C.1 General Management and Administration

1. Purpose

FEMA is authorized to provide disaster assistance to individuals and households for emergencies, major disasters, and Incidents of National Significance under the Robert T.

Stafford Disaster Relief and Emergency Assistance Act (Public Law 93-288), as amended.

The Contractor shall support the Government’s mission to construct 4 homes in Galena, AK. The mission includes initial site preparation, providing materials, and finished site work.

2. Scope

The contractor shall furnish all equipment, labor, and supervision needed to prepare home lots for construction of homes by volunteer organizations.

Work shall include clearing of trees and debris, hauling and placement of fill material, supply of materials, construction of pads and access ways and certified survey of fill elevations. All work shall be in accordance with all applicable Federal, State and local codes and regulations. The contractor shall be responsible for obtaining all necessary permits and paying any associated fees.

This work shall not include the hauling of government supplied materials to the site.

3. Locations and Facilities

Locations are within the City of Galena, Alaska, on individually-owned residential lots, as specified by FEMA. FEMA’s central point of operations will be at the Joint Field Office in Anchorage, AK, with representatives on-site in Galena. Specific lot locations are identified in Attachment 1.

4. Projected Workload

Required services may include:

• Clearing of trees and small quantities of debris from the housing site.

• Installation of fill material to required elevations.

• Preliminary site grading prior to construction of homes

• Final site work to include finished grading, driveway and walkway installation.

• Supply of 300 gallon fuel tanks with stand.

• Supply of materials for deck stairways

Expected timelines and operational requirements for the provision of services specified by this contract are included in Paragraph C6, “Additional Details”. However, the Contractor is responsible for supporting any Government alterations to the contract schedule. The Government will attempt to provide the Contractor with advanced notice of any changes to the schedule in writing.

5. Contractor Minimum Qualifications

The Contractor shall provide professional, courteous, and timely services, activities, and management to ensure that all work identified in this Statement of Work is completed.

The contractor shall provide to the COR, daily reports indication progress to date and identifying any issues that may cause delay.

The Contractor is responsible for providing personnel who can pass a background check, as specified by FEMA, for activities that require FEMA badges and / or access to FEMA facilities, equipment, and computer networks.

6. Performance Evaluation Meetings

The Contracting Officer (CO) or Contracting Officer’s Representative (COR) may require Contractor management to meet with the CO or COR and other Government Personnel to review Contractor performance. Conversely, the Contractor may request performance evaluation meetings with the CO/COR when he or she believes such a meeting is necessary.

Written minutes of any such meeting shall be recorded in the contract file and signed by the Contract Project Manager and the CO or COR. If the Contractor does not concur with any portion of the minutes, the non-concurrence shall be provided in writing to the CO/COR.

7. Pre-Contract Site Visit An onsite pre-contract visit will be conducted by FEMA and Army Corps of Engineers personnel with any contractors interested in bidding on this contract. This visit will be scheduled on/or about April 10, 2014. FEMA will notify potential bidders of the exact time and date of the meeting.

0001 Mobilization – This CLIN includes all costs to bring personnel and equipment to Galena for the construction services listed below.

0002 Preliminary Site Preparation – This CLIN includes all specified clearing, filling, removal, and site prep activities under the contract section “C.4 Construction Activities Prior to New Home Construction”. Sub-CLIN’s for this line item are aligned to specific properties.

0003 Finished Site Work – This CLIN includes all equipment and labor to comply with the “C.5 Construction Activities after New Home Construction” section of this contract. This work includes final site grading, finish grading of access driveway, and any necessary site clean-up to meet the requirements of this contract. Sub-CLIN’s for this line item are aligned to specific properties.

0004 Aggregate – This CLIN includes the total volume of gravel material in Cubic Yards to be used on all of the sites. Work under this CLIN shall be as specified in this contract under sections “C.4 Construction Activities Prior to New Home Construction” and “C.5 Construction Activities after New Home Construction”.

0005 Geotextile – This CLIN includes the Geotech fabric as specified in 12.5 foot x 300 foot rolls and all associated labor. Work under this CLIN shall be as specified in this contract under sections “C.4 Construction Activities Prior to New Home Construction” and “C.5 Construction Activities after New Home Construction”.

0006 Fuel Tanks This CLIN includes the supply and delivery of one (1) 300-gallon fuel oil tanks with stands. Stands are to be 7 feet high (bottom of tank will be 7 feet high when mounted). Work under this CLIN shall be as specified in this contract under Section C.5 construction Activities After New Home Construction.

0007 Deck Stairway Materials. This CLIN includes the supply of all deck stairway materials as specified in the Deck Stair Materials List, Attachment 4.

0008 Demobilization – This CLIN includes all costs to recover personnel and equipment from Galena.

0009 Shipping – This CLIN includes all shipping costs, for all materials, included in this contract.

C.3 Roles and Responsibilities of Contractors and FEMA

Contractor Roles and Responsibilities

A. General Roles and Responsibilities Contractor roles and responsibilities are contingent on the services required specific to the disaster event. General roles and responsibilities shall include the following:

1. Provide all materials, labor, equipment, and services necessary to successfully complete the mission at hand.

2. Provide professional, courteous, and timely customer service (to include contractor staff, subcontractors, and representatives) with flexible and transparent program management to ensure all required work is completed.

3. Effectively coordinate with FEMA’s COR or designee. Identify and notify COR of potential project constraints.

4. Provide staff that is able to obtain access to Government facilities and perform work on FEMA’s project in accordance with all applicable badging and security requirements.

5. Provide staff (to include subcontractors) that is properly licensed and trained to perform the work in accordance with all applicable Federal, State, and Local regulations.

6. Perform work and task order activities in accordance with all timetables and schedules.

7. Account for, track, and maintain any Government Furnished Equipment (GFE), if applicable.

8. Dispose of all construction debris, in compliance with all Federal, State, and Local laws, regulations, and rules.

9. Maintain accurate records, and provide all documents as required in the contract or task order.

B. Contractor Safety The Contractor shall furnish all required safety equipment and/or clothing required for personnel as mandated by Federal, State, and Local codes and regulations. The Contractor shall also maintain a safe working environment for their employees, unit occupants, FEMA staff, other contractors and the general public.

At no time will the Contractor leave unattended or otherwise unprotected, any excavation, exposed utilities, or any other safety hazards that might cause injury to persons.

The Contractor and his personnel shall comply with the Department of Labor and the Occupational Safety and Health Administration (OSHA) safety regulations, U.S. Army Corps of Engineers Safety Manual EM 385-1-1, (http://www.publications.usace.army.mil/Portals/76/Publications/EngineerManuals/EM_385-1- 1_English_2003.pdf) rules and requirements, as well as any Federal, State, County and Local laws, codes, and ordinances.

C. Identification of Personnel.

The Contractor shall be responsible for the following processes in order to facilitate background checks and badging of their personnel:

1. Provide the names and position of all personnel (including temporary personnel and/or subcontractors) to the COR in writing within two business days prior to their arrival at Galena.

2. The Contractor shall identify onsite supervisory and/or Foremen personnel to the COR upon the commencement of work.

D. Contractor Qualifications Contractor personnel performing work under this contract shall meet the following minimum qualifications:

1. The Contractor shall provide personnel with the appropriate skill set required for their position. It is the Contractor’s responsibility to ensure that all subcontractor personnel meet or exceed FEMA’s Contractor Qualification Standards..

2. If required, provide documentation of each technician’s professional certification (i.e., state licenses, certificates of training course completion, letters from previous employers certifying experience, resumes, membership affiliations to professional organizations, or other such documentation) to FEMA and other officials per State and Local requirements.

3. The Contractor shall have a process that ensures all work is in compliance with the standards outlined by FEMA and all applicable Federal, State, Tribal and Local laws and regulations.

E. Quality Control

1. The Contractor shall develop and maintain a Quality Control Plan with a schedule for completion of work, and an organizational structure to monitor performance to ensure appropriate quality is achieved. 2. Establish a process for ensuring the work is consistent with applicable standards prior to presenting the status of tasks as complete to FEMA. The performance metrics shall be an appropriate measure of compliance.

3. The Contractor shall monitor the quality of work using a quality control plan that will also serve to facilitate Government oversight and inspection (quality Assurance).

F. Permits Obtain all required permits or waivers of those permits required by Federal, State and Local regulations per the instruction of the CO or designee, including all permits required for unit transportation and installation.

FEMA Roles and Responsibilities

In order to support Contractor activities FEMA shall perform the tasks listed below. However, the scope of services provided by FEMA may vary based on disaster-specific needs and the scope of services provided by the contractor or other Federal agencies.

A. Provide the contractor with specifically chosen properties to develop as home sites.

B. Coordinate Environmental Planning and Historic Preservation assessments.

C. Address any issues that arise with disaster survivors.

D. Work with Federal, State and Local officials to obtain permitting and code waivers when feasible and appropriate.

C.4 Construction Activities Prior to New Home Construction

A. General

1. Timeframe for Mobilization Upon issuance of the Notice to Proceed (NTP) the Contractor shall make immediate arrangements to procure the necessary materials and prepare equipment for shipment to Galena. It is anticipated that materials and equipment will be loaded on the first tow leaving the Nenana Barge Facility (approximately May 15, 2014).

2. Timeframe for Completion of this Phase of the Work Activities described under this Section (C.4) shall be completed within twenty-four (24) calendar days after arrival of the first barge in Galena.

3. Coordination of Site and Utility Work

All efforts will be effectively coordinated with all Government entities, as required by Federal, State and Local laws and regulations.

Utility services shall not be interrupted except for brief periods if necessary. The Contractor shall be responsible for all costs related to protecting existing utilities.

4. Environmental and Historical Preservation Support The Contractor shall Support FEMA or designee work on any Environmental and Historical Preservation efforts as required.

5. Work Limits

The construction work limits (CWL) are defined as the lot lines of each new home site and as such all work by the contractor must be within the confines of each individual lot.

6. Surveying The Contractor shall layout the work from established benchmarks. For each feature of work, field staking shall define area limits such that the COR can easily determine if alignment and/or limit adjustments need to be made. The Contractor shall provide written certification of the top elevation of fill material (130.5 msl). This work shall be paid for under CLIN 0002, Preliminary Site Prep.

B. Site Preparation, Grading and Fill Material

1. Site Preparation

a. Removal of trees, vegetative debris or other material is required to create a construction area of 4200 square feet (60’x70’). Following this the contractor shall add fill material as necessary to bring the house footprint area (40’x50’) up to the required grade of 130.5.

Finally, the contractor shall rough grade the site to allow access and movement of construction equipment in preparation for home construction.

b. Adequate drainage (1% slope or greater) and erosion sediment control shall be provided during the entire period of construction to prevent water from collecting or standing on the area to be constructed. Roadways and individual sites are to be crowned.

c. Should the surface for any reason become rough, corrugated, or traffic marked prior to completion, such portions shall be regraded.

d. Site grades shall be checked for low spots and depressions. All low spots and depressions shall be filled so the entire site has positive drainage.

e. All material placed as fill or backfill shall consist of mineral soils with no vegetative matter. Aggregate shall not be larger than three inches (3”) in size. Unsuitable material shall be disposed of offsite.

f. This work shall be paid for under CLIN 0002, Preliminary Site Prep and CLIN 4, Aggregate.

C. Site Access

1. The contractor shall create sufficient access for equipment necessary for installation of foundations by others, and for other agencies to construct homes. This work includes installation of a gravel driveway and sufficient grading to allow the work to proceed.

2. Materials: The aggregates for the surface and base course shall be locally-available gravel material. A stabilization fabric or geo textile fabric shall underlay the aggregate.

3. All access areas upon which aggregate is to be placed, including road sub-grades, shall be stripped 6-inches before the aggregate placement is started. Material shall not be placed on surfaces that are muddy, or where unsatisfactory material remains in or under the aggregate. The entire area that is stripped shall be prepared prior to the placement of the aggregate.

4. Any excavation sub-grades that are unstable, pump, rut excessively, reveal soil conditions that are substantially different from the general site conditions, or are unsuitable for proceeding with the work shall immediately be reported to the COR. Soft or otherwise unsatisfactory material shall generally be removed and replaced with satisfactory material or other approved material as directed.

5. Compaction: During fill and backfill placement, each layer shall be spread uniformly in a maximum uncompacted lift thickness of 6 inches and compacted with equipment appropriate for the material until there is no further evidence of consolidation.

Embankments and subgrades shall be kept crowned or sloped for drainage. All work shall implement best management practices for erosion control.

6. Fabric Placement: Placement shall be in accordance with manufacturer’s recommendation.

7. This work shall be paid for under CLIN 0002, Preliminary Site Prep. Materials used for this work shall be paid for under CLIN 0004 Aggregate, and CLIN 0005, Geotextile.

D. Site-Specific Information

1. Site A: : Lot 9, Block 6

a. Existing Condition: Current home on this lot will be demolished prior to construction of new home. Lot does not need clearing and grubbing. Elevation of the ground level is 130.3, which is approximately the required elevation of 130.5; therefore gravel pad is not necessary. There is an existing fuel tank for heating in the rear of the house. Water supply from the city comes into the existing house. Power pole is on the south-eastern side of the lot.

2. Site B: Lot 6 Block 2A

a. Existing Condition: This lot is fairly developed and has an existing structure. The log home is approximately 15’ x 30’. It’s situated close to the road. This structure will be removed by others prior to the work required under this contract. A fair amount of debris is on the lot, but it would take minimal effort to clear it for new home, provided it’s situated where the existing home is now. Existing ground elevation is about 130.4 msl.

b. Site Prep: The existing structure sits on the ground cribbed up with timber blocks no more than 18” high. Demo of the structure would be best accomplished with a small backhoe or excavator, reduced down to a size adequate to load in dump truck for disposal. The home has a city water line (buried) coming into it that would need to be disconnected and isolated prior to demo. There doesn’t appear to be a holding tank for sewer on the site now. The fuel tank is in good shape and would need to be relocated until new construction. Once demolished, the existing grade would take minimal leveling and grading. The lot slopes at a gradual grade towards the back

c. Utilities (for reference only – utility hook-up’s are not required by this contract):

A power pole is near and a service drop to the new home would be around 50 feet in length. The lot has city water service and the new dwelling could have the existing line reconnected. This line can’t be moved very easily, so orientation of the home would be somewhat driven by the current location of the water service lateral. Sewer would be via insulated holding tank that would require periodic pump service from the City. Access to the holding tank would be possible without a lot of work or additional fill on the lot, regardless of where it is eventually located.

3. Site C: Lot 10 Block 6

a. Existing Condition: This lot has a 20’ x 40’ structure sitting on timber cribbing.

The side of the home close to the road has approximately 1’ of clearance between the existing grade and the sill plate (cribbing is approximately 12” high). On the opposite side of the home, the clearance between the existing grade and the sill plate is approximately 3.5’ to 4’, so the lot grade slopes down fairly steep as you proceed toward the back of the lot. Power is very close and the existing service drop was within 25’ of an existing power pole. It’s not clear if the home had a holding tank for sewer. A 10’ section of arctic pipe was observed near the home but it was not connected and it wasn’t obvious if this was an old water or sewer pipe. This structure will be removed by others prior to the work required under this contract. Existing ground elevation is about 127.9 msl.

b. Site Prep: Site prep would require a medium to large sized back-hoe to dismantle the structure, load debris and load for haul to land fill. A minimal amount of grading will be required to access this site.

c. Utilities (for reference only – utility hook-up’s are not required by this contract):

A power pole is near and a service drop to the new home would be around 25’in length. It’s not clear if the lot has city water service. If so, the new dwelling would need to have the existing city lateral located, possibly extended to accommodate the new structure, then connected. Sewer would be via insulated holding tank that would require periodic pump service from the City. Some fill may be needed to make access for service easier.

4. Site D: Lot 24 Block 7

a. Existing Condition: This is an undeveloped wooded lot. Existing ground elevation is about 131 feet msl.

b. Site Prep: Will require significant removal and disposal of trees. Site grading will be required and placement of gravel to allow access and movement of construction equipment.

c. Utilities: No utilities exist at the site.

C.5 Construction Activities after New Home Construction

A. General

1. Timeframe for Completion of Finish Work The Finished Site Work phase shall be completed within seven (7) calendar days of notice from the COR that the site is ready for finish work.

2. Coordination of Site and Utility Work

All efforts will be effectively coordinated with all Government entities, as required by Federal, State and Local laws and regulations.

3. Work Limits The construction work limits (CWL) are defined as the lot lines of each new home site and as such all work by the contractor must be within the confines of each lot.

4. Debris Cleanup Construction debris, waste materials, packaging material and the like shall be removed from the work site.

5. This work shall be paid for under CLIN 0003, Finished Site Work.

B. Site Grading

1. Site Access – The contractor shall provide a finished gravel access road from the street onto the lot and up to the new dwelling in accordance with paragraph 2 below. Finished gravel access roads shall be 12 feet wide and have a base course of 6 inches of compacted gravel.

2. Grading

a. Adequate drainage (1% slope or greater) and erosion sediment control shall be provided during the entire period of construction to prevent water from collecting or standing on the construction area. Roadways and individual sites are to be crowned.

b. The surface of the top layer shall be finished to smooth grade. Blading may be necessary for the finished surface to conform to the lines, grades, and cross sections.

c. Site grades shall be checked for low spots and depressions. All low spots and depressions shall be filled so the entire site has positive drainage.

d. All material placed as fill or backfill shall consist of mineral soils with no vegetative matter. Aggregate shall not be larger than three inches (3”) in size. Unsuitable material shall be disposed of offsite.

e. All access areas upon which aggregate is to be placed, including sub-grades, shall be stripped 6-inches before the aggregate placement is started. Material shall not be placed on surfaces that are muddy, or where unsatisfactory material remains in or under the aggregate. The entire area that is stripped shall be prepared prior to the placement of the aggregate. Final grading of the site, including the roadways will be completed as to prevent the ponding of surface water.

f. Any excavation sub-grades that are unstable, pump, rut excessively, reveal soil conditions that are substantially different from the general site conditions, or are unsuitable for proceeding with the work shall immediately be reported to the COR. Soft or otherwise unsatisfactory material shall generally be removed and replaced with satisfactory excavated material or other approved material as directed. For trenches under driveways, common backfill above the bedding shall be placed in 6" lifts and compacted with special purpose compaction equipment. Compaction equipment shall be selected to avoid damage to the pipe.

g. Soil compaction testing is not required to provide an engineered solution for stabilization of the driveways. Instead, liberal use of appropriate geotextile products such as Tensar or equivalent is required.

h. This work shall be paid for under CLIN 0003, Finished Site Work.

C. Placement of Fuel Oil Tanks

1. The contractor shall supply and install new fuel tanks on a steel frame stand at Site D.

Location of fuel tanks shall be field sited. Sites A, B and C have existing fuel tanks which may be re-used, however the contractor is required to provide a steel frame stand and to place the tank adjacent to the new home.

2. The contractor shall construct a gravel pad upon which the fuel tank and stand may be placed. Finished elevation of this pad shall be field sited however, will be sufficient in height to allow gravity feed of fuel oil from the tank.

3. This work shall be paid for under CLIN 0006, Fuel Tanks and Stands.

D. Provision of Deck Stairway Materials

1. The contractor shall provide to all deck stairway materials detailed in attachment 3.

This material will be stockpiled at an area designated by FEMA personnel.

E. Driveways, Walkways, and Pads

1. Materials: The aggregates for the surface and base course shall be locally-available gravel material. A stabilization fabric or geo textile fabric shall underlay the aggregate.

2. Compaction: During fill and backfill placement, each layer shall be spread uniformly in a maximum uncompacted lift thickness of 6 inches and compacted with equipment appropriate for the material until there is no further evidence of consolidation.

Embankments and subgrades shall be kept crowned or sloped for drainage. All work shall implement best practices for erosion control.

3. Preparation: All topsoil and surface material shall be removed and the sub-grade compacted prior to the placement of the filter or geo-textile fabric and aggregate base.

There should be no evidence of depressions from site equipment or excess rutting.

4. Finishing: The surface of the top layer shall be finished to grade, and the finished surface shall be of uniform texture. Light blading during compaction may be necessary for the finished surface to conform to the lines, grades, and cross sections. Should the surface for any reason become rough, corrugated, uneven in texture, or traffic marked prior to completion, such unsatisfactory portion shall be graded.

5. Fabric Placement: Placement shall be in accordance with manufacturer’s recommendation.

6. Payment: Work under this section shall be paid for under CLIN 0003, Finished Site Work. Materials shall be paid for under CLIN 0004 Aggregate, and CLIN 0005 Geotextile.

C.6 Additional Details

A. Quality Control The Contractor shall develop, present, maintain and update a quality control plan to monitor performance to ensure appropriate quality is achieved. The quality control plan must include processes for ensuring work is consistent with the specifications set forth in this Statement of Work. The plan shall identify processes for addressing delayed activities including, but not limited to, unit installation, unit maintenance and deactivation. The Contractor shall monitor the quality of work using this established plan, which will also be used to facilitate Government oversight of the contract and inspection of work performed.

Within the required weekly report, the Contractor shall provide information on all items that do not meet the established quality control plan. The quality control plan must be reviewed and approved by the COR within a time frame as specified by the Contracting Officer.

B. Travel Requirements Contractors that are not local to the Place of Performance shall be in compliance with the Federal Travel Regulations (FTR) and per diem levels established by the General Services Administration (GSA).

C. Performance Requirements and Performance Evaluation See Section E: Inspection and Acceptance.

D. Place(s) of Performance Galena, Alaska.

E. Period of Performance

1. All work under this contract shall be completed by September 6, 2014.

SETION E; INSPECTION AND ACCEPTANCE

Inspection and acceptance will take place in accordance with the following clauses. This will be accomplished by FEMA and Contract personnel at the Nenana barge facility. DD Form 250, (Attachment 3) will be used for this purpose.

52.246-12 Inspection of Construction (Aug. 1996)

(a) Definition. “Work” includes, but is not limited to, materials, workmanship, and manufacture and fabrication of components.

(b) The Contractor shall maintain an adequate inspection system and perform such inspections as will ensure that the work performed under the contract conforms to contract requirements.

The Contractor shall maintain complete inspection records and make them available to the

Government. All work shall be conducted under the general direction of the Contracting Officer and is subject to Government inspection and test at all places and at all reasonable times before acceptance to ensure strict compliance with the terms of the contract.

(c) Government inspections and tests are for the sole benefit of the Government and do not --

(1) Relieve the Contractor of responsibility for providing adequate quality control measures;

(2) Relieve the Contractor of responsibility for damage to or loss of the material before acceptance;

(3) Constitute or imply acceptance; or

(4) Affect the continuing rights of the Government after acceptance of the completed work under paragraph (i) of this section.

(d) The presence or absence of a Government inspector does not relieve the Contractor from any contract requirement, nor is the inspector authorized to change any term or condition of the specification without the Contracting Officer’s written authorization.

(e) The Contractor shall promptly furnish, at no increase in contract price, all facilities, labor, and material reasonably needed for performing such safe and convenient inspections and tests as may be required by the Contracting Officer. The Government may charge to the Contractor any additional cost of inspection or test when work is not ready at the time specified by the Contractor for inspection or test, or when prior rejection makes re-inspection or retest necessary.

The Government shall perform all inspections and tests in a manner that will not unnecessarily delay the work. Special, full size, and performance tests shall be performed as described in the contract.

(f) The Contractor shall, without charge, replace or correct work found by the Government not to conform to contract requirements, unless in the public interest the Government consents to accept the work with an appropriate adjustment in contract price. The Contractor shall promptly segregate and remove rejected material from the premises.

(g) If the Contractor does not promptly replace or correct rejected work, the Government may --

(1) By contract or otherwise, replace or correct the work and charge the cost to the Contractor; or

(2) Terminate for default the Contractor’s right to proceed.
