PERFORMANCE WORK

STATEMENT

OVERHEAD, JIB, UNDER RUNNING AND TOP

RUNNING CRANES AND MONORAIL SYSTEMS

Inspections and Repairs

(1 Oct 21)

OVERHEAD AND UNDERHUNG CRANES AND MONORAIL SYSTEMS

1.0. The intention of this solicitation is to obtain hoist inspections and repairs performed by a sound organization with qualified technicians and inspectors, at Luke Air Force Base, Arizona by means of a firm fixed-price contract. Hoist inspections shall be an in-depth assessment, available for all brands of overhead hoists, to help ensure regulatory compliance and assist with safety and reliability.

2.0. General Requirements. The Contractor shall furnish all labor, transportation, equipment, materials, tools, supplies, supervision, incidental engineering, and management required to perform the inspections, testing, maintenance, repair, and component replacement as required to maintain equipment listed in Appendix B with the manufacturer's original specifications. Work includes the performance of service work, preventive maintenance of equipment, routine annual inspections and tests, and other services as described herein.

2.1. Equipment, components, and parts, other than that installed under the contract, shall not be removed or replaced or deficiencies corrected while still under warranty of the manufacturer or the installer without prior approval of the Contracting Officer (CO) or Contracting Officer representative (COR).

2.2. All work shall be performed by tradesmen and craftsmen who have extensive experience and training on inspecting and repairing equipment from a wide variety of crane and hoist manufacturers and as required (if applicable) with OSHA, ASME and Hoist Manufactures

Institute standards and regulations. Evidence of any required licenses, as well as documentation of the qualifications of personnel, shall be provided to the CO prior to award of the contract.

3.0. Preventive Maintenance and Inspection. A certified inspector (CI) shall perform a complete inspection once during each 12-month period of the contract to systems identified in Appendix B. Inspections shall be coordinated with the quality assurance office and facility managers at least 72 hours in advance of the inspection. Deficiencies found during inspections shall be corrected during scheduled maintenance, ensuring optimum uninterrupted systems operations.

Inspections shall be completed not later than due date listed in Appendix C. Exact dates will be provided by COR at contract start.

3.1. The CI shall examine deficiencies and determine whether they constitute a safety hazard. If the crane is to be removed from service until it is repaired, the contractor shall notify the building manager and COR immediately.

3.2. Periodic inspections may require the removal of covers or parts to gain access.

Disassembly of the hoist or major components, not designed to be removed or opened, is not required for the sole purpose of the inspection. If the individual performing the periodic inspection finds evidence of damage or wear that constitutes a potential hazard, further inspection to determine cause or to repair the system may be required and total disassembly or removal of major components may be necessary. Consult manufacturer’s instructions, where available, for additional guidance.

3.3. The contractor shall perform Weight test, Magnetic Particle Inspections/Dye Penetrate Inspections of hoist hooks/other critical components and Functional and Operational test during each scheduled inspection to ensure reliable and continuous safe operation. Equipment listed in Appendix B shall be inspected not less than 100 percent or more than 125 percent of rated capacity.

3.4. Inspections and maintenance work shall be in accordance with applicable OSHA, Air Force OSHA, ASME/ANSI, CMAA and manufacturer's specifications.

3.5. The contractor shall submit dated and signed inspection records electronically to the COR within 10 business days after the inspections were performed. Contractor shall keep inspection records on file and shall be readily available.

3.6. Contractor shall schedule inspections with the facility managers a minimum of 72 hours in advance. Phone numbers and email address will be given to the contractor upon contract award. Several areas are located in a secure area. Facility managers will ensure escorts are provided for the scheduled inspection dates and times.

4.0. REPAIR SERVICE CALLS.

4.1. Contractor shall have a point of contact available during Luke AFB normal business operations with an initial call back response time not to exceed 1 hour after government notification of a repair service call. Repair service calls shall only be accepted by the Contractor from the COR or Luke AFB emergency call desk personnel. For after normal hours, Contractor personnel shall notify Luke AFB emergency desk at 623-856-7083 or 623-856-7231 of their arrival and departure and status of repair.

4.1.1. Routine Repair Service Calls: Will be on an as required basis, with response time not to exceed 8 hours during Luke AFB normal operations, (7:00 AM-4:00 PM), excluding weekends and federal holidays (e.g., since an 8-hour response time is required, and the operating hours are 7:00 a.m. to 4:00 p.m., then a call to the maintenance Contractor at 2:00 p.m. would require a physical response from the maintenance Contractor no later than 1:00 p.m. the following workday).

4.2. The CO or the COR shall notify the Contractor of repairs to be made from customer service calls. The Contractor shall report to the work location, survey the repair, and provide the CO or COR an estimated cost of repair within (2) business days which shall include labor, trip charges (if applicable), and materials required to complete the repairs. The Contractor shall commence repair work after notification and authorization from the CO or COR. All repair work shall be in accordance with normal commercial practices and invoiced separately on the contract. All parts and repair invoices are to be submitted with supporting documents (receipts from suppliers and vendors) acceptable to the COR prior to submitting for payment.

4.2.3. Contractor shall schedule all repairs with the facility managers a minimum of 72 hours in advance. Several areas are located in a secure area. Facility managers will ensure escorts are provided for the scheduled repair dates and times.

4.3. REIMBURSEMENTS

4.3.1. PARTS. Contractor shall be reimbursed for parts and materials purchased by the Contractor to complete repair work for repair and services at market prices as established by suppliers paid invoices. No mark up of prices by the Contractor shall be allowed. When equipment rental has been approved in advance by the COR, Contractor shall be reimbursed for rental of equipment necessary to perform work at market prices as established by supplier paid invoices, with an allowable mark up by the Contractor not to exceed 15%. Only new parts or parts of equal quality that is specified by the hoist manufacturer will be used in effecting repairs. Parts replaced will become the property of the government. The Contractor will return all replaced defective parts to the COR unless the replaced parts are used for trade in as authorized by the CO/COR. Contractor shall supply normal tools of the trade without cost to the government.

4.3.1.1. LABOR. Labor charges will be billed for repair and services for emergency or routine service calls at an hourly rate agreed to in the contract. Contractor will submit a bid of estimated labor charges to CO/COR for approval. Only approved bids will be reimbursed. Labor hours shall start and stop at Luke AFB only.

4.3.1.2. TRIP. A trip charge is defined as the cost of transportation from the Contractor’s place of business to Luke AFB and return. Trip charges shall be for service calls only (this cost is included in PM costs). Contractor shall not charge for additional labor time during travel.

4.3.1.2.1. A trip charge shall not be charged for items related to scheduled contract Preventive Maintenance, unless a proposal has been or will be submitted for work to be authorized.

4.3.1.3. If the COR calls in a service call while the contractor is on Luke AFB, contractor will charge additional labor and decide whether their current schedule can accommodate completion of the call on that day. If the service call pushes the contractors scheduled work into an additional day, an additional trip charge is authorized. Contractor will make determination at time of call, whether the schedule permits the additional call at no cost.

4.4. Supply, Repair or Replace: Supply, repair or replace all parts of every description made necessary by normal wear and tear at no additional cost to the government. Refinishing, repairing, and or replacing bridge, runway track, and trolley are excluded. Devices described are as follows for normal wear and tear:

a. Pendant station push buttons

b. Contact tips

c. Hook safety latch

d. Brake parts

e. Identification tags

4.4.1 The contractor shall have and maintain on hand locally a supply of spare parts sufficient for the normal operation, maintenance, and repair of hoists. Only new standard parts or parts of equal quality shall be used in effecting repairs. The government will reimburse the contractor for replacement parts except as listed in paragraph 4.4.

5. GOVERNMENT FURNISHED PROPERTY AND SERVICES

5.1. FACILITIES. None

5.2. UTILITIES. The Government will furnish electricity, water, and sewage as necessary for accomplishment of work in accordance with this contract. All facilities do not receive the same utility services. The Contractor shall not change or modify any utility system or component; or connect any Contractor property, equipment or system without prior CO review and approval.

5.3. MATERIALS. None

5.4. SECURITY, FIRE, AND MEDICAL SERVICES. The Government will provide police and fire protection. In the event of a medical emergency, base ambulance service for transporting an injured employee to a local hospital is available on a cost reimbursement basis.

5.5. TELEPHONE. None

5.6. SOLID WASTE COLLECTION AND DISPOSAL. None

6.0. GENERAL INFORMATION

6.1. MISSION. The overall Civil Engineer mission at Luke AFB, AZ is to plan, maintain, operate, and protect the infrastructure, facilities, and environment for assigned airmen, families, and civilian employees.

6.2. BACKGROUND (Reserved)

6.3. DAMAGES TO GOVERNMENT BUILDINGS, EQUIPMENT, OR VEGETATION. Damages to Government buildings, equipment, or vegetation are addressed in clause 52.237-2 – “Protection of Government Buildings, Equipment, and Vegetation.”

6.4. ENVIRONMENTAL REQUIREMENTS. In addition to all applicable Federal, State and local environmental codes and regulations, the following specific guidance is provided:

6.4.1. Hazardous Materials. The CO shall approve all hazardous materials brought on-site by the contractor prior to use. The contractor shall obtain approval using the application requirements of the AF Form 3952. Once the process is implemented and contractor is trained, the contractor may submit the AF Form 3952 electronically. The CO has the right to prohibit the use of hazardous materials it deems to be especially hazardous to human health and environment. In the event the CO does not approve a hazardous material for use, the CO may provide the contractor a list of suitable substitutes; however, the contractor shall retain responsibility for finding an acceptable substitute. The contractor shall take appropriate actions to comply with waste minimization and pollution prevention practices and policies.

6.4.1.1. The contractor shall maintain spill control material on hand at all times sufficient to contain a worse case spill, both volume and hazard level.

6.4.1.2. Executive Order (EO) 13148, Greening the Government through Leadership in Environmental Management, In accordance with Executive Order (EO) 13148, Greening the Government through Leadership in Environmental Management, all contracts requiring contractor personnel to perform activities on the installation that have the potential to significantly impact the environment shall comply with the following:

(1) ESOHMS Information Requirements: For contracts that include FAR Clause 52.223-5 (Pollution Prevention and Right-to-Know Information) and/or its alternative paragraph (c)(7), which states “The contractor shall provide all information needed by the Federal facility to comply with the …environmental management system as described in Section 401 of E.O.

13148, the contractor shall comply with the following:

Information Requirements:

(a) AIR: Excavation - When disturbing more than a tenth of an acre, a dust control permit must be obtained from Maricopa County Environmental Services Department. Equipment (generators, abrasive blasting, and solvent/degreaser tanks) that remains located on the base for more than 30 days must be listed and provided to the Air Program Manager. Luke AFB is located in a non-attainment area and is regulated by Maricopa County. Any activities that include earthmoving (fugitive dust) (MC Rule 310), abrasive blasting (MC Rule 312), spray coating procedures (MC Rule 315), solvent degreasing (MC Rule 331), architectural coatings (MC Rule 335), or surface coatings requirements (MC Rule 336) reference must be made to the Maricopa County Air Regulations at http://www.maricopa.gov/aq to ensure activities or products used are in compliance with these regulations.

(b) ASBESTOS: No asbestos containing material shall be used on any government installation.

A Material Safety Data Sheet (MSDS) is required for all building materials except for wood, glass, or steel. The MSDS’ shall be submitted to the Contracting Officer (CO) via the AF Form 3000 within seven (7) calendar days after issuance of the written Notice to Proceed (NTP).

(c) HAZARDOUS MATERIALS MANAGEMENT: In accordance with FAR Clause 52.223-3, the Contractor shall provide Material Safety Data Sheets (MSDS) to the Contracting Officer (CO) for all hazardous materials (HAZMAT) to be used during the project prior to contract award. The CO or COR shall provide MSDS’ to 56 CEV/CEAN for review prior to the contractor bringing products on base. The contractor shall inventory all HAZMAT, including but not limited to:

solvents, paints, degreasers, greases, Ozone Depleting Substances, and oils brought on base.

A HAZMAT inventory shall be maintained at all times and contain the nomenclature, manufacturer, part number, quantity, storage location, and the process in which the material is used. All HAZMAT stored at Luke AFB shall be managed in accordance with federal, state, local, and Air Force regulations. Identified HAZMAT shall be managed and tracked in the installation’s approved HAZMAT tracking system. If a HAZMAT is required to be tracked, the contractor shall complete required documents to support installation requirements. Contractor shall report monthly HAZMAT inventories/usage to the CO or COR. CORs shall provide inventory/usage to the Hazardous Material Management Office (Bldg 927).

(d) LEAD-BASED PAINT: Lead based paint shall not be used during any painting process. An MSDS shall be submitted for all paint products.

(e) PLUMBING FIXTURES: Reserved

(f) POTABLE WATER: Reserved

(g) WASTEWATER: No chemical products shall be dumped in any drainage system, storm drain, wash rack, septic system, or sanitary sewer. Prior to final payment, the contractor shall remove all remaining and residual chemicals from the installation. If there are questions concerning specific wastewater products, please contact 56 CES/CEAN.

(h) SOLID WASTE: Reserved

(2) ESOHMS Training Requirements: In accordance with the memorandum dated 11 Dec 2006, Conformance with Air Force Environmental Management System (EMS) Requirements for Contracts Performed on Air Force Installations, all contractor personnel working on the installation shall complete Air Force-provided initial ESOHMS awareness-level training and comply with the following:

(a) 56 CONS Contractor Administrator(s) shall provide (1) Key contractor personnel with initial ESOHMS awareness-level training at the pre-construction meeting and (2) An electronic version of the training via email upon request. It is the responsibility of key contractor personnel to disseminate the initial ESOHMS awareness-level training to all prime and subcontractor employees.

(b) The initial ESOHMS awareness-level training can be accomplished in approximately fifteen

(15) minutes.

(c) All contractor employees who work on the installation shall complete initial ESOHMS awareness-level training no later than sixty (60) days after contract award.

(d) Neither the installation nor contractor personnel are required to document contractor employee initial ESOHMS awareness-level training. However, key contractor personnel shall follow-up with all employees to ensure successful completion of the training.

(3) ESOHMS Refresher Training Requirements: Contractor employees who complete initial ESOHMS awareness-level training remain current for a period of 365 days from date of completion. Key contractor personnel shall ensure all prime and subcontractor employees are current throughout the performance of the contract.

6.4.2. Hazardous Materials Inventory. The contractor shall maintain an up-to-date hazardous material inventory with copies of MSDS(s) for all materials used on the job site. The contractor shall maintain a complete copy of all approved MSDS and AF Form 3952s.

6.4.3. Hazardous Materials Management: The contractor shall remove all hazardous materials by the end of each workday. Contractor shall not conduct process treatments (e.g. paint thinner solvent recycling) on base.

6.4.4. Final Report. The contractor shall submit a final inventory, not later than ten working days following termination/completion of this contract, of all hazardous materials used since the last hazardous usage report to the CO or COR. The final inventory will list all hazardous materials used, total amount of each used, and a description of the disposition of any remaining hazardous material.

6.5. PERMITS, LICENSES AND CERTIFICATIONS. Unless otherwise specified in this contract, obtain necessary permits, licenses, and certifications; give all required notices; and comply with applicable Federal, State, County, and local laws, codes, and regulations in performance of the requirements of this contract. Maintain records of such requirements and make the documentation available to the Government for review.

6.6. HOURS OF OPERATION. Perform all maintenance services during normal duty hours which are 7:00AM - 4:00PM, Mondays through Fridays, excluding federal holidays, unless otherwise required in the contract or approved by the CO.

6.7. RECOGNIZED HOLIDAYS. The contractor is not required to provide service on federal holidays, except in emergency situations; the holidays are:

New Year’s Day - 1 January Martin Luther King Day - 3rd Monday in January Washington’s Birthday - 3rd Monday in February Memorial Day - last Monday in May Independence Day - 4 July Labor Day - 1st Monday in September Columbus Day - 2nd Monday in October Veteran’s Day - 11 November Thanksgiving Day - 4th Thursday in November Christmas Day - 25 December

If these holidays fall on Saturday, the preceding Friday will be observed. If these holidays fall on Sunday, the following Monday will be observed. If a holiday falls on a scheduled service day, the Contractor will be responsible for rescheduling services for the first day post the holiday observance.

6.7.1. Base Closures. Work scheduled but not accomplished because of base closure due to weather, exercises, or actual alert, will be accomplished as soon as possible after reopening the base.

6.8. SECURITY REQUIREMENTS. The contractor shall comply with Security and base access requirements contained in clause 5352.242-9000 “Contractor Access to Air Force Installations.”

Contractor employees will be required to obtain and display identification badges. Anticipate delays in getting commercial vehicles on base and allow time for commercial vehicles to reach their destination by driving designated routes at posted speed limits throughout the base.

Procedures for commercial vehicle access to the base are subject to change without prior notice.

6.9. PERFORMANCE OF SERVICE DURING CRISIS OR HEIGHTENED SECURITY. In the event of crisis or heightened security caused by a national emergency, natural disaster, or other causes, continues performance as necessary in support of the Air Force mission. The contract price and delivery schedule may be adjusted to reflect any change of work that may be directed.

6.10. INTERFACES. Do not unduly interfere with regularly scheduled Government operational activities in the performance of contract requirements. In the event a Government supervisor so requests, temporarily cease work in the area and report the instructions, to include name of the Government person involved, to the CO immediately by the most expedient means. Notify the CO verbally of disputes with customers or other base contractors and follow-up in writing.

6.11. CONTRACTING OFFICER AND COR. The Contracting Officer will appoint a primary and alternate representative for management of the day to day activities of the contract. The identity, title, and authority of this representative will be provided in writing to the contractor after contact award.

6.12. SAFETY REQUIREMENTS AND REPORTS. The contractor shall perform work in a safe manner as required by OSHA Safety and Health Requirements. Provide a verbal report to the CO as soon as possible of each occurrence of damage to Government property or an accident resulting in death, injury, occupational disease, or adverse environmental impact. Provide a completed copy of required Accident Investigation Reports to the CO within five calendar days of each occurrence.

6.13. FIRE SAFETY: The Contractor shall obtain a burn permit from the Base Fire Department prior to welding or cutting operations each day he anticipates performing that type of work. All welding and burning operations necessary for the completion of this contract shall be accomplished in strict compliance with Fire Safety standards.

6.14 CONTRACTOR-FURNISHED ITEMS AND SERVICES. Except for those items or services specified as Government-Furnished, the Contractor shall furnish everything needed to perform this contract according to all its terms and conditions.

6.15 Fire Extinguishers: The Contractor shall provide and maintain fire extinguishers in accordance with NFPA standards during all welding/burning operations relative to this contract.

DEFINITIONS - TECHNICAL

Where "as shown", "as indicated", "as detailed", or words of similar import are used, it shall be understood that reference is made to this specification and the drawings accompanying this specification unless stated otherwise.

Where "as directed", "as required", "as permitted", "approval", "acceptance", or words of similar import are used, it shall be understood that direction, requirement, permission, approval, or acceptance of the ACO is intended unless stated otherwise.

ANSI. The abbreviation of the American National Standards Institute.

Adjust. To regulate, settle, bring to a more satisfactory state of normal operating condition.

Certified Inspector. Per the CMAA, a qualified inspector of cranes must have a minimum of 2000 field hours of experience related to the maintenance, servicing, repairing, modifying and functional testing of cranes and associated hoisting equipment. This experience should provide a working knowledge of how to identify deficiencies in mechanical, structural, electrical systems and components of cranes. Under no circumstance should an individual be permitted to perform inspections who has not received appropriate training and cannot fully demonstrate a working knowledge of applicable codes and regulations of the equipment being inspected.

Contracting Officer (CO). The individual designated as the primary agent of the Government to enter into, administer and obligate funds.

COR. The Government employee designated by the CO to be responsible for the monitoring of Contractor performance.

Component Part. Any part of any item or system which is detachable or removable from the main body or main assembly of the item or system; a constituent part or an essential part necessary to the performance of the system.

Contractor. The term Contractor as used herein refers to both the prime Contractor and any subcontractors. The prime Contractor shall ensure that his/her subcontractors comply with the provisions of this contract.

Contractor Representative. A foreman or superintendent responsible for project management.

Control. A mechanism used to regulate or guide the operation of a machine, apparatus, or system.

Critical Load. Materials that if damaged or destroyed during a lift would present an unrecoverable loss or unacceptable risk. Air Force materials will be designated as critical loads for purposes of lifting or hoisting by an appropriate Program Manager or the functional manager at the facility where the lift will occur. Items for consideration in making a critical load determination include, but are not limited to:

– Personnel injury or significant adverse health impact.

– Damage resulting in serious economic consequences (nonavailability of necessary funds to repair or replace the item).

– Damage resulting in unacceptable delay to schedule (mission impairment) or other destructive programmatic impact (e.g., loss of vital data).

– Undetectable damage that would jeopardize future operations or safety of a facility.

– Damage that may occur without exceptional care in handling because of close- tolerance installation, high susceptibility to damage, or other unusual characteristic.

– Damage that would result in significant release of hazardous materials.

– Damage that may occur because the item, although noncritical, must be lifted above a critical item(e.g., loads of any sort in close proximity to a nuclear ` component or near concentrations of hazardous materials).

Direct Material Costs. The actual vendor invoice charges for materials used for performance of work under this contract. Direct material costs shall include transportation charges when such charges are included on the invoice by the vendor, as well as any discounts allowed for prompt payment and discounts or rebates for core value or salvage value that accrue to the Contractor.

Hoist Mechanic. Any person employed by the Contractor who is responsible for performing maintenance, repair, and inspection services under this contract. Hoist mechanics are journeyman-level tradesmen who are qualified by virtue of a combination of formal training and work experience in the trade. Hoist mechanics perform both electrical and mechanical work on a variety of hoist systems.

Emergency Service Call. Work requiring immediate action to correct or prevent loss or damage to Government property, or to eliminate hazards to personnel or equipment.

Frequency of Service.

o Annual (A) - Services performed once during each 12 month period of the contract.

o Semiannual (SA) - Services performed twice during each 12 month period of the contract at intervals of 160 to 200 calendar days.

o Quarterly (Q) - Services performed four times during each 12 month period of the contract at intervals of 80 to 100 calendar days.

o Monthly (M) - Services performed 12 times during each yearly (12 month) period of the contract at intervals of 28 to 31 calendar days.

o Semimonthly (SM) - Services performed 24 times during each 12 month period of the contract at intervals of 14 to 16 calendar days.

Inspect. To examine thoroughly, to view closely in critical appraisal.

Maintain. To keep in a state of repair and efficiency, to preserve from failure or decline.

Maintenance. Work which is required to repair or maintain equipment systems in a safe, operational condition, or to restore these systems to initial or usable condition by overcoming the effects of wear and tear, disaster, damage, or deterioration. New construction or alteration is not included.

Provided shall be understood to mean provided in place, that is, furnished and installed, ready for use.

Quality Assurance (QA). A method used by the Government to provide some measure of control over the quality of purchased goods and services received.

Quality Control (QC). A method used by the Contractor to control the quality of goods and services produced.

Regular Working Hours. The Government's regular (normal) working hours are from 0700 to 1600, Mondays through Fridays except (a) Federal holidays and (b) other days specifically designated by the contracting officer.

Repair. Repair is the restoration of a piece of equipment, a system, or a facility to such condition that it may be effectively utilized for its designated purposes. Repair may be overhaul, reprocessing, or replacement of constituent parts or materials that have deteriorated by action of the elements or usage and have not been corrected through maintenance, or replacement of the entire unit or system if beyond economical repair.

Replace. To remove damaged, defective, or deteriorated materials or parts and install new or used materials or parts as approved.

Required. Demanded as necessary or essential to the satisfactory operation of the equipment or system.

Response Time. Response time is defined as the time allowed the Contractor after initial notification of a work requirement to be physically on the premises at the work site with appropriate tools, equipment, and materials, ready to perform the work required.

Response times are designated in the appropriate technical clauses in Section C.
