This document is the FY 2025 Southern Area SFC Hazard Mitigation Grant Optional Application Form. It provides detailed instructions and requirements for applicants seeking funding through the FY25 State Fire Capacity (SFC) Wildfire Hazard Mitigation grant opportunity.

The application form covers key details such as the project location, title, requested funding amount, matching funds, and applicant contact information. It outlines the evaluation criteria and required narrative sections, including project area description, proposed activities and accomplishments, budget narrative, strategic investment methodology, project timeline, collaboration, connection to state plans, sustainability, and assistance to underserved communities. Applicants must provide quantitative estimates for activities such as developing Community Wildfire Protection Plans (CWPPs), conducting prevention education programs, and implementing hazardous fuels reduction projects. The summary also identifies the relevant federal grant program (CFDA 10.698 - State & Private Forestry Cooperative Fire Assistance) and the administering federal agency as the USDA Forest Service.
