FORM “A” SPECIALIZED PROJECT EXPERIENCE AND PAST PERFORMANCE

READ CAREFULLY: The purpose of this form is to provide supporting project information for Evaluation Factor 1, Project Experience and Past Performance. This form is to be completed by the offeror. The offeror is to list construction projects performed from May 2010-RFP that are similar to the work described in this solicitation. It is especially important that the offeror disclose instances in which their past performance may be considered by others to be less than fully satisfactory. The offeror should tell their side of the story and describe remedial corrective action that has been taken or will be taken to correct the deficiency. Failure to do so may result in a determination that an offeror has been less than candid with the government, which could result in an unfavorable assessment of the offeror’s past performance record. Fill in the information by typing or printing in readable font. Form “A” consists of 2 pages per each project..

Form A (con’t) PROJECT SPECIFIC INFORMATION (Should reflect specific DETAILED project information as it relates to the offerors ability to successfully meet the standards for Evaluation Factor 1: Project Experience and Past Performance for purposes of this contract. Narratives should only be 1 page. Narratives should include but not be limited to: modification and change order management, subcontractor management efforts utilized on the project; any unique approaches that added to the quality of the final project, and any cost management efforts and schedule development , design services summary (including % of design), design concept, any value ads as well as AE coordination/communication.

FORM “B” MANAGEMENT AND TECHNICAL APPROACH

Read Carefully. The purpose of this form is for the offeror to provide evidence of its ability to manage both scheduling and budget aspects of a construction project. Using this form, offerors are expected to provide an example of a project where schedule and costs were different than initially projected. The offeror should provide a separate page for the schedule example, as described in the solicitation. FORM B: Management & Technical Approach in a reasonably readable font.

FORM “C” MANAGEMENT & TECHNICAL APPROACH

Read Carefully. The purpose of this form is for the offeror to provide evidence of its ability to manage all aspects of projects that will usually be encountered on task orders under this contract. Offerors will be expected to provide detailed and specific information with regard to their responses to specific management and technical information in the following core areas: Risk Management, Resources and Technology, and Sustainability. The offeror shall demonstrate a clear concise understanding of the contract requirements in its detailed explanations. FORM C: Management & Technical Approach in a reasonably readable font.

READ CAREFULLY: The purpose of this form is to provide information for Evaluation Factor 3: Key Personnel and Staffing. This form is to be completed by the offeror. The offeror is to give professional background and experience information about the qualifications of those individuals currently employed by the firm who have performed work on at least one of the “similar” projects outlined in Evaluation Factor 1: Project Experience and Past Performance, and is expected to work on task orders under this contract.

READ CAREFULLY: The purpose of this form is to provide supporting information for Evaluation Factor 4: Project Location. This form is to be completed by the offeror. The offeror should provide general information about one (1) project for each geographic location that shows they are able to perform construction related services in each of the 5 geographic areas of Region 6. Because contracts will not be awarded based on a geographic region, it is important that contractors awarded this contract have the capabilities and necessary resources to compete for projects (task orders) throughout the entire GSA Region 6. Offerors will provide information indicating they have acted as either the prime contractor or subcontractor providing construction services on projects located throughout the 5 major geographic areas of the Region as specified below.
