About this file

The document is a Foreign Country of Concern Attestation form (Form No. 7PUR_1355_Attestation.docx) related to a Florida Department of Transportation (FDOT) District Five solicitation for 2 Gbps broadband internet service. The procurement is structured as an Invitation to Bid (ITB) for a three-year contract from July 1, 2025, to June 30, 2028, with no renewal options. Bids are due on March 25, 2025, at 10:00 AM, with a public bid opening at 11:00 AM and an intended award date of March 31, 2025. The service requires two potential connection options: direct service at the District Office in DeLand, Florida, or hand-off to FDOT field equipment at four specified locations.

The solicitation includes specific requirements such as providing a monthly service price, a /28 IP address block with 14 usable addresses, and guaranteeing 99.99% network availability with 24/7 monitoring and maintenance. Pricing will be evaluated based on the lowest responsive bid, with potential preferences for Veteran Business Enterprises, drug-free workplace programs, and Minority Business Enterprises. Vendors must be registered in the State of Florida's MyFloridaMarketPlace system, comply with E-Verify requirements, and meet various regulatory standards including Title VI Civil Rights Act compliance. A 1% MyFloridaMarketPlace transaction fee will be applied to contract payments, and there are strict communication restrictions between the solicitation release and the notice of intended award.
