ANSWERS 1-3 AMD 0004
Roof Fall Protection Services, Pentagon
RFP #HQ003419R0233
Amendment 0004
Question 1: Can you send over a list of all of the fall protection systems (lengths and quantities) so we can accurately account for the tags that would be needed and the time it will take to inspect each system?
Answer 1: The fall protection systems are comprised of three (3) types: a) Monorail beam and Trolley Carriage system on sloped roofs at A and E Rings; b) Cable system (Horizontal Life Line (HLL)) on sloped roofs at corridors; c) Cable system (Horizontal Life Line (HLL)) on flat roofs at B, C and D Rings; with these specifications and distances: d) 13,200 linear feet (2.5 miles) of Horizontal Life Line (HLL) (124 sections ); and e) 5,455 linear feet (1.1 mile) of Monorail Beam Trolley system.
Question 2: Wanted to ask about a requirement of the Price Proposal. Specifically Volume III: Price Proposal under section 2)a.i and 2)a)ii. What is SF 33 and SF 30?
Answer 2: This solicitation is on a Standard Form (SF) 1449, and an SF 30 is an amendment to a solicitation or a modification to an existing contract. Federal Acquisition Regulations (FAR) Part 53 describes the various forms available for a solicitation or contract action. Section L-4 is hereby updated to show that this requirement is on an
SF 1449.
Question 3: Contract line item was changed in Amendment 3 so the description given on the original RFP isn’t the same. Are we still required to use the original solicitation to write our prices on?
Answer 3: A conformed copy of the solicitation (conformed as of Amendment 0004) will be provided to offerors to input their prices into the contract line item numbers (CLINs) in Section B.

Performance Work Statement For
Roof Fall Protection Services, Pentagon
1.0 PROJECT OVERVIEW
1.1 Project Background and Objective
The Department of Defense (DoD), Washington Headquarter Services (WHS) maintains and operates DoD facilities in the National Capital Region (NCR). Pentagon Building Management Office (PBMO) has overall responsibility and upkeep for the Pentagon.
Currently, the Pentagon has two (2) different types of fall protection systems for the Pentagon Roof. The first protection fall system is a horizontal lifeline system consisting of stanchions and cable system. The second fall protection system is a railed trolley system. Approximate quantities are: 5,500 linear feet of railed systems and 13,200 linear feet of horizontal lifeline systems. Fall protection systems were installed in a phased multi-contract effort through differing contractors between 2001 and 2013. Three (3) different manufacturers were utilized to install the horizontal lifeline system and railed trolley systems. Fall Protection manufacturers were:
Hy-Safe Technologies, Union Grove, WI Approximately 40% of horizontal and trolleys Pro-Bel, Toronto, Canada Approximately 20% of horizontal and trolleys Safety Systems of America, Sanford, NC Approximately 40% of horizontal and trolleys
Design of the beams (including flange thickness & web thickness), splice joints, expansion joints, and trolleys vary by manufacturer. Previous contract included furnishing and installing a standardized trolley to work on all three rail systems. All legacy trollies have been replaced with seventy (70) standardized trollies by Safeguard Industries.
This project, which is to obtain services to include inspection, repairs, and certification for the various Pentagon Fall Protection Systems, is solicited as a firm-fixed-price contract with one annual contract line item number (CLIN) that is for the purchase of Indefinite-Delivery, Indefinite-Quantity (IDIQ) work. The resulting contract will be a Single Award, IDIQ contract.
1.2 Project Location
The principal place of performance is at the Pentagon, 1155 Defense Pentagon, Washington, D.C. 20301-1155.
The term "Pentagon" means that area of land (consisting of approximately 227 acres) and improvements thereon, including parking areas, located in Arlington County, Virginia, containing the Pentagon Office Building and its supporting facilities (10 U.S.C. 2674(f)(3)).
The project is located at the following site: Pentagon Reservation: Arlington, VA
1.3 Period of Performance
The Period of Performance (PoP) for this project must not exceed one (1) year for the Base period with four
(4) one- year options. The Contractor must refer Performance Work Statement (PWS) Attachment A2: “FSD Construction Management General Requirements” Sections 2.2.2D.1.f and 5.0 for post award scheduling requirements and Contract Completion requirements.

Repair Plan to include all repairs and modifications necessary to ensure system stays in compliance with ANSI Z359 & OSHA Title 29 CFR Part 1926 Subpart M; all parts meet manufacturer’s specifications and design intent for fall protection systems; or recommend replacement of parts that do not meet manufacturer’s specification. Repairs to include but not limited to painting of monorail system, tensioning of horizontal lifeline system, refinishing of support posts, rail alignment & adjustment, splice/expansion joint connection plate repair, and lubrication of trolleys.
Plan is to include recommended intervals for tasks. These repairs will be ordered on a separate task order. See Section H for task order guidelines.
II. IDIQ Repairs – Annual Repairs ordered on task orders to include but not limited to painting of monorail system, tensioning of horizontal lifeline system, refinishing of support posts, rail alignment & adjustment, splice/expansion joint connection plate repair, and lubrication of trolleys. Plan is to include recommended intervals for tasks. In the Base Period, these repairs will be based on the Comprehensive Repair Plan.
However, in Option years 1 through 4, potential repairs will be identified by the PBMO, the contractor, and other customers. These repairs will then need to have a statement of work generated, and the project negotiated on a task order. See Section H for task order guidelines.
III. Certification – For each individual system of the Fall Protection System, contractor will be responsible for providing Certifications. Contractor will be responsible for providing annual certifications for the Horizontal Lifeline System for the base and each option year. Contractor will be responsible for Certifications for the Railed Trolley System every 2 years (Base Year, Option Year 2, & Option Year 4). 5.0 PERFORMANCE-BASED MATRIX
5.0 The Performance-based Matrix below summarizes the key performance requirements, standards of performance, and methods of Government Surveillance for this contract.
5.0.1 CPARS: The primary incentive mechanism for this contract, other than payment of contract price, will be the use of the Contractor Performance Assessment Reporting Systems (CPARS) at www.cpars.gov.

A CPARS assesses a contractor’s performance annually (or interim reports, if necessary) and provides a record, both positive and negative, on a given contractor during a specific period of time. These reports remain available to all Government agencies for source selection purposes for a period of three years.
Each assessment is based on objective and subjective factors and supported by program and contract management data, such as cost performance reports, quality reviews, technical interchange meetings, financial solvency assessments, contractor operations reviews, equipment downtime and functional performance evaluations.
