STATEMENT OF WORK

INTRODUCTION AND SCOPE OF WORK

I. This service contract is to provide all labor, tools, material, and special equipment necessary for a contractor to perform Annual Backflow Testing and Certification as required by the City of Riviera Beach and Florida Department of Health. All testing and certification will be performed at WPB VA Health Care System, 7305 N. Military Trail, West Palm Beach, FL. Contractor shall follow all state, local, and federal guidelines. There are 44 backflow preventers of various sizes and manufactures that require testing. In the event such systems and equipment do not meet requirements of the previous reference documents or other applicable documents, contractor will issue certifying documentation subject to listed recommendations (provide a summary sheet detailing discrepancies and recommendations to resolve discrepancies) to bring system and equipment into compliance with aforementioned standards. Work will be performed at the WPB VA Health Care System, 7305 N. Military Trail, West Palm Beach, FL.

II. REQUIREMENTS

Prior to any worker for the contractor begins work, the contractor prepares a safety plan and ensures workers understand and comply with the safety measures while working at VA Health Care System and equipment.

a. Perform an Annual Backflow Testing and Certification

i. Riviera Beach Ordnance Article V, Section 20-236 requires that State Certified Backflow Prevention Technicians perform the annual testing and certification utilizing properly calibrated test equipment designed specifically for testing and certifying backflow prevention devices.

ii. Devices that fail testing and require repair are beyond the scope of this contract. The contractor shall notify the Plumbing Shop Supervisor of any failed devices. Separate arrangements will be made for repair and/or replacement.

iii. The city of Riviera Beach mandates current certification of all backflow prevention devices be completed, and all testing and reports be submitted prior to 1 July of each year. Contractor is responsible to meet this deadline.

iv. After backflow devices have been certified a written report shall be submitted to the City of Rivera Beach and to the Plumbing Shop Supervisor at the VA Health Care System. This report shall include all inspection sheets for each backflow device tested.

III. All work is scheduled to be performed during normal hours of VA operations.

a. The work will take approximately 2 days to complete IV. All written reports will be clear and legible and reflect how the measurements were made, where they were made (locations) and compared with specified standards for each test.

a. Test results will be in an easy-to-read format.

V. DELIVERY

a. Performance of services shall be rendered as identified above.

b. Scheduling of services shall be coordinated with the Plumbing Shop Supervisor, telephone 561-713-6321.

i. Contractor shall report to the Plumbing Shop Supervisor, commencing work.

VI. PROGRESS AND COMPLIANCE

a. Contractor shall perform services on this order only to the extent indicated on this agreement. No additional services/repairs will be made without the approval of the Plumbing Shop Supervisor and the Contracting Officer.

b. Contractor shall provide service tickets, at the time that services are rendered, for all services performed. These services tickets shall be signed by a Plumbing Shop employee in order to confirm services were rendered.

c. All payments shall be made in arrears. Upon receipt of Contractor’s invoices, the Plumbing Shop Supervisor will certify invoices through referenced service tickets, assuring that all services were performed in accordance with contract terms.

VII. Additional requirements:

a. All work shall be in accordance with VA standards, and other related codes.

b. All work shall be completed within 90 days from contract award.

c. All shutdowns shall be coordinated with COR in advance

d. Workmen are subject to all the rules of the Health Care System.

e. Contractor tools and equipment shall be continuously under direct line of sight supervision of the contractor. NO tools / equipment can be left at the individual work sites over night – must put away each day.

f. Contractor shall clean-up work sites DAILY. NO debris can be left at work sites.

VIII. A site visit is highly recommended to allow prospective contractors to verify all existing conditions and include all such implications in the proposal.

a. The Contracting Officer is the only one authorized to schedule any site visit.
