CLIENT AUTHORIZATION LETTER

DEAR “CLIENT”:

We are currently responding to the Federal Emergency Management Agency (FEMA) Request for Quotation No. HSFE60-17-R-0003 for the Customer and Data Services for the Risk MAP Program.

FEMA is placing emphasis in their procurements on past performance as a source selection factor. They are requiring that clients of entities responding to their solicitations be identified and their participation in the evaluation process be requested.

In the event you are contacted for verification and/or information on work we have performed, you are hereby authorized to respond to those inquiries.

We have identified Mr./Ms. of your organization as the point of contact based on their knowledge concerning our work.

Please provide your comments on the attached “Past Performance Questionnaire” and return them to: Lora Eskandary at the following email address:

lora.eskandary@fema.dhs.gov

The SUBJECT LINE on the email shall be as follows: “Past Performance Questionnaire for HSFE60-17-R-0003”. If SUBJECT LINE does not provide this reference, the Government will not open the document; thus, the information contained in the questionnaire will not be evaluated.

PAST PERFORMANCE QUESTIONNAIRE

CDS Solicitation HSFE60-17-R-0003

The Department of Homeland Security (DHS)/FEMA recently released a request for proposal for the Customer and Data Services for the Risk MAP Program. Offerors were asked to provide information on contracts similar in nature, size and complexity to this requirement on which they had performed in the past. An offeror may submit you as a reference for past performance. (See blocks 1 & 2 below).

We need your opinion on how well the contractor performed on this contract. Please fill out the attached questionnaire and return to Lora Eskandary at the following email address: lora.eskandary@fema.dhs.gov

The SUBJECT LINE on the email shall be as follow: “Past Performance Questionnaire for HSFE60-17- R-0003”. If SUBJECT LINE does not provide this reference, the Government will not open the document.

Your input will be confidential. DHS/FEMA may contact you to clarify or verify any information provided. Please note, your comments will be used to form the basis for evaluation of this offeror. 1. QUALITY OF SOLUTION: The contractor comprehended the goals, thoroughly researched the problem, and presented a comprehensive, complete, feasible, and effective solution. (Met the needs and performed successfully).

2. EFFECTIVE AND EFFICIENT USE OF

RESOURCES: The contractor deployed, in a timely manner, the proper amount and type of personnel, facilities and other resources to support the project. Use of subcontractors was appropriate in supporting the project.

3. COMMUNICATION: The contractor worked and communicated well with client management, staff, and contracting officials as well as with his or her own staff and subcontractors.

4. TIMELINESS OF PERFORMANCE:

The contractor planned and proposed realistic schedules, successfully monitored performance and completed work on time.

5. CUSTOMER SATISFACTION: The contractor exhibited a commitment to customer satisfaction and achieved customer satisfaction.

6. RESPONSIVENESS AND

ATTENTIVESNESS: The contractor exhibited reasonable and cooperative behavior in response to client changes, criticism/rejection of contract deliverables and in detecting and correcting errors, poor performance, and other problems.

7. COST CONTROL: The contractor successfully planned and proposed realistic costs and monitored performance, and consistently operated at or below budget.
