INSTRUCTIONS
Instructions for items other than those that are self-explanatory, are as follows:
Item 1 (Contract ID Code). Insert the contract type identification code that appears in the title block of the contract being modified.

For a solicitation amendment, change order, or administrative change, the effective date shall be the issue date of the amendment, change order, or administrative change.
For a supplemental agreement, the effective date shall be the date agreed to by the contracting parties.
For a modification issued as an initial or confirming notice of termination for the convenience of the Government, the effective date and the modification number of the confirming notice shall be the same as the effective date and modification number of the initial notice.
For a modification converting a termination for default to a termination for the convenience of the Government, the effective date shall be the same as the effective date of the termination for default.
For a modification confirming the contacting officer's determination of the amount due in settlement of a contract termination, the effective date shall be the same as the effective date of the initial decision.

Item 14 (Description of Amendment/Modification).
Organize amendments or modifications under the appropriate Uniform Contract Format (UCF) section headings from the applicable solicitation or contract. The UCF table of contents, however, shall not be set forth in this document
Indicate the impact of the modification on the overall total contract price by inserting one of the following entries:

Government’s Response to Solicitation Questions
Question Number 1 – Do we need to submit specific resumes with this or should we just outline our capabilities?
Government’s Response – The contractors are to submit specific resumes for individuals that they are proposing for this contract, this is important to know the actual qualifications of the professionals and how they fit into the network committee..
Question Number 2 Our company is a Edmonton, Canada, are we eligible to bid?
Government’s Response – Yes
Question Number 3 – To provide the consulting services do we need to travel to the clients location, if so how often?
Government’s Response – As stated in the Statement of Work;
Travel
Consultants may be asked to travel for one-day site visits for the launch of each project and for steering committee meetings for the Blueprint Neurotherapeutics Network. The consultant shall make his/her own travel arrangements. The COR shall assist the consultant in identifying a hotel at an acceptable rate, if an overnight stay is required.
General Requirements
Consultants may serve on Blueprint Lead Development Teams (LDTs), which require a regular time commitment of 3-10 hours per week, including meeting and preparation time. Each LDT meets for approximately 2 hours every 1-2 weeks by teleconference. Consultants may be asked to serve as co-chairs of LDTs.
A consultant serving as an LDT co-chair shall facilitate LDT discussions, work with an NIH project manager by email and telephone to maintain and communicate the overall project strategy to other LDT members, and advise the principal investigator on the preparation of milestone reports. Co-chairs for LDTs are selected based on breadth of expertise and previous experience serving in a comparable role.
A teleconference call with all consultants is held twice per month to discuss programmatic issues of broad interest.
