Statement of Work Fisherman Hospital Scope of Work:
Damage assessment of Hospital spaces inundated with water during the declared storm event DR-4337-FL.
Deliverable of assessment information to include at a minimum:
1) Dimensions of each room, including the walls, ceiling, and flooring.
2) Type of materials associated with the walls, Ceilings, and flooring (drywall. Plaster, etc.)
3) Digital photographs of all walls, flooring, ceiling, and other contents of each space high lighting specific damage.
4) Description of all apparent or evidential damage associated with the above items to include:
a. Assessment of water intrusion impact of electrical components (wiring, conduit, outlets, fixtures, etc.)
b. Assessment of water intrusion impact on HVAC, duct work and insulation
c. Assessment on all mechanical electrical and plumbing of the facility FEMA to provide current documentation vendor will use in validating claimed damages. Due to the stated and/or documented presence of mold and asbestos, vendor is responsible for all safety measures and equipment appropriate for entry into the facility.
Facility that has remained inaccessible since the event, PA needs to a Contract assistance with damage assessments of Fisherman’s Hospital in Monroe County.
