ATTACHMENT 19 - ADDITIONAL PROPOSAL INSTRUCTIONS
Title: Development of State Health Policy Database for Research (SHPDR)
I. Offerors Questions about the Solicitation/Instructions
Questions should be sent via E-mail to: banderso@mail.nih.gov and will be answered in writing.
Typically questions and answers are posted as a solicitation amendment, at the discretion of the
Contracting Officer. Please review the Statement of Work and RFP carefully before asking for any clarifications.
Questions should be submitted by: April 10, 2012
The instructions below are in addition to the primary SECTION L - INSTRUCTIONS, CONDITIONS, AND NOTICES TO OFFERORS regarding submission of the Technical proposal and the Business/Cost proposal. You should carefully review SECTION L, SECTION M – EVALUATION FACTORS FOR
AWARD, and the STATEMENT OF WORK (attached to the solicitation) when preparing offers.
II. Additional Technical Proposal Instructions
Note 1 to Offerors (508 Compliance): Additional information about testing web site contents/documents for Section 508 compliance, including specific checklists, by application, can be found at: http://www.hhs.gov/web/508/index.html under "helpful resources."
Note 2 to Offerors: (Phases): The proposed phases and sequence of effort in the Statement of Work is suggestive and is not prescriptive; offerors may propose alternate phases/ modifications to this approach, with explanation. The Contractor is encouraged to provide his/her own innovative ideas/approaches/methodology for addressing the contract objectives and to ensure a useful research database of CY2010 is completed. However, the deliverables described in the Statement of Work are required.
Note 3 to Offerors (Timelines): Offerors must propose timelines for completion of each task under this contract (including each potential update); in addition, a timeline should be proposed for each of the mailto:banderso@mail.nih.gov http://www.hhs.gov/web/508/index.html
Attachment 19 – Additional Proposal Instructions Page 2 of 4 phases proposed in Task 1. (The government estimates that Task 1 can be completed within two (2) years of contract award and each of the updates accomplished within one (1) year from start.)
Note 4 to Offerors (Transition Plan): Submit a general plan/approach with your proposal, for migration of the database, assuming this will be required at the end of the 5-year period. A more detailed Transition
Plan will be required as a deliverable after contract award.
III. Additional Business Proposal Instructions
The business proposal must contain sufficient information to allow the Government to perform a basic analysis of the proposed cost or price of the work. This information shall include the amounts of the basic elements of the proposed cost or price. These elements will include, as applicable, direct labor, fringe benefits, travel, materials, subcontracts, purchased parts, shipping, indirect costs and rate, fee, and profit.
Note 1 to Offerors (Tasks):
The Tasks described in each Task Area in the Statement of Work are the actual tasks that will/may be assigned, based upon the government’s needs. It is anticipated that Task Area 1 will be awarded with award of the contract; other Task areas are indefinite and will be initiated upon approval of task orders.
The exact schedule or quantity of work in this IDIQ contract is unknown.
Note 2 to Offerors (Required Budgets and Assumptions):
Budget Assumptions:
1. Use your proposed timelines (i.e. the length of time you estimate to complete Tasks) for the development of budgets (not necessarily the Government estimates of up to 2 years to complete the 2010 database, and up to 1 year for each update).
2. Use the common start dates listed below for each Task.
3. For preparation of budgets, Task 1, Task 2 and Task 5 have some overlap in year 2 and the effort for these tasks is concurrent; this may actually be the case during contract year 2, (e.g., where there is effort to complete Task 1and startup for Tasks 2 and 5 will be initiated).
4. For the purposes of preparing a cost proposal for the expert panel meetings, assume $1,500 for each non-federal participant requiring travel, subsistence, and lodging.

2. You must provide the following five (5) separate budgets, using the assumptions listed, in Excel spreadsheet format:
a) TASK 1 – provide a fully-loaded budget, by phases, based upon your timeline for completion of this task, assuming work will start with award of the contract (August 1, 2012).
b) TASK 2 (CY2012 Update) – provide a fully-loaded budget, based upon your timeline for completion of this task, assuming work will start on August 1, 2013.
c) TASK 3 (CY 2014 Update) - provide a fully-loaded budget, based upon your timeline for completion of this task, assuming work will start on August 1, 2015.
d) TASK 4 (CY2016 Update) - provide a fully-loaded budget, based upon your timeline for completion of this task, assuming work will start on August 1, 2016.
e) TASK 5 (Database Maintenance and Helpdesk Services) – provide a fully-loaded budget, based upon providing these services for 12 months beginning August 1, 2013.
Note 3 to Offerors (Narrative Discussion): Important - provide a narrative with the cost proposal explaining how costs for each Task and cost category were developed, the basis of the estimates, and why they are considered fair and reasonable.
