STATEMENT OF WORK

1. TITLE OF PROJECT:

Department of Homeland Security, Office of Service Center Operations, Weekly Truck Shipment of Sensitive Files Which Include Non-Classified A-Files

2. PERIOD OF PERFORMANCE:

Date of Award to September 30, 2017

3. PLACE OF PERFORMANCE:

Cargo shall be picked up at the first floor loading dock of the California Service Center located at

24000 Avila Road, Laguna Niguel, CA 92677 and delivered to addresses identified below.

4. PROJECT BACKGROUND:

The California Service Center requires a weekly pick up of pre-wrapped pallets of non-classified

Receipt and A-files from the Service Center for transport to the National Records Center in Lee's

Summit, Missouri (NRC), and the Remote Maintenance File Facility in Harrisonburg, Virginia

(HBG) where they will be stored. The destinations shall alternate each week unless otherwise identified.

5. TASKS:

For transport to HBG in Virginia, the truck shall arrive at the California Service Center on Friday to transport the pallets. The driver shall deliver the pallets to HBG within 7 total days from pickup.

Delivery must be made on a business week day. HBG does not accept deliveries on weekends.

For transport to NRC in Missouri, delivery shall fall on the first and third Wednesday of each month. The truck shall arrive at the California Service Center on the preceding Friday to ensure delivery is made on these two Wednesdays. NRC will not accept delivery from the CSC on any other day or week.

The driver shall not arrive earlier than 8:00 AM or later than 3:30 PM. No deviations to the arrival time at the pickup point shall be made without consent from the point of contact, Kathy Weiss, who must be notified of any change or delay. Should Ms. Weiss not be available, the contractor may notify California

Service Center Warehouse personnel @ 949-463-2976.

Prior to the arrival of the transporting truck, the California Service Center warehouse staff will shrink-wrap and stack the pallets on the dock. The driver of the transporting truck shall load the stacked pallets into the shipping container (trailer) at the California Service Center's dock and unload the same at the delivery location's dock. California Service Center personnel will not load the pallets onto the truck.

The driver will transport the pallets exactly as they were loaded onto the vehicle at the California

Service Center.

The driver shall provide a seal and place it on the outside of the container (trailer) for transport. The cargo, once loaded and sealed into the container (trailer), shall not be accessed or rearranged unless in the presence of a government employee. The seal shall be either metal or plastic and must have an identification number. This number shall be recorded on the Bill of Lading at the time the container

(trailer) is secured.

Neither the driver nor any other person shall tamper with, break, or replace the seal between the California Service Center and HBG or NRC. The same type of seal shall be used for both destinations.

Should it become necessary to tamper with, break, or remove the seal due to unforeseen circumstances, such as law enforcement requirement or weigh station inspections, etc., the contractor shall notify Ms. Kathy Weiss at 949- 389-3080 immediately to report the reason for such actions. In addition, the contractor shall replace the seal with a new one and provide Ms. Weiss with the replacement seal ID # immediately. If Ms. Weiss is not available, the contractor shall leave a voicemail message and send an e-mail explaining the situation. In any such case, the contractor shall provide copies of all written documentation (if it exists) regarding the incident to Ms. Weiss.

The contractor shall provide a dedicated truck with a container (trailer) which shall transport only California Service Center pallets, regardless of quantity, to destination. Nor the contractor or any subcontractors shall be brokers. The shipment of pallets, including as few as one only, shall be considered a Full Truck Load (FTL). The driver shall not mix loads to transport contents from other businesses, individuals, or government agencies with the California Service Center pallets. The driver shall not create compartments inside the container with intent to transport items from other entities.

Contents of pallets will include cardboard file storage boxes containing paper files and documents.

Pallets will be loaded with up to 45 boxes per pallet. Each pallet weighs approximately 1,440 lbs.

and is 57" tall, 46" long, and 37" wide. The number of pallets may vary from week to week. The following information shall be used for planning purposes:

Twenty five (25) pallets per shipment to NRC

Fifteen (15) pallets per shipment to HBG

The driver shall provide California Service Center warehouse personnel a Bill of Lading at pick up of the pallets for transport.

The truck driver shall deliver the contents from dock to dock within seven calendar days from the date of pick-up. In the event of a delay in delivery 1 day or more, even though it may fall within the allotted 7 calendar day delivery time frame, the contractor shall immediately notify Ms. Weiss at the phone number listed above. All three locations involved (origin and destinations) have normal dock facilities with normal dock heights. All three locations can provide the truck driver with a pallet jack to load and unload the shipment.

After each delivery the contractor will e-mail Kathy Weiss, California Service Center point of contact for the contract, the date and time the pallets were delivered to the destination. This information may be included on a revised Bill of Lading in lieu of e-mail.

The contractor shall be responsible for reimbursing USCIS for any cost associated with a compromised or lost shipment if it is determined that the contractor violated the procedures set forth in this Statement of Work.

12. CONTRACTOR ACCOUNTABILITY PROCESS

CSC places manifests in each box and transfers them electronically to NRC and HBG. NRC and HBG personnel inspect the pallets of files upon delivery. If they find that some files are missing or have been tampered with during transport, they contact the CSC CPAU unit. CPAU contacts the USCIS contracting office. The contracting officer opens an investigation with the contractor.

If the investigation finds that the Contractor violated or did not follow both the procedures stated in this Statement of Work and the written terms in the Formal Contract, the Contractor will reimburse the Government for any cost associated with the compromised files.
