1.2 Background Overview:

The scope of Department of Defense Medicine and the Military Health System (MHS) which includes Navy Medicine, Army Medicine, Air Force Medicine and associated supported organizations, extends from the deployed General Medical Officer (GMO) to a Graduate Medical Education (GME) faculty member or researcher at a major medical center. Each of these health professionals has different information needs, some of which are common to most and others that are very specialized. The Navy Medicine Electronic Library (NMeL) and the component supported organizations offerings currently focus on the content that is valuable to a broad range of Department of Defense Medicine and Navy health professionals. This core collection should meet the requirements stated below. These resources should be supplemented locally with specialized content appropriate to GME programs, research and/or a specialized mission. All Department of Defense Medicine and Navy Medicine health professionals would have access to content beyond the NMeL offerings through the Navy medical library network. Building and maintaining the Navy Medicine electronic Library, requires a combination of professional skills that include contracting, technical expertise and professional library skills. The task is very complex and requires full-time management. The current service virtual libraries rely heavily on content from aggregators. In the future Navy Virtual Medical Library offerings should include titles from individual publishers that meet broadly held information requirements, that are not available via current content or clinical resource aggregators or when it is more cost effective to purse content from the individual publishers that meet the broadly held information requirements.

1.2.1 Background Telelibrary Project:

Beginning in the mid-1990s the Telelibrary Project, under the direction of Naval Medical Information Management Center (NMIMC), delivered a core set of evidence-based medical references to all Navy health professionals in support of patient care, research and education. When the Telelibrary Project was established electronic evidence-based medicine resources were delivered on CD-ROM. These resources were considered to be computer assets rather than library resources and the funds to support and manage these resources were established at NMIMC rather than in the decentralized Navy medical libraries. Over time these resources migrated to the Internet and the Telelibrary Project developed into a virtual medical library. Management of the Telelibrary Project demanded contracting and medical library skills, but the funding and management remained at NMIMC and the librarians were not integrated team members and were informed rather than consulted on collection management. The Telelibrary Project remained a program of NMIMC for about fourteen years until NMIMC was transformed into the Navy Medicine Information Systems Support Activity (NAVMISSA) in 2009.Navy Medicine electronic Library (NMeL) When NAVMISSA assumed the management of the Navy virtual medical library, the Telelibrary Project was renamed the Navy Medical electronic Library (NMeL). The Program Manager of NMeL sought the assistance of Navy medical librarians as subject matter experts to advise and provide guidance on the management of the NMeL core collection of online clinical reference resources. Working in partnership the NMeL Program Manager and the Navy medical librarians have added utilities and required clinical resources that deliver content through an Open Athens portal to Navy health professionals. Navy medical libraries, through local Navy medical library websites, integrate NMeL resources with those purchased locally to support specialized patient care, research and graduate medical education at their institutions. The NMeL Program Manager has managed funding, worked with vendors, managed contracts, maintained access through the central portal, and resolved technical issues. Navy medical librarians have provided subject matter expertise to the virtual library and have enhanced the resources of the Navy Medical electronic Library with interlibrary loan and reference services. This collaboration maximizes the funds spent on evidence-based medicine information resources in Navy Medicine to meet clinical and Graduate Medical Education functional needs. Military Health System Virtual Medical Library. As the practice of military medicine has become increasingly joint an initiative began in the spring of 2013 to broaden the virtual and physical library collaboration beyond Navy Medicine to the three services and their military medical libraries to form the Military Health System (MHS) Virtual Medical Library. Army, Air Force and Navy Medicine, Armed Forces Medical Library Virtual Medical Library and Uniformed Services University of the Health Sciences (USUHS) Learning Resource Center each maintain their own electronic medical libraries. Many of the resources offered in these virtual libraries are similar. The goal of the MHS Virtual Medical Library is to align the three existing service-wide virtual libraries, the Defense Medical Virtual Library, and USUHS virtual library to meet the information needs of Department of Defense health professionals, maximize the DOD investment and forge a strong relationship between the MHS Virtual Medical Library and DOD medical libraries. This initiative has developed along with the Department of Health Affairs (DHA). A charter for the MHS Virtual Medical Library Integrated Product Team (DVMLIPT) was approved by the Clinical Portfolio Management Board (CPMB) in August 2014, and priorities have been established to consolidate contracts. The Navy has been identified as the Lead Agent for this effort with the Department Head, Library Services, Naval Medical Center Portsmouth (NMCP) and the NMeL Program Manager leading the effort. The idea is a valuable strategic initiative for military medicine.
