SOLICITATION NUMBER: HSBP1012R0047

ISSUANCE DATE: July 25, 2012

CLOSING DATE: August 22, 2012, 4:00 p.m. EST

SUBJECT: Solicitation for a Small Business Vision Consultant Ladies/Gentlemen:

The United States Customs and Border Protection (CBP), represented by the Human Resources Management Division (HRM), is seeking pricing and technical proposals (Standard Form 1449 only) from qualified Small Business Contractors to provide non-personal services as a Vision Consultant under a non-personal service Indefinite Quantity Contract, as described in the attached solicitation.

Submittals must be in accordance with the attached information at the place and time specified.

Applicants interested in applying for this position MUST submit the following materials:

1. Price and Technical Proposal, Completed form SF-1449 (including SF-1449 continuation sheets as needed).

NOTE: Submission of a price and technical proposal in addition to the required forms is mandatory. A submitted resume, however, is considered supplemental application material. Submission of a resume alone or in lieu of the SF-1449 form IS NOT a complete application. All information to be evaluated must be contained in the SF-1449 form and must not depend on references to your resume. Failure to provide the required information and/or materials will result in your not being considered for employment.

2. Supplemental document specifically addressing:

A. Price Proposal for each of the five years (base plus four option years)

Estimated yearly reviews:

Base Year: 1056 each

Option Year One: 1056 each

Option Year Two: 1020 each

Option Year Three: 1020 each

Option Year Four: 1020 each

The cost for each year should be broken down into three categories: 1) Cost per vision consultation, 2) Cost per day for testimony, and 3) Cost for Travel.

B. Technical Proposal detailing each of the four (4) Evaluation Factors listed below:

Evaluation Factor 1: Technical Approach The Offeror demonstrates an understanding of the requirements and the immediate capacity to accomplish the work requirements.

Evaluation Factor 2: Transition Plan Approach

The Offeror demonstrates the ability to deliver, within the required timefame services to ensure seamless and continuous services.

Evaluation Factor 3: Past Performance Qualifications The Offeror demonstrates through past performance evaluations and resumes of proposed personnel, along with copies of current licenses.

Evaluation Factor 4: Management Approach

The Offeror demonstrates their approach to the requirements in the Statement of Work (SOW).

Factor 1: 50 Points Factor 2: 10 Points Factor 3: 20 Points

Factor 4: 20 Points Total Evaluation: 100 Points NOTE: The Evaluation Factors are worth 100 points. Applicants are required to address each of the Evaluation Factors on a separate sheet describing specifically and accurately the Offeror’s information relevant to each factor. Even though cost is not a numerically rated factor, apart from the TET, the Government (Procurement Office) will conduct a cost and/or price evaluation of each offeror’s cost/price proposal.
