SUMMARY OF CHANGES
1. (a) In accordance with Federal Acquisition Regulation (FAR) 15.307, this amendment represents the conclusion of discussions. You are hereby given an opportunity to submit a fmal proposal revision. The common cut-off date for receipt of final proposal revisions is identified in paragraph 5 below. Final proposal revisions shall be in writing and the Government intends to make award without obtaining further revisions.
(b) The fmal revised proposal will be the basis for the fmal evaluation; therefore, final proposal revisions shall be submitted in the same format as the original proposals. Offerors shall incorporate responses to the evaluation notices into their revised proposals. Offerors are reminded that Initial Responses to Evaluation Notices (EN) demonstrated an understanding of the EN. Only final proposal revisions will be used for evaluation purposes.
(c) To assist in the evaluation process, offerors shall include a matrix summarizing all revisions made to the original proposal (matrix will not be counted against the page limit). Page and font sizes remain unchanged (see Section L).
(d) Offerors shall submit copies of the proposal volumes that have been revised or provide replacement, and/or additional pages to be inserted into the original proposal. If a proposal volume has not been revised from the original proposal, offerors are not required to submit additional copies of that volume. However, offerors are requested to indicate in their cover letter to the proposal, which volumes have and have not been revised, and instructions for inserting and/or removing pages from the original proposal.
The following are clarifications/changes to the solicitation.
2. Equipment identified in Attachment 02, pages 15 and 29 (Maintenance Truck and Oil Vac Trailer) will be available for use exclusively by the contractor.
3. Section J, Attachment 11, supersede Sample Task 2 with the attached Sample Task 2 dated 08 Ju12008.
4. Section M, page 68, supersede para. 3.a. with the following.
Cost/Price will be evaluated for reasonableness, realism and completeness. The cost/price factor will not be rated.
The cost/price proposal will contain two sections. The first section will address CLINs 0001-0004 and will contain labor rates for each labor category proposed, per year, per location. These rates will be analyzed in accordance with FAR 15.404-I(c) to determine if they are in accordance with the Davis Bacon Act and NM state law. The second section will address the sample tasks and will contain a total price for each sample task. This section of the proposal will be analyzed in accordance with FAR 15.404-1(b), and will be evaluated forreasonableness.

In accordance with FAR 52.215-1, Offerors are responsible for submitting proposals, and any modifications or revisions, so as to reach the Government office designated in this paragraph by the time specified. Any proposal, modification, or revision received after the exact time specified for receipt of offers is "late."

SAMPLE TASK 2 - RECONFIGURE BURSTER SLABS AT
LARGE TEST STRUCTURE 2 (LTS-2)

SECTION 1 - SCOPE
1.1 BACKGROUND.
1.2 OBJECTIVE. The objective of this delivery order is to remove the existing burster slab and remove the 8' soil layer within the limits shown on the drawings, and construct a 46" thick burster slab with a 72" earth cover on top of LTS-2.
1.3 SCHEDULE. The Contractor shall begin work under this delivery order upon notice to proceed (NTP).
All work shall be completed within three (3) months.
SECTION 2 - APPLICABLE DOCUMENTS

2.4 Bring discrepancies between this document, the drawings, and specifications as well as between those documents and the basic Contract, to the immediate attention of the COTR.
SECTION 3 - REQUIREMENTS.
3.1 PARTS. Not used.
3.2 CONSTRUCTION. The Contractor shall provide all labor, materials, and equipment (except as noted) required to accomplish the work.
3.2.1 Part I: Remove existing burster slabs and remove existing 8' soil layer lAW the attached removal drawings.
3.2.2 Part II: Construct burster slab with earth cover over LTS-2 to include the following:
(1) Construct a reinforced concrete burster slab above Bays 2 through 4 lAW Drawing SAMPLE-LTS 2, Sheet 3, and the specifications. Provide roofing felt on top of the existing concrete roof to prevent the new concrete from adhering to the existing concrete. Also, provide a keyed joint lAW Drawing SAMPLE-LTS 2, Sheet 4, and the specifications.
(2) Place previously removed soil in 8" lifts (max) over Bays 1 through 4, backfilling and compacting lAW the drawings and the specifications. Restore the soil ramp on the west side of LTS-2 to its original condition to allow equipment access to the top of the structure.
3.2.3 The Government shall provide all concrete (including all raw materials) at the Government owned concrete batch plant.
(1) The Contractor shall provide a 3.5 cubic yard bucket loader with operator at the batch plants to charge the aggregate hoppers in support of concrete production.
(2) The Contractor shall provide operators and concrete transit mixers and shall be responsible for the transportation of the concrete from the batch plant to the job site (approximately 5 miles one way) and its placement.
SECTION 4 - SPECIAL REQUIREMENTS/CONDITIONS
4.1 The wage determinations included in the basic contract shall be used for work covered in this delivery order. If overtime is required, such costs shall be clearly shown in the Contractor's cost proposal.
4.2 Reports and Submittals.
4.2.1 The Contractor shall submit a hazard analysis for the project and update (as required) the site safety and health plan for construction events included in this delivery order lAW Specification Section 01110 (Saf~ty, Health, and Emergency Response) of the basic contract.
4.2.2 The Contractor shall submit a project schedule lAW Specification Section 01310 (Project Schedule) of the basic contract.

4.2.3 The Contractor shall submit a complete set of as-built survey data to the Project Engineer NLT 3 days after construction is complete.
4.2.4 The Contractor shall record and submit the soil compaction results measured by others at each lift of soil layer backfill together with the date and location where measured.
