(&"Arial,Italic"Name of Company&"Arial,Regular") Price Element Breakdown for Solicitation (&"Arial,Italic"RFQ or RFP #&"Arial,Regular") &D

For the purpose of this price breakdown model, these elements should be separately broken out as shown, even if it is the contractor's accounting practice to include these items in their Overhead and/or G&A rate. However, the contractor should reduce their reported Overhead and G&A amounts for the impact of these separately identified price elements to the extent they are already included in their Overhead and G&A. This will ensure that price elements are not duplicated in the breakdown.

When a weighted average of wage rates is used, the contractor shall explain the basis of their computations in this text box.

Instructions (also see Price Element Breakdown Instructions):

a. Complete the cells shown above in Rows 3 through 6 to detail the amount of (1) Paid Break and/or Lunch hours included in your pricing and (2) Non-Paid Break and/or Lunch hours. Row 7 automatically will compute the total.

b. For Rows 9 and 10 provide the total Paid break and/or Paid lunch break costs included in your "Model for Price Realism" worksheet.

c. For Row 12 provide the total Relief costs for providing any Paid or Unpaid breaks included in your "Model for Price Realism" worksheet.

d. Row 14 automatically computes the total cost for Paid Breaks, Lunches, and Relief.

e. In the text below provide a detailed narrative explaining (1) the basis of estimate for paid break/lunch costs included in your "Model for Price Realism" worksheet (e.g. XX amount of break /lunch time per an 8 hour shift) and (2) relief costs included in your "Model for Price Realism" worksheet. This narrative shall (1) explain which cost elements from the "Model for Price Realism" Worksheet contain the break and relief costs detailed above including the amount of such costs included in each identified element, (2) the computations used to determine the paid breaks/lunch costs included in the pricing, and (3) the computations for the costs of providing relief. Additional worksheet tabs may be added if necessary to provide this requested Relief and Break pricing information

f. Note that the data from this spreadsheet will NOT automatically populate the applicable cells in Row 52 of the Model for Price Realism spreadsheet.

Instructions (also see Price Element Breakdown Instructions)

a. The purpose of this worksheet is to provide additional pricing information regarding Other Positions (i.e. other than PSOs and direct billed Supervisors) that are included in your Labor Hour price build-up. This work sheet should exclude supervisors that will be billed direct from attachment 6a. Please note that the Total Costs without G&A and profit for each period will NOT automatically populate the Other Positions cells in row 93 of the "Model for Price Realism" tab. Any applicable G&A and profit for the Other Positions should be accounted in the "Model for Price Realism" tab" .

b. For purposes of this worksheet, full-time equivalent employees equal the number of employees on full-time schedules plus the number of employees on part-time schedules converted to a full-time basis to staff the contract. For instance, if 5,000 hours are quoted for a particular position and on average the annual productive hours of a full time employee for that position is 1800 hours (e.g. 2080 less 80 holiday less 160 vacation leave less 40 sick), the equivalent FTE to staff the labor category is calculated as 5000/1800 = 2.8 FTE. Note that for staffing to an FTE level, a contractor may use a combination of full time and part time employees. This information shall be entered in the applicable cells for each Other Position category. This information may be used for Government Price Realism evaluation purposes.

c. NOTE - The NWDTP Instructor costs in row 6 above should not include the NWDTP Instructor training costs captured under the "NWTDP Instructor Training" tab.

Travel and Per Diem Costs for Initial 40 Hour NWDTP Instructor Certification (S.O.W. 6.7.3.1.1.2)$ - 0
Total Cost for NWDTP Instructor Training$ - 0

a. The purpose of this worksheet is to provide additional pricing information regarding the number of individuals that are included in your costs for the initial 40 hour NWDTP Instructor Training (See SOW Sections 6.7.3.1, 6.7.3.1.1, and 6.7.3.1.1.2). Please note that the additional 32 hours of co-teaching and observation training (See SOW Sections 6.7.3.1 and 6.7.3.1.2), as well as the annual 16 hour recertification (See SOW Section 6.7.3.2), should not be priced on this spreadsheet. These training activities will occur during normal NWDTP PSO training provided by the NWDTP Instructor. There is a line item on row 6 of the "Other Positions" tab to record the costs for the NWDTP Instructor(s) other than the initial training priced on this worksheet. Also note that the "Total Costs for NWDTP Instructor Training" for each period will NOT automatically populate the corresponding cells in Row 63 (Training -X Ray Screening training) of the "Model for Price Realism" tab. Accordingly, the vendor should make sure that the appropriate costs from the “NWDTP Instructor Training” tab are presented in row 58 of the “Model for Realism” tab. This information may be used for Government Price Realism evaluation purposes.

b. With regard to National Weapons Detection Training, the FDA will allow training to be conducted on their equipment at their facilities during non-peak hours (weekends or outside of normal business hours). In addition, the Government will allow a deferment period of up to 120 days after the commencement of contract performance to get the contractor’s Instructors trained and certified at FLETC and to train their PSOs on the screening equipment.

Narrative Description on Basis of Estimate for proposed NWDTP Instructor Training Cost Describe the basis of estimate for the "Total Cost for NWDTP Instructor Training" amounts by contract period. For example, costs represent wages, fringes, travel costs, per diem, etc. The basis of estimate should describe how the portion of those costs applicable to the PSOs and Supervisors respectively was calculated.

This information may be used for Government Price Realism evaluation purposes.
