About this file

The document is an attachment to a Request for Proposals (RFP) issued by the Florida Department of Highway Safety and Motor Vehicles (FLHSMV) for Aircraft Maintenance and Repairs. The RFP seeks to establish contracts with up to three vendors to provide scheduled and unscheduled routine maintenance, inspections, and repairs on the Department's fixed-wing aircraft in accordance with applicable Federal Aviation Regulations. The contract term is for three years with an optional three-year renewal. The attachment provides a Price Sheet for respondents to specify their hourly labor rates and flat-rate pricing for various aircraft models and services, including regular maintenance, after-hours maintenance, mobile maintenance, and offsite repairs.

The Price Sheet must be completed and signed to be considered a responsive proposal. The Department reserves the right to negotiate the selected vendor's pricing. Respondents shall provide firm pricing inclusive of all costs, fees, expenses, travel, lodging, and consumables. The Mobile Maintenance Hourly Rate must include all expenses associated with travel to the work site. The original and renewal contract terms will have separate pricing sheets, with the formulas in the spreadsheet automatically calculating the average rates for each.

ATTACHMENT F - PRICE SHEET

FLHSMV-RFP-003-23

Respondent Name:
The Respondent shall provide a dollar amount in each yellow cell below, including both the Original Term and Renewal Contract Term.

Prices shall be inclusive of all costs, fees, expenses, travel, lodging services, and consumables (i.e. all purpose cleaner, wipes). NOTE: The Mobile Maintenance Hourly Rate must include all expenses the Respondent will incur in providing Mobile Maintenance, including, but not limited to, travel to the site where mobile maintenance is to be provided, and the fuel, labor, and other expenses necessary to travel to the site where mobile maintenance is to be provided. The Respondent is not permitted to bill the Department for, and Mobile Maintenance Hourly Rate does not apply to, the travel time required to travel to provide mobile maintenance; that expense must be included in the Mobile Maintenance Hourly Rate.

When the Respondent makes entries on this spreadsheet, the formulas contained in this spreadsheet will automatically perform the calculations for the "A. Average Rate - original contrct term" and the "B. Average Rate - Renewal contract term".

The Price Sheet must be completed and signed to be considered responsive. The rates submitted by the Respondent shall be firm.

NOTE: The original term of the contract is for three (3) years with an optional renewal term of up to three (3) years. This is not a guaranteed six (6) year contract.
