STATEMENT OF WORK FOR PROJECT
598-22-131
Building 170 – 1E Courtyard / Patio Repairs, N. Little Rock
Arkansas

Title of Project: Building 170 – 1E Courtyard / Patio Repairs
GENERAL INFORMATION
A. The contractor shall prepare a detailed Project Management Plan (PMP) to meet all objectives. For every task, the Contractor shall identify in writing all necessary subtasks (if any), associated costs by tasks, and associated dates.
B. All employees of general contractor and subcontractors shall comply with VA security management program, be identified thru proper badging by project/employer, and be restricted from areas of unauthorized access.
C. Special Consideration: All contractors and visitors are required to adhere to Universal
Masking Guidance which requires people to wear a cloth face covering or facemask over their mouth and nose while in any building of Central Arkansas Veterans
Healthcare System (CAVHS). Daily healthcare screening is required before entrance to VA facilities due to COVID-19. No one with a cold or flu like symptoms will be allowed on VA property. In the event symptoms occur while on site, the contractor will notify the COR immediately. The individual(s) maybe required to be COVID tested and quarantine until released by Infection Control to return to site.
D. Contractor employees who work in or travel to VHA locations must comply with the following:
D.1 Documentation requirements:
D.1.1 If fully vaccinated, shall show proof of vaccination.
D.1.1.1 NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record
Card (CDC Form MLS-319813_r, published on
September 3, 2020), or a copy of medical records documenting the vaccination.
D.1.2 If unvaccinated and granted a medical or religious exception, shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug
Administration (FDA) for emergency use or full approval. This includes tests available by a doctor’s order or an FDA approved over-the-counter test.
D.1.3 Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel.
D.1.4 Documentation will not be collected by the VA; contractors shall, at all times adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.
D.2 Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates.
D.2.1 Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the
COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.
D.3 Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities.
DESCRIPTION OF WORK
Contractor shall provide all management, supervision, qualified personnel, labor, transportation, tools, equipment, taxes, materials, licenses, permits, insurance, certifications, safety data sheets, and services required to complete the various removal and replacement requirements to the Building 170 1E Courtyard / Patio in North Little
Rock, Arkansas, identified in the Scope of Work below. The contractor shall accomplish all services below to meet the requirements of this Statement of Work (SOW). Contractor shall comply with all State of Arkansas, federal, and local requirements to include but not mailto:https://www.va.gov/covid19screen/ mailto:https://www.va.gov/covid19screen/ limited to Occupational Safety and Health Administration (OSHA). All workmanship shall comply with all applicable codes, specifications, local ordinances, and be in general accordance with industry standards.
1. SCOPE OF WORK
1.1 Contractor shall provide all management, supervision, qualified personnel, labor, transportation, tools, equipment, taxes, materials, licenses, permits, insurance, certifications, safety data sheets, and services required to complete the various removal and replacement requirements to NLR Building 170 – 1E Courtyard patio identified below. The work is to include:
A. Contractor to conduct (1) site visit to evaluate and document the existing conditions of the 1E Courtyard / Patio located in Building 170. Contractor shall gather required existing conditions to verify dimensions of existing patio.
B. Contractor to demolish and remove the existing brick and mortar patio surface, as well as any other associated construction debris. Contractor to remove mortar to existing concrete elevation, if possible. Contractor to demolish and remove the existing brick wall spanning from the 1E patio to the lower roof located above the
Ground floor hallway. All construction debris shall be removed and hauled away outside of the Building 170. Portions of the existing fence on the south edge of the patio may be removed and re-assembled to facilitate the removal of construction debris. Access to the rear of the building adjacent to the 1E Courtyard/Patio is limited and is blocked by the existing covered pedestrian walkway. The existing covered pedestrian walkway shall not be altered or damaged in the removal of the construction debris. A Demolition Plan detailing the construction debris removal process shall be approved by North Little Rock VA Planning and Design via the project COR. Approval from the project COR will be required before the demolition process can begin.
C. Contractor to install new brick veneer wall on wall ledge spanning from 1E patio to lower hallway roof. Install weep holes at 6’-0” O.C. at the bottom of the veneer to drain any moisture. Brick veneer shall be installed per Section 04 20 00 UNIT
MASONRY.
D. Contractor to remove the existing exterior door on the east side of the 1E Courtyard.
Contractor to remove door opening devices while protecting existing power supplies for door opening mechanisms.
E. Contractor to grade surface to incorporate a landing at outside of existing door opening. Landing shall be 4’-0” by 8’-0” wide centered on the existing door opening.
Landing shall be installed to meet the requirements of PG-18-13 VA Barrier Free
Design Standards (January 1, 2017 rev 11/1/18).
F. Contractor to apply a layer of concrete to create a relatively smooth surface to place the roof TPO on top of. Contractor to maintain the new landing grades at the door and begin sloping new grades to the existing drain. Surface shall meet TPO manufacturer’s specifications for surface material, smoothness etc. Contractor shall slope patio from exterior walls to drains with 1% minimum slope and 2% maximum slope in any direction.
G. Install new roof TPO to the top of the concrete deck. TPO shall be installed by a roofer with a minimum of 2 years of roofing experience and professional knowledge in the installation of TPO roofing. Start TPO on side walls to ensure waterproof seal.
TPO material selected shall be installed per Section 07 54 23 THERMOPLASTIC
POLYOLEFIN (TPO) ROOFING. Roofing material shall be designed to be abrasion resistant rated and material specifications shall be submitted to the COR for approval before purchase of the TPO material. Ensure TPO roofing is sloped to drains with
1% minimum slope and 2% maximum slope in any direction. Extend TPO roofing to back of brick veneer the sidewalls and adhere to achieve a waterproof seal at the edge of the patio.
H. Contractor shall install protection around drains on the 1E patio to allow for free drainage into the top of the floor drains to avoid plugging of drains.
I. Contractor shall bring in materials from the outside of the 1E Courtyard/Patio.
Contractor to provide a Laydown Area plan detailing the proposed laydown area and means of bringing materials into the laydown area for approval from the COR.
J. Contractor shall install concrete pavers placed on sand / gravel bedding and spacers to interlock the pavers in place. Pavers shall be similar in size, color and configuration to the Building 170 1E/1F courtyard that is accessed off of corridor C1B1. Contractor to submit material selection for COR approval before ordering materials. Bricks shall be installed with slope and spacing requirements meeting PG-18-13 VA Barrier Free
Design Standards (January 1, 2017 rev 11/1/18). Contractor to ensure finished grades at landing meet slope requirements.
K. Contractor to install automatic door in existing door opening location. Match existing door opening size. Install hand operated door opening device on inside of door for opening and mount automatic door opening device and hand operated door opening device on exterior of door. Install door per Section 08 31 13 ACCESS DOORS AND
FRAMES, Section 08 71 00 DOOR HARDWARE & Section 08 71 13 AUTOMATIC
DOOR OPERATORS.
L. Contractor to install new gutter system to drain the existing awning over the exterior door into the 1E Courtyard. Extend piping to the existing exterior finished grade.
Provide splash blocking at the outfall of the gutter piping.
M. Upon completion and approval of the Building 170 Courtyard/Patio the existing fence that was disassembled to facilitate removal of material and bringing in material shall be re-installed. Fence shall match existing configuration, finish and color.
N. Contractor to install protection around drains at the lower hallway roof section to allow for free drainage into the top of the floor drains to avoid plugging of drains.
O. Prep the existing lower hallway roof to allow for installation of new TPO. Prepare surface per manufacturer’s requirements and ensure roof is sloped to existing drains.
P. Install new roof TPO to roof deck. TPO shall be installed by a roofer with a minimum of 2 years of roofing experience and professional knowledge in the installation of
TPO roofing. Install TPO on side walls to ensure waterproof seal. TPO material selected shall be installed per Section 07 54 23 THERMOPLASTIC POLYOLEFIN
(TPO) ROOFING. Roofing material shall be rated for patio live load of 40 psf. Ensure
TPO roofing is sloped to drains with 1% minimum slope. Extend TPO roofing up onto the sidewalls and adhere to achieve a waterproof seal.
Q. Contractor to remove construction barriers and clean up construction debris when construction is completed. Contractor to provide documentation for project completion and close-out as required.
2. PERIOD OF PERFORMANCE
2.1 The project start date shall be within five days after notice to proceed work. The period of performance is 120 days following Lead time to buy equipment (About 2 weeks lead time).
If for any reason a deliverable cannot be delivered as scheduled, the Contractor is required to submit a request for a time extension to the Contracting Officer and the COR. The request must include the reason(s) for the delay, the impact on the overall project, and the impact on the cost of the project. The Contracting Officer will consider each request based on its merits and will, if approved, issue a modification to the contract. The Contractor is required to proceed as originally scheduled until such modification is issued. The
Contracting Officer is the only official authorized to make changes to the contract.
3. SPECIFIC REQUIREMENTS
3.1 Final Clean up: On completion of project and after removal of all debris, rubbish, and temporary construction, Contractor shall leave the construction area in a clean condition satisfactory to the Resident Engineer. Cleaning shall include off the station disposal of all items and materials not required to be salvaged, as well as all debris and rubbish resulting from demolition and new work operations.
3.2 Demolition Plan. The Contractor shall prepare a Demolition Plan detailing how the demolition and removal of construction debris will occur. Because of the limited access to the back of the building to get to the 1E Patio, the existing covered pedestrian walkway will need to be accessed. The Demolition Plan will need to detail how pedestrians using the walkway will maintain access while meeting safety requirements during demolition. Demolition Plan shall detail how the Contractor plans to protect the existing walkway and repair any damages that may occur to the covered walkway, sidewalks or grassy areas.
3.3 Laydown Area Plan. The Contractor shall prepare a Laydown Area Plan detailing how the storage of demolition material being removed will occur and storage of material for the patio installation will occur. Because of the limited access to the back of the building to get to the 1E Patio, the existing covered pedestrian walkway will need to be accessed. The Laydown Area Plan will need to detail how pedestrians using the walkway will maintain access while meeting safety requirements during construction. It shall also detail how the Contractor plans to protect the existing walkway, sidewalks or grassy areas and repair any damages that may occur.

4.2 The General Contractor (GC) is responsible for ensuring that the specifications are tailored to the specific requirements of the project and are compatible with the current contract provision. Contractor to notify COR and CO of any discrepancies for clarification of project requirements.
4.3 GOVERNMENT FURNISHED INFORMATION
The following is a list of government furnished information found on the VA’s Technical
Information Library website (http://www.cfm.va.gov/TIL/) to be used for this project, as applicable. Sections named on (4.1 – VA MASTER SPECIFICATIONS) are within below
VA standards.

5 COORDINATION AND SCHEDULING
5.1 Any interruption in service must be scheduled and coordinated with the COR 30 days prior to starting work. It is the CONTRACTOR'S responsibility to develop a work plan and schedule detailing, at a minimum, the procedures to be employed, the equipment and materials to be used, the interim life safety measure to be used during the work, and a schedule defining the duration of the work with milestone subtasks. The work to be outlined shall include, but not be limited to the task being performed.
5.2 To ensure such executions, Contractor shall furnish the COR with a schedule of approximate dates on which the Contractor intends to accomplish work in each specific area of site, building or portion thereof. In addition, Contractor shall notify the COR 30 days in advance of the proposed date of starting work in each specific area of site, building or portion thereof. Arrange such dates to ensure accomplishment of this work is mutually agreeable to the COR and Contractor.
5.3 Building 170 will be occupied during performance of work. Contractor shall take all measures and provide all material necessary for protecting existing equipment and property in affected areas of construction against dust and debris.
6 FIRE SAFETY PLAN
6.1 Establish and maintain a site-specific fire protection program in accordance with 29 CFR
1926.
6.1 Maintain free and unobstructed access to facility emergency services and for fire, police and other emergency response forces in accordance with NFPA 241.
6.2 Do not block exiting for occupied buildings, including paths from exits to roads.
Minimize disruptions and coordinate with COR.
6.3 Do not impair automatic sprinklers, smoke and heat detection, and fire alarm systems, except for portions immediately under construction, and temporarily for connections.
Provide a fire watch for impairments more than 4 hours in a 24-hour period.
6.4 Perform and safeguard hot work operations in accordance with NFPA 241 and NFPA
6.5 Coordinate with the VA Fire Department @ 501-257-4351 to obtain permits from fire department at least 1 hour in advance of performing any hot work. Designate contractor's responsible project-site fire prevention program manager to permit hot work.
6.6 Prevent smoke detectors accidental operation. Remove temporary covers at end of work operations each day.
6.7 Smoking is prohibited in all VA facilities and on grounds.
7 DEMOLITION
7.1 Perform demolition in such manner as to eliminate hazards to persons and property;
minimize interference with the use of adjacent areas, utilities and structures or interruptions of use of such utilities; and to provide free passage to and from such adjacent areas and structures.
7.2 Provide safeguards, including warning signs, barricades, temporary fences, dust control, warning lights, and other similar items that are required for protection of all personnel during demolition and removal operations.
7.3 Prevent spread of flying particles and dust. Sprinkle rubbish and debris with water to keep dust to a minimum. Do not use water if it results in hazardous or objectionable condition such as, but not limited to flooding or pollution. Vacuum and dust the work area daily.
7.4 Wherever a cutting torch or other equipment that might cause a fire is used, provide, and maintain fire extinguishers nearby ready for immediate use. Instruct all possible users in use of fire extinguishers.
7.5 Before beginning any demolition work, the Contractor shall survey the site and examine the drawings and specifications to determine the extent of the work. The contractor shall take necessary precautions to avoid damages to existing items to remain in place, to be reused, or to remain the property of the Medical Center; any damaged items shall be repaired or replaced as approved by the COR.
7.6 The Contractor shall coordinate the work of this section with all other work and shall construct and maintain shoring, bracing, and supports as required. The Contractor shall ensure that structural elements are not overloaded and shall be responsible for increasing structural supports or adding new supports as may be required as a result of any cutting, removal, or demolition work performed under this contract.
7.7 Do not overload structural elements. Provide new supports and reinforcement for existing construction weakened by demolition or removal works. Repairs, reinforcement, or structural replacement must have Resident Engineer’s approval.
7.8 Debris, including brick, concrete, stone, metals, and similar materials shall become property of Contractor and shall be disposed of by him daily, off the Medical Center to avoid accumulation at the demolition area. Materials removed shall become property of contractor and shall be disposed of in compliance with applicable federal, state, or local permits, rules and/or regulations be hauled to VA specified disposal site.
7.9 Clean-up: On completion of work of this section and after removal of all debris, leave site in clean condition satisfactory to COR.
8 CHANGES
8.1 The contractor is advised that only the Contracting Officer, acting within the scope of the contract has the authority to make changes which may affect the contract terms and conditions, quality, quantity, price, and/or delivery. It is highly encouraged and advised that the contractor contact the Contracting Officer for approval in the event there are changes prior to proceeding. No other individual has the authority to make binding changes and/or commitments.
8.2 If the contractor commits to such changes at the direction of any other person than the
Contracting Officer, the changes shall be considered to have been made without proper authority and no adjustment shall be made in contract price to cover any increase in costs incurred as a result thereof.
9 SECURITY REQUIREMENTS
9.1 General Contractor’s employees shall not enter the project site without appropriate badge. Contractor must obtain ID badges in accordance with Medical Center
Memorandum M-1, Part VII, Change 25, Paragraph 8.07C. They may also be subject to inspection of their personal effects when entering or leaving the project site.
9.2 All contractor employees are required to be issued a security badge. The determining factor as to the type of badge issued us the contractors employee’s access requirements and the length of the contract period of performance. Contractor is required to follow the
Security Requirements outlined.
10 PROJECT AREA LIMITATIONS
10.1 All work shall be performed inside the 1E Courtyard / Patio at Building 170, North
Little Rock, Arkansas.
11 OPERATIONS AND STORAGE AREAS
11.1 The Contractor shall confine all operations (including storage of materials) on
Government premises to areas authorized or approved by the Contracting Officer.
The Contractor shall hold and save the Government, its officers and agents, free and harmless from liability of any nature occasioned by the Contractor's performance.
Workmen are subject to rules of Medical Center as applicable to their conduct. Plan and execute the work with minimal impact to the normal functioning of Medical Center as a whole, including patient care, operations of utility services, fire protection systems and any existing equipment, and with work being done by others.
12 PHASING
12.1 No phasing on this project.
13 INFORMATION SYSTEMS OFFICER, INFORMATION PROTECTION
13.1 The contractor will not have access to VA Desktop computers and nor will they have access to online resources belonging to the government while conducting services. If removal of equipment from the VA is required, any memory storage devices, such as hard drives, solid state drives and non-volatile memory units will remain in VA control and will not be removed from VA custody.
14 PRIVACY OFFICER
14.1 The contractor will not have access to protected Patient Health Information (PHI) and nor will they have the capability of accessing patient information during the services provided to the VA and if removal of equipment from the VA is required, any memory storage devices, such as hard drives, solid state drives and non-volatile memory units will remain in VA control and will not be removed from VA custody. All research data available for Contractor analyses is de-identified.
15 RECORDS MANAGER
15.1 The contractor Citations to pertinent laws, codes and regulations such as 44 U.S.C.
chapters 21, 29, 31 and 33; Freedom of Information Act (5 U.S.C. 552); Privacy Act
(5 U.S.C. 552a); 36 CFR Part 1222 and Part 1228.
15.2 Contractor shall treat all deliverables under the contract as the property of the U.S.
Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest.
15.3 Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records.
15.4 Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act.
15.5 Contractor shall not create or maintain any records containing any Government
Agency records that are not specifically tied to or authorized by the contract.
15.6 The Government Agency owns the rights to all data/records produced as part of this contract.
15.7 The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data.
16 HOURS OF OPERATION
16.1 The contractor is responsible for conducting business during normal business hours to facilitate communication between the Government and the Contractor, Monday through Friday except Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings.
16.2 If a holiday falls on a Sunday, the following Monday will be observed as the National
Holiday. When a holiday falls on a Saturday, the preceding Friday is observed as a
National Holiday by U.S. Government agencies. Also, included would be any day specifically declared by the President of the United States. Contractor is not required to provide any service(s) on nationally recognized U.S. Government holidays, nor shall the contractor be paid for these holidays.
16.3 The Contractor must always maintain an adequate workforce for the uninterrupted performance of all tasks defined within this solicitation when the Government facility is not closed for the above reasons. When hiring personnel, the Contractor shall keep in mind that the stability and continuity of the workforce are essential.
16.4 The healthcare systems operations are 24/7, 365 days a year, therefore on occasions;
the Government may process orders for Veterans outside of normal business hours.
The Contractor will be required to provide services under these circumstances.
16.5 Normal operational hours for the North Little Rock Central Arkansas Veterans
Healthcare System are Monday through Friday from 7:30am – 4:30pm. Use of overtime is not applicable.
17 OVERTIME AND HOLIDAYS
17.1 Any overtime and/or holiday pay that may be entitled to the Contractor’s employee(s) shall be the sole responsibility of the Contractor and shall not be billed to nor reimbursed by the Government.

19 ACCEPTANCE
19.1 Pre-Final Inspection: The Government will perform the Pre-Final Inspection to verify that the facility is complete and ready to be occupied. A Government Pre-Final Punch
List may be developed as a result of this inspection. Ensure that all items on this list have been corrected before notifying the Government, so that a Final Acceptance
Inspection with the customer can be scheduled. Correct any items noted on the Pre-
Final Inspection in a timely manner. These inspections and any deficiency corrections required by this paragraph need to be accomplished within the time slated for completion of the entire work or any increment of the work if the project is divided into increments by separate construction completion dates.
19.2 Final Acceptance Inspection: The Contractor’s QC Inspection personnel, plus the superintendent or other primary management person, and the Contracting Officer’s
Authorized designee is required to be in attendance at the Final Acceptance
Inspection. Additional Government personnel can also be in attendance. The Final
Acceptance Inspection will be formally scheduled by the Contracting Officer’s or
Authorized designee based upon results of the Pre-Final Inspection. Notify the
Contracting Officer through the COR office at least 14 days prior to the Final
Acceptance Inspection and include the Contractor’s assurance that all specific items previously identified to the Contractor as being unacceptable, along with all remaining work performed under the contract, will be complete and acceptable by the date schedule for the Final Acceptance Inspection. Failure of the Contractor to have all contract work acceptably complete for this inspection will be cause for the Contracting
Officer to bill the Contractor for the Government’s additional inspection cost in accordance with FAR Clause 52.246-12 titled “Inspection of Construction”.
