Attachment 1 PERFORMANCE OBJECTIVE: C.5.1 Sample Security, Identification and Processing Order

If Performance Standards are achieved

Section C.5.1.2: Contractor shall if required provide chain-of-custody for samples submitted by USGS.

Contractor shall document the signature, date and time of the person receiving custody of the samples. The status of the custody seal must also be noted on arrival.
COR will perform 100% inspection of samples requiring this chain of custody. If proper documentation is not available, this may result in an unsatisfactory rating.
Full payment of invoices, if proper documentation is furnished as requested.

Sections C.5.1.1 FISMA requirements

Contractor shall have covered employees take end-user computer security training and sign non-disclosure statement. Contractor must have effective information security controls.

COR will check training records and copies of non-disclosure statements to be sent to the COR.

Full payment of invoices if all FISMA requirements are met

Sections C.5.1.4 Receipt of samples and log-in

Contractor must be able to receive samples directly from field. Contractor will log-in samples according to requirements in contract.

COR will check weekly report with ASRs. Failure to receive samples properly and send weekly report at least 95% of the time may result in an unsatisfactory rating.

Full payment of invoices.

Section C.5.1.4.3 - Sample processing order
Samples are to be processed on a first-in first-out basis unless authorized otherwise.
COR will check reporting order. If this requirement is not followed (without approval from the COR) an unsatisfactory rating may be given

Section C.5.2.2.

Analyze samples according to methods listed in contract.

Contact COR 100% of the time if problems with sample analyses are encountered. Any modifications require approval of the COR before implementation. Use of modified or alternate methods without prior approval from the COR is considered a critical defect.
COR will perform 100% inspection of data deliverables and will conduct technical review on 100% of the analytical-results reports. In some instances reruns may be required. COR will keep formal records of any correspondence with contractor on requests for approval to use modified methods.
Full payment of invoices. If Performance Standards are not met, partial payment of invoices may occur.

Section C.5.3.2 Internal Quality Control

Contractor shall analyze a blank, a laboratory control sample, and a duplicate with every analytical batch.

COR will perform 100% inspection of data deliverables and will conduct technical review on 100% of the analytical results reports to ensure the requirements are met. If standards are not met, an unsatisfactory rating may be given.
Full payment of invoices if QC criteria are met. Partial payment may occur if exempt QC criteria (specified on list of QA/QC criteria) do not meet contract requirements.
Section C.5.3.3 External Quality Assurance
Approximately 5% of samples delivered to contractor will be QA blind samples
COR will perform 100% inspection of deliverables. If QA samples (which may be duplicates) do not meet acceptance criteria, reruns may be requested for some of the samples or for the entire batch.
Full payment of invoices if QA criteria are met.

If Performance Standards are achieved

Section C.5.4: Prepare and provide Analytical Results reports, Raw Data reports, and electronic data deliverables (EDD) files in specified format for each SDG.
Data deliverables (Analytical Results report, Raw Data report, and EDD files) are properly prepared for each SDG and do not require changes or revisions in at least 95 percent of the reports received by the COR during a quarter. In most cases, required revisions to data deliverables will be completed within three business days by the Contractor after notification from COR.
COR will perform 100% inspection of data deliverables and will conduct technical review on 100% of the analytical results reports COR will notify the Contractor if specified data deliverables are incomplete or revisions are needed. If more than 5% of the SDGs require revisions to the data deliverables during a quarter, an unsatisfactory rating will be documented in quarterly evaluation worksheet prepared by the COR.

Full payment of invoices if EDDs are properly and completely formatted.

If specified data deliverables for a SDG are not properly prepared, payment will not be approved until data deliverables are revised and completed at no cost to the Government.

PERFORMANCE OBJECTIVE: C.5.5 Provide Analytical Results of Sample Analyses and Data Deliverables within Specified Time Schedules

Section C.5.5 Provide analytical results and complete data deliverables within specified normal 45 day turn-around-time.

Data deliverables for each sample delivery group (SDG) will be completed within specified TAT..

COR will track and monitor the delivery of all data deliverables to the USGS. If more than 10% of the data deliverables are delivered past required time schedules during a quarter, an unsatisfactory performance rating will be documented in the evaluation worksheet (see section C.7.5) prepared by the COR.
Full payment of invoices if turn-around-times are met. Liquidated damages ( C.5.6.1) will be imposed for late data delivery.

Section:C.5.6 Provide data that fulfill the QA/QC acceptance criteria.

Chemical yields, spike recoveries, and blanks shall fulfill requirements in Section C.5.6

Duplicate analyses shall fulfill QA acceptance criteria.

Other criteria will be met as required.

The COR will review 100% of the data reported. Contractor will be notified if QA criteria are not met. .

QA performance criteria evaluations are listed in C.5.6 Full payment of invoices if criteria are met.

Partial payment shall occur for data deemed usable but which requires qualifiers.

Subsection C.5.7: Provide a primary source of contact.

The Contractor will also have other personnel as listed in MARLAP E 4.2 Personnel

The contractor shall have available 100% of the time a contact to act for the Contractor on contract matters that relate to the daily operations of the contract.

The Contractor shall have a full time radiochemist available that can handle complex or problem samples. The contractor shall also have sufficient qualified laboratory supervisors and data reviewers.

Unavailability of a contract contact will be evaluated as unsatisfactory performance and documented in the quarterly PA report.

Failure to be able to handle samples with complex matrices may result in an unsatisfactory rating in the PA report.

Failure to maintain sufficient qualified personnel may result in an unsatisfactory rating in the PA report.

Full payment of invoices.

Payment will be denied for samples with unacceptable data.

Partial payment will occur for data deemed usable but which requires qualifiers.
