PEFORMANCE WORK STATEMENT
Leased Equipment and Ancillary Services
General Information
Leased Equipment
1.0 Background
Historically, there has been an absence of adequate and appropriate support and care for disaster survivors within shelters. The Leased Equipment and Contracted Services list herein has been developed in conjunction with the FEMA Office of Disability Integration and Coordination with guidance from subject matter experts in emergency management.
This document identifies, and seeks leased equipment and contracted services to sustain and support, a 100 person shelter for a minimum of 7-days, which FEMA may provide as supplemental assistance to shelter providers, when requested by a State, pursuant to Section 403 (b) of the Robert T. Stafford Disaster Relief and Emergency Assistance Act (Public Law 93-288) as amended.
2.0 Scope:
FEMA is establishing a contracting vehicle for leased equipment, herein defined as “LE” LE which consists of line item LE-008 (described in paragraphs 5.0, 5-1 and Table 5-1).
Contractor(s) shall provide individual supplies and ancillary services listed in paragraph 5.1 on an ala carte or as needed basis, as requested by FEMA for rapid deployment under emergency conditions. The contractor(s) shall provide a Technical Representative to facilitate communication with the FEMA Contracting Officer Representative (COR); the Contracting Officer (CO) is the only individual authorized to make contractual decisions on behalf of the organization.
3.0 Deliverables and Delivery Schedule
Emergency Response/Surge Delivery:
Each contractor(s) shall be able to deliver the minimum and maximum amounts of leased equipment listed in paragraphs 5.0 and 5.1 on an ala carte basis as requested by FEMA for emergency response, for delivery within 48 hours of issuance of a task order, and to any location within the CONUS specified by the FEMA Task Order. Contractor must have the ability to continuously electronically monitor and report location of shipments from origin to destination.
4.0 Record keeping and Acceptance:
A FEMA representative will sign for receipt of leased equipment at each location.
contractor (s) shall maintain accurate records of all shipments and deliveries and ensure that proper signatures by FEMA representative are obtained for tracking and accountability purposes, in accordance with government regulations.

5.1
GN001: Generator Services: Requirement consists of leasing 30-Kilowatt diesel generators with service and maintenance for use at designated disaster shelters and kitchen facilities. The contractor shall provide all management, tools, equipment, and labor necessary to lease, deliver, maintain and repair a minimum of 1 up to a maximum of 200, 30- Kilowatt diesel generators. The contractor will provide service, maintenance and supervision of generator placement, setup, tear down and removal. The contractor’s services, operations and maintenance activities include but are not limited to:
a. Contractor shall perform system operation check after delivery.
b. Contractor shall provide a 7 day a week/24 hour service number in case of routine or emergency repairs. No generator shall be inoperable for more than 24 hours.
c. Contractor shall pick-up generators within 24 hours of notification of requested service termination.
d. Contractor shall replace non-functional generator if repair is not feasible at no additional cost within 24 hours of determination.
LE-010: Forklifts, All Terrain Service: Requirement consists of delivery of all terrain forklifts with a 6,000 pound lift capacity. The contractor shall provide all management, tools, equipment, and labor necessary to lease, deliver, and service a minimum of one (1) and up to a maximum of 200 all-terrain forklifts with a minimum of 6,000 pounds lift capacity for use at designated disaster kitchens and warehouse facilities. The contractor shall ensure all forklifts are in a sound mechanical condition and shall meet all applicable federal, state and local safety laws and regulations. Forklifts will be operated by qualified forklift drivers who are on-site. The contractor shall ensure all delivered forklifts are
3 | P a g e suitable for their intended use. The contractor’s services, operations and maintenance activities include but are not limited to:
a. Perform forklift operational check after delivery.
b. Provide a 24 hour service number in case of inoperable equipment and the need for emergency repair. No forklift shall be inoperable for more than 24-hours.
c. Remove leased or rented forklifts within 24-hours of notification.
LE-011: Pallet Jack (Hand) Service: Requirement consists of delivery of industry standard hand pallet jacks with weight capacity between 3,500 through 4,500 pounds.
The contractor shall provide all management, tools, equipment, and labor necessary to lease, deliver, and service a minimum of two (2) up to a maximum of 400 hand pallet jacks with weight capacity between 3,500 through 4,500 pounds for use at designated disaster kitchen and warehouse facilities. The contractor shall ensure all hand pallet jacks are in sound mechanical condition and shall meet all applicable federal, state and local safety laws and regulations. The contractor shall ensure all delivered hand pallet jacks are suitable for their intended use. The contractor’s services, operations and maintenance activities include but are not limited to:
a. Perform pallet jack operational check after delivery.
b. Provide a 7-day week, 24 hour service number in case of routine or emergency repairs or replacement. No pallet jack shall be inoperable for more than 24 hours.
c. Remove leased or rental hand pallet jacks within 24 hours of notification.
LE-012: Hand Trucks (Dollies): Requirement consists of delivery of industry standard hand truck Dollies with weight capacity between1,500 through 2,000 pounds. The contractor shall provide all management, tools, equipment, and labor necessary to lease, deliver, and service a minimum of one (1) up to a maximum of 200 hand trucks (i.e.
dollies) with weight capacity between 1,500 through 2,000 pounds for use at designated disaster kitchen and warehouse facilities. The contractor shall ensure all hand truck dollies are in sound condition and meet all applicable federal, state and local safety laws and regulations. The contractor shall ensure all delivered hand trucks are suitable for their intended use. The contractor’s services, operations and maintenance activities include but are not limited to:
a. Perform hand truck operational check after delivery.
b. Provide a 7 day a week, 24 hour service number in case of routine or emergency repairs or replacements. No dolly shall be inoperable for more than 24 hours.
c. Remove leased or rented hand trucks within 24-hour notification.
LE-015: Power Washers Services: Requirement consists of pressurized washers. The contractor shall provide all management, tools, equipment, and labor necessary to lease, deliver, and service a minimum of four (4) and up to a maximum of 800 pressurized gas washers for use at designated disaster kitchen and warehouse facilities. The pressurized gas power washers are the only acceptable power washers to fill this requirement. The contractor shall ensure all delivered pressurized gas power washers are suitable for their
4 | P a g e intended use. The contractor’s services, operations and maintenance activities include but are not limited to:
a. Perform pressurized washer operations check after delivery.
b. Perform preventative, routine or emergency maintenance as needed.
b. Provide a 7-day week, 24 hour service number in case of routine or emergency repairs or replacements. No power washer jack shall be inoperable for more than 24 hours.
c. Remove leased gas pressurized washers within 24 hours of notification.
5.2
FEMA leased equipment is not to be substituted and ancillary contracted services are not to be changed unless approved by the cognizant Contracting Officer (CO).
6.0 Place of Performance:
During a disaster response, leased equipment and ancillary or requisite services place of performance will be determined by the FEMA task order. This includes but is not limited to designated shelters, kitchens and warehouse facilities.
7.0 Period of Performance
The period of performance shall become effective on the date of contract award and remain in effect for one (1) base year with four (4) 12-month option years.
8.0 Reporting
The contractor (s) shall maintain accurate records of all deliveries, repairs and pick-ups. The contractor(s) shall ensure proper signatures are obtained for tracking and accountability purposes, in accordance with government Section 8.1 Reporting Requirements. Contractor invoice will show date of FEMA acceptance along with rental equipment provided and received along with the date rental equipment was returned.
8.1 Reporting Requirements
Daily Reports – Following the award of a Task Order, contract awardees are responsible for providing the following daily reports to the FEMA COR or designated representative; at a minimum the reports should contain the following:
1) Total leased or rental equipment items by type and site location with operational status (e.g. 1 out of 2 power washers operational, 1 forklift in-operable (e.g. awaiting parts, service…etc.).
2) Total number of equipment currently under repair – specifically show date equipment identified as non-operational and corrective action plan (e.g. 2 washers found to be unserviceable at 5:00pm on July 26 – projected repair date/time by 4:00 pm on July27th).

3) The contractor shall provide any other limiting equipment factors and constraints to the FEMA COR.
