ADMINISTRATIVE INFORMATION
The point of contact for administrative concerns at Naval Medical Logistics Command (NMLC), 693 Neiman Street, Fort Detrick, MD 21702 is Yun Hong, 301-619-4162, yun.p.hong.civ@mail.mil.
CONTRACTOR SUPPORT
Notice: Navy Use Of Support Contractor For Contract Closeout Functions
Naval Medical Logistics Command (NMLC) uses two private contractors in support of the contract closeout process. Those companies are Contracting Resources Group (CRG) of Federal Hill, MD, doing business under the authority of the Small Business Administration’s 8(a) program and the Ability One Program, doing business under the authority of the Javits-Wagner O’Day Act (41 U.S.C. § 47).
The contract closeout process includes activities such as processing deobligation modifications, obtaining contractor and requiring activity concurrence, preparing the DD-Form 1594 (Contract Completion Statement), and preparing closed files for archiving. Support contractors may perform additional administrative duties, including filing and processing simple administrative modifications. Performing these functions require the contractor employees to have access to the contract file. Therefore, information you provide to the Government or information already in the possession of the Government may be viewed by these support contractors in the course of performing contract close-out functions. The information that may be made available to the contractor may include pricing and technical proposals and performance information.
NMLC has signed Non-Disclosure Agreements with each support contractor employee and has required both contractors to provide a Conflict of interest Mitigation Plan to ensure these employees are firewalled from all business development activity.
By signature of this order, the offeror and its subcontractors consent to access of their business sensitive/confidential or proprietary data by the Government’s support contractor personnel in order to perform close out services.

INFORMATION ASSURANCE
In accordance with Department of Defense Instruction (DoDI) 8500.01, all acquisitions of Automated Information
Systems (AIS) to include, outsourced IT-based processes, and platforms or weapon systems with connections to the
Global Information Grid (GIG) must be certified and accredited. The primary methodology for certifying and accrediting Department of Defense (DoD) information systems/devices is the DoD Information Assurance
Certification and Accreditation Process (DIACAP) ; being transitioned to Risk Management Framework (RMF) of
DoD Instruction (DoDI) 8510.01 and is applicable to all DoD IT that receive, process, store, display, or transmit
DoD information. These technologies are broadly grouped as DoD IS, platform IT (PIT), IT services, and IT products. This includes IT supporting research, development, test and evaluation (T&E), and DoD-controlled IT operated by a contractor or other entity on behalf of the DoD.
After contract award, vendor must subscribe to Office of Personnel Management (OPM) National Agency with local
Agency and Credit (NACLC) for vendor personnel required to log-onto any device/system connected to the
MTF/DoD network. This applies to any Vendor personnel accessing the DoD networked device on-site or remotely and during installation. Vendor responsibilities for ensuring personnel security include, as required per the DOD regulations meeting the following requirements:
a. Follow the DoD guidelines for submittal of (IT) security background checks and ensure all contractor personnel are designated as IT-I, IT-II, or IT-III where their duties meet the criteria of the position sensitivity designations.
Contact the service Program Management Office (PMO) for guidance on the appropriate IT levels for personnel on the contract. Any vendor personnel that will be accessing the medical device, remotely or in person, while installed on a DoD network will be required to have a National Agency Check and Credit Checks (NACLC) completed.
Typically, this requires an investigation to support a "Public Trust Position" and requires the person(s) to complete and submit a Standard Form 86 (SF 86), Questionnaire for Public Trust Positions, via the Electronic Questionnaires for Investigations Processing (e-QIP). Questions relating to SF86 and the eQIP process may be directed to 1-888-
282-7682 or online at http://www.dss.mil/index.html.
b. Initiate, maintains, and document personnel security investigations appropriate to the individual's responsibilities and required access to Sensitive Information (SI).
c. Immediately report to the appropriate Service POC and deny access to any automated information system (AIS), network, or information if a contractor employee filling a sensitive position receives an unfavorable adjudication, if information that would result in an unfavorable adjudication becomes available, or if directed to do so by the appropriate Service representative for security reasons.
d. Ensure that all vendor personnel receive Information Assurance (IA) training before being granted access to DoD
AISs.
The Vendor shall comply with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) (P.L. 104-
191) requirements, specifically the administrative simplification provisions of the law and the associated rules and regulations published by the Secretary, Health and Human Services (HHS). This includes the Standards for
Electronic Transactions, the Standards for Privacy of Individually Identifiable Health Information and the Security
Standards.
If an Authorization to Operate (ATO) or an Authorization to Connect (ATC) has not been granted for the device/system, the following artifacts shall be included with your proposal; System Identification Profile (SIP), Architectural Drawing, DIACAP Implementation Profile (DIP). Templates have been included with this solicitation as Appendix 1 Sample Wireless System Architecture, Appendix 2 Vendor Name System Model PIA , Appendix 3

1) Either the Wired System Architecture or the Wireless System architecture, depending on which architecture applies to the system.
2) System Description
3) PIA
The AF Clinical Engineering Team, IA Element, will review the templates submitted within 10 business days. If the completed templates are deemed acceptable, the AF Clinical Engineering Team IA Element will notify Navy
Contracting to have the Vendor considered for contract award. If the templates are incomplete, the Vendor will be considered technically unacceptable.
If an ATO or an ATC has been granted for this specific equipment, the Vendor must submit a copy of the approval with the submission of their proposal.
Failure to agree to Cybersecurity or Service IA requirements shall result in rejection of the Vendor’s quote. Failing to disclose that a device/system cannot meet IA requirements may result in termination for cause in accordance with
FAR 52.212-4(m). If the Vendor does not comply with the above, the Government reserves the right to cancel this contract/delivery order at no cost to the Government.
The Vendor shall be required to obtain and maintain a current ATO/ATC as long as the equipment is commercially supported. This includes timely patch management, annual reviews and reaccreditations.

(B) The taxpayer is delinquent in making payment. A taxpayer is delinquent if the taxpayer has failed to pay the tax liability when full payment was due and required. A taxpayer is not delinquent in cases where enforced collection action is precluded.
(ii) Examples.
(A) The taxpayer has received a statutory notice of deficiency, under I.R.C. Sec. 6212, which entitles the taxpayer to seek Tax Court review of a proposed tax deficiency. This is not a delinquent tax because it is not a final tax liability. Should the taxpayer seek Tax Court review, this will not be a final tax liability until the taxpayer has exercised all judicial appeal rights.
(B) The IRS has filed a notice of Federal tax lien with respect to an assessed tax liability, and the taxpayer has been issued a notice under I.R.C. Sec. 6320 entitling the taxpayer to request a hearing with the IRS Office of Appeals contesting the lien filing, and to further appeal to the Tax Court if the IRS determines to sustain the lien filing. In the course of the hearing, the taxpayer is entitled to contest the underlying tax liability because the taxpayer has had no prior opportunity to contest the liability. This is not a delinquent tax because it is not a final tax liability. Should the taxpayer seek tax court review, this will not be a final tax liability until the taxpayer has exercised all judicial appeal rights.
(C) The taxpayer has entered into an installment agreement pursuant to I.R.C. Sec. 6159. The taxpayer is making timely payments and is in full compliance with the agreement terms. The taxpayer is not delinquent because the taxpayer is not currently required to make full payment.
