Outlook Nebraska is a nonprofit National Institute of the Blind (NIB) associated agency that provides job opportunities and training for the blind and visually impaired. We operate under the AbilityOne program which provides preferred vendor status. Adding this Federal Acquisition Regulation 58.208-9 to the contract greatly enhances our mission and ensures the service providers compliance with the FAR Act. Let me know if I can answer any questions.
Response #1:
Offers for this procurement, which is 100% set-aside for Service-Disabled Veteran-Owned Small Business
(SDVOSB) concerns, are solicited only from SDVOSB concerns, as such, offers received from concerns that are not
SDVOSB concerns will not be considered.
Question #2:
What is the incumbent contract price?

Question #3:
The store operational hours / contractor work schedule in the PWS states that RSHA is conducted on Monday, Wednesday, Friday, and Saturday, however the FDS Distributor Schedule (Exhibit 4-3) states that RSHA is only conducted on Monday, Wednesday and Friday. Please advise which is correct.
Response #3:
This amendment addresses the removal of 4:00pm-7:00pm (SAT) from the Contractor Work Schedule for RSHA to coincide with no deliveries to handle for that day at Exhibit 4-3 FDS Distributor Delivery Schedule.

Question #4:
I was wanting to get any and all FOIA info for total pricing on the last award of this contract for Whiteman Air
Force Base Commissary stocking. Do you have the total award price for this current/ending contract or a copy of that award I can review. I was unable to find it on FBO. We are trying to get an understanding of the scope of work and number of employees currently in place that we would have to hire on as part of the contract requirements to run this program. As all current employees who would wish to do so would convert over to our payroll if we are the successful bidder.
Response #4:
a. Refer to the information provided in the response above regarding prior contract award pricing.
b. The below information is provided for submitting a request for Agency records. Prior to submitting a request, please visit the FOIA Electronic Reading Room to see the document you are seeking that has been publicly made available. Please review the FAQs before submitting a request.

c. To assist with understanding general work effort required during performance, interfacing with the
Government is permitted in accordance with FAR 52.237-4500 which provides for an Observation Period at the option of the awardee that may be up to two (2) weeks prior to full take over of the contract.
d. FAR 52.222-17 Non-displacement of Qualified Workers requires the incumbent (if not the awardee) to furnish a certified listing of all service employees working under the existing contract (and subcontracts) not less than 30 days before expiration of the contract. The Awardee shall offer employment to those employees and ensure they have a right of first refusal of employment under the contract in positions for which they are qualified.
Question #5:
Day custodial starts one hour before day stock, but always end one hour before stock starts. Will the government be performing the duties that the contractor is expected to be doing during the contractor operating hours?
Response #5:
The Government requires contractor personnel to be available to perform Day Custodial tasks during all of the hours indicated for day custodial operations when custodial care is generally limited to rapid response and quick fix action necessary to maintain clean and safe conditions.
Question #6:
Why does day custodial start prior to day stock hours are there any other tasks to do except clean up? snow? and bathroom checks?
Response #6:
DeCA policy guidance has established times for Day Custodial to correlate with a two-hour delay start time and end-time of one-hour prior to store closure. Day custodial tasks also include maintenance and resupply of restrooms, empty trash receptacles, dispose of waste materials, removal of debris and litter, replenish supplies to prevent outages, replace light bulbs, and report facility or equipment defects. Remove unsafe conditions in commissary and outside areas.
Question #7:
How does the government dismantle displays, is it in grocery carts, or boxes (original boxes, or repack boxes). If the merchandise is in repack boxes is the box labeled with nomenclature? expiration date? Will there ever be different items commingled in the same box?
Response #7:
When displays are dismantled by the Government residual display merchandise is repacked in boxes that may include one or more line items which are assigned by aisle. Government personnel may also label the box with the product name; case quantity; and/or UPC to identify similar products. To maintain the standards for shelf stocking, it is the contractor’s responsibility to rotate display stock.
Question #8:
Where are these repack cases stored. Is the contractor responsible for storing and pulling these residual display cases ?
Response #8:
The residual display cases are located by aisle within the RSHA. Making disposition of residual display merchandise is not included in this requirement, therefore storing residual stock from displays does not apply. This amendment revises Section C-4 Specific Tasks to remove verbiage from paragraph 4.4.2.7. Store Merchandise.
Residual display merchandise dispositioned by other than contractor personnel is included in pull merchandise.
Question #9:
Will the government ensure that the baler is not full and that if it is full that a bale is tied prior to the beginning of the contractor shift?
Response #9:
Government employees will process bales during hours when contractor personnel are not scheduled to work.
Question #10:
Please identify which offices are the responsibility of the contractor to clean.
Response #10:
The square footage (2,086) and floor types (VCT, Carpet) for the office areas are identified on the chart of Exhibit
4-6-2 Night Custodial Surveillance Frequencies/Quality Standards PRS(7). Refer to Exhibit 4-7 Facility Layout
(room number legend) that lists the below office areas. This amendment revises Exhibit 4-7 Facility Layout to remove Medical Food Inspection Office (VET) and replace with Grocery Manager’s Office.
Office Areas
Store Director’s Office
Administrative Reception
Administrative Area
Training room
Store Manager’s Office
Receiving Manager’s Office
Produce Manager’s Office
Meat Manager’s Office
Grocery Managers Office
Question #11:
4.1.1. Indicates that the Government must inform the contractor of unsatisfactory work and therefore provide an opportunity for re-performance. Furthermore this paragraph indicates that the contractor may be responsible for the cost of re-inspection, please quantify this amount.
Response #11:
Joint inspections (QAE and PM) are encouraged. However in cases when surveillance results indicate defective performance, the costs associated with Government re-inspection of work for defective services is based on the
Quality Assurance Evaluator’s (QAEs) labor rate and total time to re-inspection.
Question #12:
When items on shippers expire, is the contractor also responsible for payment of these items under the “damage caused by contractor for failure to rotate” paragraph #6.1?
Response #12:
The expired merchandise damage that can be attributed to the contractor applies to merchandise the contractor is responsible for stocking, not the items on shippers which vendors are responsible for stocking.
Question #13:
Are items located on shippers and pallet displays also considered as part of the damage, when considering expiration dates?
Response #13:
Merchandise the contractor is responsible for stocking, which includes displays and not the items on shippers, is attributed to the contractor when considering expired merchandise damage.
Question #14:
1. a) Reference: Proposal Submission (Format & Content), Para (a) General (2), RFP Page 77, which states: “To ensure consideration of all required information by the various evaluators, you may have to present the same information, in the same or different format, in more than one volume. Ensure information and statements on similar topics (e.g., number of work hours and productivity rates) are consistent throughout the proposal.”
b) Comment: The stated guidance is elusive. Required information in the respective Volumes I, II and III should be clearly and unambiguously stated. Leaving the interpretation of ‘all required information … in the same or different format’ up to the offeror presents a high probability of inconsistencies and distorted evaluations.
c) Question: Will the government please be exact and descriptive on the required information – by Volume?
Response #14:
The same information (e.g., number of work hours and productivity rates) that may have to be presented consistently throughout the proposal in electronic format (Microsoft Excel) in more than one volume includes Volume I, Cost
Breakout and Volume II, Typical Daily Work Schedule and Direct Labor Summary. The same information that may have to be presented in different format (Microsoft Word) includes Volume II, Technical Capability and Personnel
Proposal. The proposal must be submitted in three distinctly severable parts consisting of Volumes 1, II, and III as follows:.
VOLUME 1 (original and 2 copies with CD of Cost Breakout)
Section A
Complete blocks 15-18 of the Standard Form 33, Solicitation, Offer and Award, acknowledge receipt of amendments, and sign by an authorized representative on behalf of the offeror.
Section B
The Section B (Supplies or Services and Prices/Costs) details proposed pricing for the base and each option period of work effort for subline items that shall be consistent with the Cost Breakout. The Section B shall be consistent with the annual totals generated in the Cost Breakout.
Section K
The Representations, Certifications and Other Statements of Offerors located in Section K of the solicitation is completed by the offeror. The System for Award Management (SAM) is checked if the offeror is currently registered and has completed the Representations and Certifications section of SAM electronically, to ensure the
North American Industry Classification System (NAICS) code corresponds to the solicitation as updated by the offeror through maintaining an active registration in the SAM database.
Cost Breakout
Attachment 4, Cost Breakout, which must be submitted in hard copy as well as on a compact disk (CD), details proposed costs for the base and each option period of work effort that shall be consistent with Section B. If there are mathematical calculation differences between the hard copy and CD, the hard copy shall take precedence.
VOLUME II (original and 4 copies with CD of Direct Labor Summary)
Technical Capability and Personnel Proposal
Attachment 5, Technical Capability and Personnel Proposal, offeror provides requested information, including the question first followed by responses that may be narrative, charts, diagrams or other visuals that clearly and accurately represent the response. Pages in excess of the number specified for the volume will not be considered.
Typical Daily Work Schedule (TDWS)
Attachment 6, Typical Daily Work Schedule, offeror completes TDWS form by entering data requested, ensuring data entries agree with the information provided in other parts of proposal and computations are correct.
Direct Labor Summary
Attachment 6, Direct Labor Summary (DLS), which must be submitted in hard copy as well as on a compact disk
(CD), offeror completes the DLS form by entering data requested, ensuring data entries agree with the information provided in other parts of proposal and computations are correct. If there are mathematical calculation differences between the hard copy and computer disk, the hard copy shall take precedence.
VOLUME III (original and 2 copies)
Attachment 7, Past Performance Information, offeror provides the below information:
1. Reference for each contract completed during the past three (3) years and each contract currently in process.
2. Copy of each non-federal government customer notification letter that authorized customer to provide past performance information to the government.
3. Newly formed entities without prior contracts should list contracts as required above for all key personnel.
4. Submit past performance information for companies that have changed names under any prior company names.
5. Submit past performance information from all companies forming the joint venture and indicate the estimated percentage of contribution being made by each company.
Question #15:
2. a) Reference: Proposal Submission (Format & Content), Para (a) General (3), RFP Page 77, which states: “Page limits apply to volumes and sections as indicated below. Beginning with the first page, only the number of pages up to the specified limit will be considered. Tabs and dividers will not be considered in the page count. Pages in excess of the number specified for the volume will not be considered.”
b) Comment: This statement implies a possible Volume Cover, Cover Letter, Table of Contents, and List of
Abbreviations and Acronyms (e.g. typically non-page counters generally presented as pages i thru iv, etc.) will be counted towards the Volume II (Technical) 24-page limit.
c) Question: Will the government please clarify and confirm the typically administrative pages such as a Cover
Page, Cover Letter, Table of Contents, and List of Abbreviations/Acronyms will not count towards the 24-page
Volume II limit?
Response #15:
The items not considered in the page count are Typical Daily Work Schedule, Direct Labor Summary, tabs, dividers, and pages of Volume II that are not sequentially numbered beginning with page one (1) up to the specified limit of
24 single-sided or 12 double-sided pages.
Question #16:
3. a) Reference: Proposal Submission (Format & Content), Para (a) General (4), RFP Page 77, which states: “The
Cost Breakout and Direct Labor Summary must be submitted in hard copy as well as on a CD in Microsoft Excel
2003” and further mention of “Microsoft Word 2003 for the Past Performance Data Form.”
b. Comment: Microsoft has released four newer versions of Excel and Word since 2003. The newer versions are user-friendlier where older versions can be cumbersome and can create format errors and issues if importing baseline data or information that was created in a newer version. In fact, we are having difficulty now with the macros and printing the Cost Breakout and Direct Labor Summary … with strong suspicion the outdated Excel version is causing the difficulty. While it is probably too late for this solicitation, suggest DeCA convert and update your spreadsheets prepared forms to the latest versions of Excel and Word (at least 2013).
c. Question: None.
Response #16:
The electronic format indicated for the Cost Breakout and Direct Labor Summary may be submitted in the latest version of Microsoft Excel.
Question #17:
4. a) Reference: Proposal Submission (Format & Content), Para (c) VOLUME II with, in parentheses: “original and
4 copies with CD of the Direct Labor Summary”; Para (c)(1), Line 4, which states: “This volume is limited to 24 single-sided or 12 double-sided pages not including the Typical Daily Work Schedule and Direct Labor Summary forms.” and Para (c)(2) TYPICAL DAILY WORK SCHEDULE, which speaks to data-entry into the Attachment 6
Excel spreadsheet.
b) Comment: The reference in Para (c) does not mention the Typical Daily Work Schedule as a file on the CD; Para
(c)(1) implies a printed version of the Typical Daily Work Schedule; while the reference in Para (c)(2) implies a data file. It is not totally clear if a hard copy and/or digital CD versions are required.
c) Question: Does Volume II require a printed Typical Daily Work Schedule and/or a digital version on the Volume
II CD?
Response #17:
Data entries are entered in the electronic format (Microsoft Excel) indicated for completing the Typical Daily Work
Schedule (TDWS). The submission requirements for the TDWS are needed in hard copy only.
Question #18:
5. a) Reference: Proposal Submission (Format & Content), Para (d) VOLUME III, (1) Reference Requirements, RFP
Page 78, which states: “Provide a reference for each contract completed during the past three (3) years and each contract currently in process using the form provided at Attachment 7.”
b) Comment: To provide a reference for each (and every) completed and current contract within the past three years is extreme; we provide diverse services and hold a number of small contracts. Most federal acquisitions ask offerors to provide up to three or five contracts that are recent (within three years) and relevant (similar in size, scope and complexity) where the source selection authorities, in conjunction with access to CPARs, etc., are able to properly assess a company’s ability to manage projects and programs in efficient and effective fashion.
c) Question: Will the government consider revising the ‘each’ (which implies every) discriminator and limit past performance references to a set number of reasonable (recent & relevant) reviews?
Response #18:
DeCA will use references that include contracts entered into with the Federal Government during the past three (3) years and each contract currently in process, in addition to the past performance information contained in the Past
Performance Information System (PPIRS) that is within three years (six for construction and architect-engineer contracts) of the completion of performance of the evaluated contract or order, and information contained in the
Federal Awardee Performance and Integrity Information System (FAPIIS) e.g., terminations for default or cause.
DeCA will use references that include contracts entered into with agencies of state and local governments, and commercial customers. Past performance information from those non-federal government customers, newly formed entities, companies that have changed names, and joint ventures should list each contract completed during the past three (3) years and each contract currently in process. If more than one contract is with the same customer, only send that customer one notification letter.
Question #19:
6. a) Reference: RFP Attachment 7, Past Performance Information.
b). Comment: The attachment is referred to as a ‘Form’ implying it must be ‘filled in’ and collated in Volume III at the rate of one-page per contract example. However, the ‘Form’ has some inefficient aspects: The ‘Customer
Address’ offers four lines when two might suffice; the address is likely duplicated under ‘Customer Point of Contact
Address’ later in the ‘Form’; the ‘Point of Contact’ section requests a Fax number and not an e-mail address; the
‘Description of Service/Product (Include sufficient details to indicate the variety of tasks, standard of care, and degree of tolerance permitted):’ offers two lines of text to provide such ‘sufficient detail’ and there is no need to include instructions per se in the ‘Form’ itself; the ‘Problems Encountered and Corrective Actions Taken’ portion offers ten lines of text, when fewer might do; etc.
c. Question: Will the government accept an offeror-developed format or revised ‘Form’ that provides the same information as in Attachment 7 (e.g. Customer address, period-of-performance, contact value, etc.) with more space/additional emphasis on providing the description (to assess similarity in size, scope, and complexity); and other applicable info (e.g. e-mail), within a one-page limit?
Response #19:
Reference information is limited to 1 single-sided page per reference using the required form.
Question #20:
7. a) Reference: Proposal Submission (Format & Content), Para (d) VOLUME III, PAST PERFORMANCE
INFORMATION, (5) PAGE LIMITATIONS AND NUMBERING, RFP Page 78, which states, “Reference information is limited to 1 single-sided page per reference using the required form.”, and, Section M, PROPOSAL
EVALUATION, (b) FACTOR – PERFORMANCE CAPABILITY, and Sub-factor: Past Performance, RFP Page
81.
b) Comment: To expound on the previous question and the purpose of Volume III where the Performance Capability
Factor is approximately equal to Price, the ‘Form’ offers only two lines for ‘Description’ and 10 lines for ‘Problems
Encountered’ in order for the source selection authorities to (per Para (d)(1) (i) thru (iv) on RFP Page 82) essentially ascertain:
(i) similarity or dissimilarity of size, scope, and complexity of experience to current contract requirements, and recentness of performance;
(ii) the offeror's demonstrated ability to provide quality services;
(iii…
