sites and expanding the areas of the lung that can be biopsied with the scope. Decreased forward oblique angle of 20° facilitates easier endoscopic orientation and allows the operator to keep the endoscopic image in full field of view while navigating the airways during a procedure.

Superior needle orientation: Steeper puncture angle provides a more perpendicular needle orientation towards the target allowing for smoother target penetration between cartilage rings and other critical vessels. Largest working channel (2.2 mm) which allows for a wider range of biopsy tools and better suctioning for clearing blood and mucus from the airway and is the only EBUS bronchoscope that can accommodate the 19g needle which is essential for diagnosis requiring histological samples. The government requires a minimum of 2.2 mm working channel and Olympus competitors can only provide a maximum of 2.0 mm, meaning Olympus provides the only acceptable working channel size for government usage.

Olympus provides superior bronchoscopy equipment and is preferred for its advanced features and a comprehensive list of bronchoscope models, not just EBUS bronchoscopes. All these
