SECTION SF 1449 - CONTINUATION SHEET

SOLICITATION/CONTRACT FORM

The required response date/time has changed from 16-Jan-2015 04:00 PM to 20-Jan-2015 10:00 AM.

The solicitation issue date has changed from 24-Dec-2014 to 09-Jan-2015.

The following have been modified:

ADDENDUM TO 52.212-1

FAR 52.212-1 is hereby amended to reflect the changes shown below as to the specific paragraph revised.

Para (b) Submission of Offers. The following amends this paragraph with respect to the information and documents required for submission in response to this solicitation and requirements for oral presentation of those proposals determined to be in the competitive range.

(1) ELECTRONIC OFFERS. Offeror’s shall only submit electronic offers in response to this solicitation. The offer must arrive by the time specified in the solicitation to the following email address:

(2) Offeror’s must submit email to verify receipt of the offer, if receipt is not received within 24 hours of proposal submittal. The Government will not be responsible for any failure of transmission or receipt of the offer, or any failure of the Offeror to verify receipt of the emailed offer.

A SITE VISIT will be held at 10:00 A.M. EST on , Thursday 8 Jan 2015; offerors interested in attending the site visit should provide POC information that the WRNMMC COR will need for obtaining badges required for base entry by 1:00 P.M. EST on Monday, 5 Jan 2015) to John Wohlever john.k.wohlever.civ@mail.mil and Victor Purcell, victor.k.purcell.ctr@mail.mil . Questions pertaining to this requirement must be received electronically by 12:00 P.M EST Tuesday, Jan 13 2015.

(3) The Government intends to award a firm fixed-price non-personal services contract under the authority of 10 USC Section 2304 and 41 U.S.C. 253, and the implementing provisions of DFAR 237.104(b) (ii). Offerors must quote a firm-fixed price for each Contract Line Item.

(4) Any questions with respect to the solicitation shall be submitted via e-mail in the attached Q&A format to: victor.k.purcell.ctr@mail.mil and martin.rios15.civ@mail.mil and must arrive at this email address, by not later than 12:00 PM EST Monday, 13 Jan 2015. Telephone inquiries will not be accepted. All questions submitted; and, the Government’s answers thereto, will be posted as a supplemental notice in FedBizOpps. The Government may amend the solicitation if it so chooses.

(5) PROCESS FOR AWARD. Award will be made on a Lowest Price Technically Acceptable basis. Award will be made to the Offeror provided its technical proposal conforms to the requirements of the solicitation; it has an acceptable record of past performance; its price is determined to be fair and reasonable by the contracting officer; and the contracting officer determines the Offeror is responsible.

The Contracting Officer intends to award without discussion, but reserves the right to do so in his or her sole discretion.

(6). The Government may reserve the right to reject a proposal if the Offeror misrepresents or withholds past performance information or information regarding its certifications or business condition, or its capability to perform the requirements of this contract.

(7). BASIS FOR AWARD. The award decision will be based on the evaluation of all factors set forth in this solicitation. Award will be made to the responsible Offeror whose proposal conforms to the solicitation requirements and represents the lowest price technically acceptable to the Government.

(8) CONTENT OF OFFERS. The Solicitation response should be completed, signed and received at this office not later than the date/time specified in Block 8 of the solicitation (Standard Form [SF] 1449). The information as provided by the Offeror shall be the basis for the Government’s evaluation of the Offeror’s Technical Response to the Solicitation. Please note that to be deemed responsive, your offer must be constructed in the manner stated below, and include all of the following requested information:

Proposal Formats and Copies:

SIZE: The Offeror’s Proposal documentation shall not exceed the equivalent of forty (40) 8.5” x 11.0” single-spaced type written pages (with all diagrams, charts and/or figures contained therein). Any other pertinent information that the Offeror may feel helps explain his technical approach or technical description of his proposal shall be contained within this presentation space as well.

COPIES: Offerors shall provide two separate electronic copies of their proposals. Copy One (1), Not-Sanitized; Copy Two (2), Sanitized. Copy two shall not contain the name of or reference to the Offeror, any Joint Venture Partner, or any Subsidiaries. This copy will be used in the Evaluations. Offerors are advised hereby that without a Sanitized copy of their Proposal their submission must be deemed as non-responsive, as it cannot be evaluated.

FORMAT. Submit your proposal in three (3) separate parts:

FACTOR I: TECHNICAL

Sub-factor 1: STAFF COMPENSATION AND RETENTION PLAN. The Offeror shall describe his proposed plans, processes, procedures and system(s), and all of his supporting data & documentation in which he describes his technical response to the technical requirement posed in the Government’s RFP. The Offeror’s Technical Proposal shall be contain a Staff Compensation & Retention Plan, and Appendix if necessary. The Compensation and Retention Plans must address the following:

The Offerors’ in their Staff Compensation and Retention Plan shall describe their planning assumptions, key decisions, and the strategy they propose that will both initially staff; and, throughout the life of the contract retain, the quantity and quality of workforce as is called for by the requirements of the PWS and required by the Scope of the Work, that the Government requires, as the Offeror understands it.

The Offeror in their Staff Compensation and Retention Plan shall also complete a Compensation Plan Spreadsheet (constructed according to the Excel-Spreadsheet template which is provided with this Solicitation), and insert their complete spreadsheet, in this chapter of their response to Solicitation. The spreadsheet should be constructed to identify the Offeror’s proposed staff compensation plan for the base year; and all option years of the contract (limit of 10 pages).

Sub-factor 2: QUALIFICATION OF PERSONNEL. Résumés of contractor personnel proposed to be assigned must be submitted with the proposal to ensure that the key position qualifications comply with paragraph 3.1.3 and 3.1.9 of Attachment #1 of the PWS.

FACTOR II: PAST PERFORMANCE. Offeror’s are required to submit information, on a single page, identifying federal, state and local government and private contracts performed within the past three years, which are similar in scope, type of work, and complexity to the effort described in this solicitation. If none, Offeror should so state and provide information on other references. The Information provided by the contractor in his Part II Past Performance response shall state:

a.) The type of service provided, (should be similar to services being requested) b.) Name, telephone and fax numbers of a point of contact in the administering contracting office;

c.) Award/contract number, the term of the contract, the total dollar amount of the contract.

The government will contact the Offeror’s sources as part of its past performance assessment. The government is not responsible for the failure of any source to respond to a request for a past performance assessment on a timely basis.

All Offeror’s must submit a copy of the PAST PERFORMANCE QUESTIONNAIRE (contained in this Solicitation) to each individual enterprise that they list in their Part II Past Performance response to this solicitation. The questionnaire should be emailed or faxed directly to the individual enterprise. Offeror’s shall state to each enterprise that their responses should be emailed to: victor.k.purcell.ctr@mail.mil and martin.rios15.civ@mail.mil. The Offeror should instruct their past performance POCs not to send the completed copy back to the Offeror, but to the POC listed on the questionnaire.

FACTOR III: PRICE. Pricing data shall be filled in on the Schedule on the Government’s RFP document as it is requested be stated therein and as described to be done in the PWS. If additional space is needed for a description of price breakout it should be included as part of the Technical Compensation Plan Breakout which is described above.

The price data provided in the schedule will be used to assist in the price analysis that is described in EVALUATION OF THE PROPOSED PRICES, in ADDENDUM TO FAR 52.212-2, EVALUATION – COMMERCIAL ITEMS OF THIS RFP. In the preparation of this documentation the Offeror is requested to observe the following:

(f) Price Schedule. DO NOT round figures in the pricing schedule.

(g) Complete and sign SF 1449, page 1, Blocks 17a & b, 30a, b & c.

(h) Signed amendments (if any).

(i) Representations and Certifications found at FAR 52.212-3 Alt 1 and 52.209-7 MUST be completed.

Para (g) Contract award. This paragraph is amended to state that the Government may conduct discussions prior to contract award.

Para (m) Proposal Preparation Costs. This Solicitation does not commit the Government to pay any costs incurred in the submission of a RFP, or in making any necessary studies for the preparation thereof, or for any visit(s) the Contracting Officer may request for the purpose of clarification of the offer.

Para (n) The non-FAR Part 12 discretionary FAR, DFARS, AFARS, and LOCAL provisions included herein are incorporated into this solicitation either by reference or in full text. If incorporated by reference, see provision 52.252-1 herein for locations where full text can be found.
