SECTION I DESCRIPTION OF SERVICES

1.0. DESCRIPTION OF SERVICES.

The Contractor shall provide all management, licenses/certifications, laboratory testing, tools, supplies, equipment, transportation and labor to develop and implement an Integrated Pest

Management (IPM) plan for pest control services at Edwards Air Force Base (AFB), NASA, Air

Force Plant 42 (AFP42) and Air Force Research Laboratory (AFRL). Services shall be performed in a manner that will ensure the health and general well-being of installation personnel, and effectively control pests. Pest control is the prevention, monitoring, and eradication of unwanted pests to keep the number of pests down to a level where the risk to people, food and the environment is minimized. Pest Control services shall be performed in accordance with (IAW) industry commercial standards, Air Force Manual, AFMAN 32-1053 Integrated Pest Management

Program, (06 Aug 19) and Department of Defense Instruction (DoDI), 4150.07, “DoD Pest

Management Program,” dated 29 May 2008 incorporating Change 2, 31 August 2018 that applies to contract pest management operations as well as the 412TW IPM Plan. The Contractor shall provide a copy of their current California Department of Pesticide Regulation Pest Control

Business Main License; a current copy of the Company's Registration issued by California

Structural Pest Control Board; and a current copy of Pest Control Operators License Branch 2 and

3 issued by the California Structural Pest Control Board License to the individual(s) identified as the company’s qualifying manager(s) with their proposal. The Contractor shall provide a copy of

Employees State of California Structural Pest Control Board Applicators License Branch 2 and 3 to the Installation Pest Management Coordinator (IPMC) within ten (10) calendar days of contract award or prior to application, whichever comes first. The Contractor’s technician shall contact the

Contract Officer Representative (COR) upon arrival to Edwards AFB, NASA, AFP42 or AFRL and prior to departure from the installations.

1.1. INTEGRATED PEST MANAGEMENT PLAN.

1.1.1. The Contractor shall develop and submit an IPM plan for approval to the IPMC within ten

(10) calendar days after contract award. The IPM plan shall be written IAW the outline prescribed in DoDI 4150.07 (Enclosure E) and shall adhere to sections of DoDI 4150.07 and AFMAN 32-

1053 that apply to contract pest management operations. All pesticide applications shall be recorded IAW Government directed procedures. In addition, the IPM plan shall comply with all applicable Environmental Protection Agency (EPA), Occupational Safety and Health

Administration (OSHA), Federal, and State/Local regulatory guidance on pesticide usage, disposal and applications.

1.1.2. The Contractor’s IPM plan shall establish the strategy and methods for conducting a safe, effective, and environmentally sound pest management program. In developing their IPM plan, the Contractor shall address continuous monitoring, pest response and removal procedures, record keeping, warranties, pest education, and communications to prevent pests and disease vectors and bird control on airfields. The IPM approach shall use targeted (effective and environmentally sound) methods, including habitat/facility modification, biologically/genetic/cultural control, mechanical/physical control, and where necessary, the judicious use of least hazardous pesticides.

1.1.3. A list of all pesticides to be used for pest control services shall be included in the IPM plan.

Only pesticides registered for use in the state of California and approved by the Armed Forces Pest

Management Board shall be used. This list is located in Appendix J. The Contractor shall provide electronic copies of the Safety Data Sheets (SDS) and labels to the COR along with the IPM plan.

Appendix J lists the Pesticides currently authorized for use.

1.1.4. The IPM plan requires approval by the Command Pest Consultant prior to implementation.

The IPM plan shall be reviewed and updated annually or as strategies change. The IPM plan shall be submitted to the COR and IPMC within 10 calendar days after contract award, and then prior 10 calendar days prior to the end of each contracted fiscal year and revised when strategies and chemicals change. The plan shall include changes and additions to chemicals and strategies for all pest control categories.

1.2. BASIC SERVICES.

1.2.1. Common Pests. Common pests are described as any insect, invertebrate, arachnid, rodent, mammal, bird, or snake that causes an economic loss, or presents a hazard to Air Force property, personal health or morale. These pests include mice, rats, squirrels, gophers, birds, bats, ants, bees, termites, snakes, mosquitoes, gnats, beetles and spiders. This list is not all inclusive and shall not be considered to be a complete list of the pests the Contractor shall be responsible to control.

1.2.2. Structural Pest Control Services.

1.2.2.1. Monthly Food Facility Inspections Services. Inspections of food service facilities and child care food facilities shall be accomplished on a monthly basis. Inspections shall include facility sanitation, pest exclusion as well as the presence of pests. Facility sanitation and pest exclusion is the responsibility of the Facility Manager. However, preventive measures will be the primary pest management control tool followed by trapping and chemical application if necessary. All inspections and treatments performed within the month shall be included in the monthly service charge. The Contractor shall complete all requests for treatment/service at food preparation and consumption facilities, and child development centers within four (4) business hours of notification, or coordination with the COR or IPMC. Treatments shall be performed prior to the facility opening, or at a time approved by the COR or IPMC. The Contractor shall coordinate all pesticide applications at food preparation and consumption facilities, and child development centers with Public Health, COR and IPMC prior to treatment. The Contractor shall provide a written schedule (email is acceptable) to the COR at least five (5) business days prior to the next service month to the COR and IPMC. Inspections and treatments include all areas of the facility that have food preparation, food storage and dining areas, see Appendix B. These areas shall include adjoining rooms and openings to the exterior of the facility. These areas will include fly control. If fly light traps are utilized, the Contractor shall ensure all light bulbs are working and the capture pad is not saturated with flies. If bulbs are found defective or capture pads are full, the Contractor shall remove and replace as required to control flies. The Contractor shall not use pest electrocution devices (bug zappers).

1.2.3. Service Calls for Commercial and Administrative Facilities. The Contractor shall complete all pest control requests within eight (8) business hours from receipt of request.

Contractor shall coordinate with the facility managers on pest management practices to keep pests under control using the least amount of pesticides. The Contractor shall contact Facility

Manager or alternate prior to any pest control actions.

1.2.4. Removal/Collection of Dead Animals. The Contractor shall remove or collect dead animals along roadways and in common use areas within 24 hours from receipt of request. Large animals along roadways shall be disposed of by moving to a location away from the roadway that is not visible to passing traffic. Large dead animals in common areas shall be disposed of at the base landfill. Small dead animals (rabbits, squirrels, etc.) shall be disposed of in a dumpster, away from the roadway, or at the base landfill. If disposed of in a dumpster, small dead animals shall first be placed in a plastic garbage bag. Facility Managers will be responsible for disposal of dead animals within their respective facility boundaries. Dead dogs and cats shall be taken to the

Base Veterinarian to have them scanned for the presence of an identification microchip before disposal.

1.2.5. Pest Control Service for Athletic Fields, Parks and Improved/Unimproved Common

Areas. Athletic fields parks and common area pest control service requests shall be approved in writing by the COR or IPMC prior to work being accomplished. The Contractor shall provide in writing (email is acceptable) an estimate of the hours required to perform services. The pest control for these areas shall be primarily gophers, squirrels, and rabbits. Other pests may be included, based on the written request from the COR or IPMC. The control method used and notifications shall comply with approved Federal, State, County, DoD, AF and Local regulations.

The Contractor shall start pest control actions within one (1) business day from receipt of written request from COR or IPMC. Notifications shall be posted to advise personnel of pending actions a minimum of 24 hours before treatment and for at least 72 hours after pest control treatment is completed. Service will include posting notification of times personnel are not to enter the area.

For control actions that are scheduled to be started after a weekend, notices shall be posted the last business day before the weekend.

1.2.6. Mosquito and Larvae Control. Contractor shall perform treatment and insure effectiveness of treatment of areas that have been identified as mosquito harborage and breeding areas by Public Health, COR or IPMC. All treatments to identified areas shall be with written approval from COR or IPMC.

1.2.7. Emergency Pest Control Service. Upon notification by the Contracting Officer (CO), COR or IPMC, the Contractor shall perform emergency services to include after business hours and weekends (See 4.0 Hours of Operation). The Contractor shall commence services within one

(1) hour from time of notification during normal business hours and within two (2) hours outside normal business hours. Prior to contract start, the Contractor shall provide the CO, COR and the

IPMC with the contract manager’s complete contact information to include a 24-hour phone number and email. Service shall be performed within specified times and ensure the pests are eliminated/removed. For purposes of this PWS and IAW DoDI 4150.07, emergency services shall be utilized for the control of potential medically important pests, such as venomous

1.2.8. Pest Control Cleanup Services. Products of pest activity, including feces, nesting materials, animal, bird and rodent carcasses shall be removed and disposed of IAW applicable laws. Areas contaminated with pest feces, to include bird droppings, shall be cleaned and disinfected. All requests for Pest Control Cleanup service shall be via written request from the

COR or IPMC. The Contractor shall provide in writing (email is acceptable) a cost estimate of the area that requires cleanup services in square feet to the COR.

1.2.9. West Nile Virus (WNV) Bird Sampling. Test dead birds for West Nile virus using oral swab technique IAW with California Department of Public Health protocol (Appendix H).

Forward WNV test results to the COR and IPMC upon receipt. All requests for WNV testing shall be via written request from the COR or IPMC.

1.3. OVER AND ABOVE (O&A) SERVICES AND PROCEDURES.

1.3.1. O&A Services. O&A services required outside the Contractor’s control. This permits the

Government and the Contractor to negotiate materials and labor, which are within the general scope of the contract and are necessary to perform services beyond routine service calls, and are not specifically identified and priced. The Contractor shall not proceed with the O&A work until a work order has been approved and a Task Order awarded. The Contractor shall commence work and complete work by the mutually agreed upon dates.

1.3.1.1. O&A Procedures. The following procedures shall apply for all O&A work requests pertaining to this contract:

a. The COR shall request an O&A work request form (shown in Appendix G) from the

Contractor for work at any dollar amount per occurrence. The Contractor shall submit the form, with all applicable fields completed, to the COR within five business days for non-emergencies and within one business day for emergencies.

b. All work requests submitted with a total value of less than $3000.00 will be reviewed and approved by the COR. All O&A work requests of $3000.00 or more will be reviewed by the COR, but must be approved by the CO to make the determination of a fair and reasonable price.

c. The COR for this contract shall –

1. Promptly review the O&A work request for technical acceptability and the reasonableness of the price.

2. Verify that the proposed work is required, appropriate, and is not covered under the basic contract line item(s).

3. Verify that funding is available for the O&A work request.

4. The COR shall inform and coordinate with other appropriate Government agencies on base as necessary, in a timely and professional manner.

5. Approval will be done in writing, unless under emergency conditions where oral approval may be used which may only be given by the CO. All oral approvals will be followed up in writing. The approval will establish a firm-fixed price for the work request. Failure of the Contractor to provide the information required on the O&A work request, within the specified time frame, may be grounds for the

Government to openly compete the O&A service. If the Government and the

Contractor cannot agree upon a firm-fixed price for the O&A work request, the

Government has the right to openly compete the O&A work.

1.4. PESTICIDE MIXING AND STORAGE.

All pesticides shall be stored off-base. All unused pesticides, empty pesticide containers and residue shall be disposed of properly at an approved off-base disposal area.

Chemical mixing for immediate application may be accomplished at the site of application/treatment and only state certified applicators may mix or apply pesticides IAW

AFMAN 32-1053, para. 3.15.5. Integrated Pest Management Program (06 Aug 2019).

Contractor shall provide a spill container at mixing areas to insure that no chemicals impact an area that is not being treated. The Contractor shall have an operational emergency eyewash kit available at each mixing location. In the event the Contractor spills or releases any hazardous substances; example, substances listed in Code of Federal Regulation 40(CFR)302, the

Contractor shall immediately notify the COR.

1.5. FACILITY MANAGER TRAINING.

Contractor shall provide training to facility managers, food service managers, on methods to reduce and prevent pest infestations. This training shall be accomplished during monthly food service facility inspections.

1.6. PEST CONTROL RECORDS AND REPORTS.

1.6.1. Pesticide Usage Daily Report. The Contractor shall forward an electronic report of pesticide usage and pest control services provided to the COR, by 1300 each business day following the day services are provided. The report shall include the service call request number and any other comments (See Appendix K).

1.6.2. Pesticide Usage Monthly Report. The Contractor shall forward an electronic report of pesticide usage and pest control services provided for the entire month to the COR electronically within the first five business days of the following month (See Appendix K).

1.6.3. Pest Control Monthly Invoice Summary Report. The Contractor shall provide COR with a monthly summary of services provided (See Appendix L) electronically within the first five business days of the following month.

SECTION II SERVICES SUMMARY

2.0. PERFORMANCE OBJECIVE. The contract service requirements are summarized in performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimally acceptable levels of service required for each requirement. The

Services Summary (SS) and the Contractor’s Quality Control Plan provide information on contract requirements, the expected level of Contractor performance, and the expected method of

Government validation and confirmation of services provided. These thresholds are critical to mission success. Procedures as set forth in the FAR 52.212-4 (a), Contract Terms and Conditions -

Commercial Items, Inspection/Acceptance, will be used to remedy all deficiencies. During the first initial 30 days of the contract, two additional errors on each Performance Objective shall be allowed in an effort to identify normal phase-in problems.

2.1. QUALITY CONTROL PLAN.

The Contractor shall develop and maintain an acceptable quality control plan (QCP) to ensure systematic inspection of facilities for pests, a customer service call/emergency response system for pest removal, and a continuous pest prevention education program for facility managers and occupants, food handling managers, and housing occupants. The Contractor shall develop and implement procedures to identify and prevent defective services from recurring on all services required by this PWS. As a minimum, the Contractor shall develop quality control procedures that address the areas identified in Section II, Services Summary. The Contractor’s QCP shall be written and submitted to the COR and IPMC within 10 calendar days from the date of award for acceptance by the CO and within 30 calendar days prior to the start of each option year or as changes occur. The Contractor shall identify their quality control representative in writing to the

Government COR and IPMC for notification and resolution of valid customer complaints or

Quality Control issues.

2.2. QUALITY ASSURANCE.

The Government will inspect and evaluate the Contractor’s performance to ensure services are received IAW requirements set forth in this PWS. The Contractor shall provide work schedules to the COR IAW paragraph 1.2.2.1. of this PWS. The COR will inspect by validating actual work performance, physically checking an attribute of the completed task, checking a management information report, investigating customer complaints, conferring with facility managers, or otherwise inspecting the task or its results to determine whether or not performance meets the standards contained in this PWS. The COR will use the Contractor’s work schedule or modified version thereof, to record validation results. Results of the validation then become the official Air

Force record of the Contractor’s performance. When a performance threshold has not been met or

Contractor performance has not been accomplished, the COR will initiate and provide the CO a

Corrective Action Report (CAR) for issuance to the Contractor. The Contractor shall respond to the CAR IAW instructions provided and return it to the CO within 10 calendar days of receipt.

Contractor Employee Certification.

Contractor shall have a State of California certified pest control manager specifically assigned to support this contract available within one hour to meet with

Government personnel

(telephonically or in person) during business hours. All pesticide

2.3. MEETINGS.

2.3.1. Meetings. The CO, Functional Commander, COR and other Government personnel as appropriate, and the Contractor shall meet bi-annually on the first Tuesday in the months of

February and August to discuss the Contractor’s performance. The following issues shall be discussed: opportunities to improve the contract, required contract modification(s), unsatisfactory inspections and trends against each performance objective observed, positive performance and steps taken by the Contractor to prevent unsatisfactory occurrences in the future. The Contractor shall provide a summation of unsatisfactory inspections and customer complaints and provide insight into any identified trends.

2.3.2. Meeting Minutes. The minutes of these meetings will be reduced to writing, signed by the

CO, and any other signatures as deemed appropriate, distributed to the functional area and the

Contractor. Should the Contractor not concur with the minutes, the Contractor shall provide a written notification to the CO identifying areas of non-concurrence for resolution within five (5) business days of receipt.

SECTION III GOVERNMENT FURNISHED SERVICES

3.0. GOVERNMENT PROVIDED SERVICES. The Government will provide the following services listed here:

3.1. UTILITIES. Electricity, gas, water and sewage service as necessary for accomplishment of work IAW this contract. Additionally, the Contractor shall adhere to all base level utility conservation practices or requirements. The Contractor shall be responsible for operating under conditions that prevent waste of utilities.

3.2. SECURITY FORCES. The Government will provide general on base Security Forces services. Edwards AFB Security Forces telephone extension is 911 for emergencies and (661)

277-3340 for routine calls. AFP42 Security Forces telephone extension for emergencies is 911 and for routine calls is (661) 272-6600.

3.3. FIRE DEPARTMENT. The Edwards AFB Fire Department telephone extension is 911 for emergencies and (661) 277-3643 for routine calls. The AFP42 Fire Department telephone is (661)

272-6740/6741 for fire dispatch and (661) 272-6628 for the Deputy Fire Chief.

SECTION IV GENERAL INFORMATION

4.0. HOURS of OPERATION.

The Contractor shall perform pest control services during normal business hours which are 0700-

1700, Mondays through Fridays, excluding Federal holidays. Emergency services may be required after normal business hours, weekends or Federal holidays. Pest control services for

AFP 42 shall be performed during normal business hours which are 0615-1445, Monday through

Friday, excluding Federal holidays.

4.1. CONTRACT MANAGER.

Contractor shall provide a Contract Manager and Alternate Contract Manager responsible for the performance of work under this contract along with a 24 hour phone number for emergencies.

The name of this individual and alternate shall be designated in writing to the CO and COR within ten (10) calendar days of award of the contract. The Contract Manager or alternate must be able to read, write, speak, and understand the English language, both spoken and written. The

Contract Manager shall ensure employees have current personal identification and valid certifications required for the performance of work to be accomplished.

4.2. SPECIAL QUALIFICATIONS.

The Contractor shall have a State of California certified and licensed pest control manager specifically assigned to support this contract available within one hour to discuss issues with

Government personnel during required service hours. The Contractor shall have and maintain a current California Department of Pesticide Regulation Pest Control Business Main License. All

Contractor employees shall comply with California Code of Regulations Title 3, Division 6.

All Contractor employees that apply pesticides shall have a California pesticide application certification. On DOD land and property, direct supervision is not permitted for

Contractor’s applicators. The Contractor shall be responsible to ensure that the licenses, certificates and training of Contractor personnel are kept current. The Contractor shall provide a copy of each Employees State of California Structural Pest Control Board Applicators License

Branch 2 and 3 that will be applying chemicals on Edwards AFB to the COR and IPMC within

10 days of contract award or prior to application, whichever comes first. An updated copy will be supplied each time there is a change to the license/certification. The applicators shall have in their possession and utilize Personnel Protective Equipment as required by the SDS and label.

4.3. CONTRACTORS PERSONNEL APPEARANCE.

Contractor’s personnel shall present a neat appearance and be easily recognized. This may be accomplished by wearing distinctive clothing bearing the name of the company or wearing badges that contain the company name, employee name and photo.

4.4. CONTRACTOR VEHICLES.

Contractor’s vehicles must clearly display the name of the company on them. All Contractor vehicles must be maintained and in working order IAW all Environmental Protection Agency

(EPA) guidelines and used IAW all manufacturers’ vehicle safety recommendations. All vehicles must be registered and insured.

4.4.1. Vehicle Operation. Personnel operating motor vehicles shall comply with State of

California, Edwards AFB and AF Plant 42 requirements at all times. All Contractor employees that are required to operate vehicles in the flight line area shall successfully complete the

Edwards AFB Flight Line Vehicle Training requirements, and maintain certification, before operating any vehicle on the flight line.

4.5. SAFETY REQUIREMENTS AND REPORTS.

4.5.1. The Contractor shall comply with all applicable parts of Title 29, CFR, OSHA, Part 1910;

Title 29, CFR, Safety and Health Standards for Federal Service Contracts, Part 1925; Title 40, CFR, Parts 150-189, and Title 49, CFR, Hazardous Materials Regulations, Part 171, while on an

Air Force installation, to ensure safe working conditions for contract personnel and a safe environment for the occupants of Air Force facilities.

4.5.2. General Safety and Health Responsibilities and Accountability. The Contractor bears sole responsibility for compliance at all times with Federal, State, and Municipal safety and health laws, regulations, and standards that apply to the work performed under this contract, including the

OSHA. The Contractor shall take all reasonable steps and precautions to maintain compliance with applicable regulations/requirements, prevent accidents, and preserve the health and safety of contractor and government personnel performing or in any way coming in contact with the performance of the work required by this contract. The Contractor shall also take additional immediate actions/precautions as requested by the CO when the CO determines that such measures are necessary for safety and health purposes. Contractor/subcontractor supervisors shall monitor worksites and operations daily to ensure safe working conditions and continuing compliance.

Hazardous or noncompliant conditions identified by the CO shall be promptly corrected, and corrective action(s) taken shall be reported in writing to the CO within five (5) days of the CO’s notification to the contractor. In cases of serious and/or recurring hazards/noncompliance, the contracting officer may stop the work in question pending correction of the hazardous/noncompliant condition, and/or order the removal of the involved

Contractor/subcontractor employees from the government installation/ facility. In such cases, the

Contractor will not be granted a cost adjustment nor an extension of time due to the work stoppage or employee removal action. Failure of the Contractor to effectively resolve serious and/or recurring hazards or noncompliance may result in termination of the contract.

4.5.3. Compliance. The Contractor shall submit a Safety and Health Plan and corresponding site safety checklist to the CO within 15 business days after contract award. The Contractor’s plan shall include appropriate measures to ensure the Contractor reacts promptly to investigate, correct and track alleged safety and health violations and/or uncontrolled hazards in Contractor work areas. Additional, installation specific references and policies may be included or attached to this section. The plan shall include:

Plan Requirement

1. Demonstrate a management commitment to employee safety and health;

2. Identify the application of the safety and health plan to sub-Contractors;

3. Identify the roles and responsibilities of the following individuals: management;

supervisors; employees; safety coordinator; Provide names and contact information for key contractor personnel, and alternates, who may be contacted concerning safety and health matters or emergencies;

4. Identify applicable safety rules and regulations;

5. Include a worksite hazard analysis to include base-line hazard identification and required control measures;

6. Include a job site analysis to include hazards of tasks required to control measures;

7. Identify employee safety and health training requirements and the documentation process;

8. Include a workplace inspection frequency, to include identifying the individual conducting the inspections;

9. Include employee hazard reporting procedures;

10. Identify individual(s) responsible for corrective action hazards;

11. Identify first aid/injury procedures;

12. Identify procedures for accident investigation and reporting;

13. Identify emergency response procedures; and

14. Identify the process for tracking controlled hazards in Contractor work areas

Contractor is responsible for establishing these requirements for all sub-Contractors who qualify as sub-Contractors under the resulting contract. Reporting and Investigation of Mishaps, Injuries, and

Illnesses. The Contractor shall immediately notify the CO of any work related mishaps that involves, or appears that it may involve, fatality, total or partial permanent disability of any employee, inpatient hospitalization of any employee, or property damage over $1,000, regardless of whether such matters involve Government or private personnel/property. The Contractor shall also notify the CO of any illness or injury that is, or appears that it may be, an OSHA recordable illness/injury as defined by 29 CFR 1904 as soon as reasonably possible, but not later than on the first (1) business day following the day on which the illness/injury occurred, regardless of whether such matters involve Government or non-government personnel. The Contractor shall investigate all mishaps, illnesses, and injuries, report the results of such investigations, and cooperate with any

Government investigation of such matters, as requested by the contracting officer.

4.6. SECURITY REQUIREMENTS.

All Contractor employees will be subject to a criminal history background check IAW Air Force

Manual (AFMAN) 31-113, Installation Perimeter Access Control, and 412th Test Wing Integrated

Defense Plan (IDP) 31, Volume I (the AFMAN and IDP are both limited access documents but more details on the requirements/process in terms of background checks can be provided by the

Contracting Office upon request). Vehicle registration, proof of insurance, and a valid driver's license must be presented for vehicles. Installation Commanders may deny access and credentials based on information obtained during identity vetting that indicates the individual may present a threat to the good order, discipline, and morale of the installation.

Base Access for Contractor personnel who wish to obtain long-term Base access via an

AFMC)Form 496, Application for AFMC Identification Card, IAW AFFARS Clause 5352.242-

9000, Contractor Access to Air Force Installations, the Prime Contractor shall submit a written request on company letterhead to the CO with the following information (prior to submitting a request for Base access via an AFMC Form 496): 1) contract number, 2) location of work, 3) expiration date (must be on or before contract’s expiration date), and 4) names of employees/ subcontractor employees needing access to the base, to include each employee’s company name.

The Contractor shall meet with the COR at least 30 days prior to Base pass expiration in order to insure ample time for the Government to process appropriate documents.

It is recommended that the Prime Contractor provide a copy of the request letter to each employee for verification purposes when the employee comes to the Contracting Office at Building 2800 to get their AFMC Form 496 signed. Employees for which no request letter can be found or provided will result in the employee being asked to return on another date/time so verification can be made.

Base passes will be limited to the length of the contract not to exceed 3 years from the date the

AFMC Form 496 is submitted to the Contracting Office for signature.

The AFMC Form 496 will be reviewed/approved by the CO, then taken by the Contractor employee to the Edwards AFB Pass & Registration Office in Building 3000 for the security portion and issuance of the Base pass. To complete this process for employees who will perform pest control services at AFP 42; the AFMC Form 496 will be reviewed/approved by the CO, then taken by the Contractor employee to the Plant 42 Pass & Registration office located in Building

560, second floor.

The Contractor shall comply with all base security regulations. Additionally, the Contractor shall comply with actions dictated by force protection condition changes/measures. For real-world situations/exercises, changes to force protection conditions could impact Contractor performance of work and can include denial of access to the installation to perform work

Only employees who will be working on the contract consistently/frequently should be submitted for a long term Base pass.

The Contractor shall comply with all security requirements imposed by the Installation. Specific security considerations will be handled on an individual basis as required.

The contractor shall report suspicious activity (refer to Edwards AFB Contractor Antiterrorism Awareness pamphlet) to Security Forces at 661-277-3340 in a timely manner.

The Contractor and its employees shall comply with all base traffic regulations.

No States are subject to Real ID Act enforcement at this time. The Department of Defense will accept standard State-issued driver's licenses and identification cards from all U.S. States and territories as proof of identity. Please visit the Department of Homeland Security's (DHS) website to get the latest information, https://www.dhs.gov/current-status-states-territories. Once Real ID

Act is enforced, the Contractor shall comply fully.

Contractor personnel who require routine physical access to the installation shall comply with

Level I - Antiterrorism Awareness training requirements. Routine physical access applies to

Contractor personnel whose contract, performance work statement/statement of work has a 45 day or more period of performance. AT Level I training provides contractor employees with the requisite knowledge necessary to remain vigilant for possible terrorist threats. AT Level I training can be taken at https://jkodirect.jten.mil/Atlas2/page/login/Login.jsf for non-Common Access

Card (CAC) holders. CAC holders may complete AT Level I training (Force Protection course) via the Advanced Distributed Learning Service at https://golearn.adls.af.mil/login.aspx. Tracking for each contractor or subcontractor employee is the responsibility of the COR or unit

Antiterrorism Representative.

The Contractor shall be responsible for safeguarding all Government property, equipment and materials provided for Contractor use. At the end of each work period, all Government facilities, equipment, and materials shall be secured.

During performance of the contract, the Contractor shall be responsible for obtaining required identification for newly assigned personnel.

The Contractor shall be responsible for prompt return (within three (3) calendar days) of credentials and vehicle passes for: 1) any employee who no longer requires access to the work site;

2) upon request by a CO; and 3) upon completion or termination of the contract. The Prime

Contractor shall ensure that all Base identification passes issued to employees and subcontractor employees are returned to the issuing office. Failure to comply with these requirements may result in withholding of payment.

Contractor employees are prohibited from possessing weapons, firearms or ammunition, on themselves or within Contractor-owned or privately owned vehicle while on Edwards AFB.

The Contractor shall request base access through the COR for each employee via phone call, three

(3) business days prior to (1) Site Visit (2) Performance of Work.

4.7. ENVIRONMENTAL CONTROLS.

4.7.1. The Contractor shall comply with Hazardous Material Identification and Safety Data Sheet requirements. The Contractor shall ensure all hazardous materials are authorized and managed in compliance with applicable sections of Air Force Instruction 32-7086, Hazardous Materials

Management and Edwards AFB Hazardous Materials (HM) guidance to ensure compliance with

Emergency Planning and Community Right-to-Know Act of 1986 (42 U.S.C. 11001–11050), and the Pollution Prevention Act of 1990 (42 U.S.C. 13101–13109). The Contractor shall list all hazardous material to be delivered under this contract on the Contractors Hazardous Materials

Usage Tracking Form (HMUTF) form, (Appendix M).

4.7.2. The Contractor shall complete and provide the HMUTF form to the Contracting Officer

(CO) prior to the contract start listing all HM (i.e., any chemicals/materials procured for this project and prior to the hazardous material entering the installation for approval by 412

CES/CEVC. In addition, A Safety Data Sheet (SDS) for each Hazardous Chemical requested that is listed in OSHA Hazard Communication Standard 29 CFR 1910.1200. shall accompany the

HMUTF form. After initial notification of approval, the Contractor must continue to provide updates quarterly to the COR by the 5th business day of the month following each quarter until the throughout the term of contract. The Contractor should contact 412 CEG/CEVC at (661) 277-

1401 or via e-mail at 412CEG.CEVC.HMWorkflow@us.af.mil for guidance regarding tracking of

HM usage and regulatory requirement information. Contact AFRL at 275-5135 for specific AFRL guidance prior to project initiation. The COR will forward all related HM documents to the 412

CEG/CEVC HM Program Manager.

4.7.3. Any products that meet the criteria of “Hazardous Waste” must be bar coded and tracked until permanently removed from government property. Criteria for identifying hazardous waste are contained in Subpart C of 40 CFR Part 261. NOTE: Products containing chlorofluorocarbons

(CFC’s) or ozone depleting substances shall not be allowed or approved for use.

4.7.4. The contractor shall ensure that all contractor employees complete the local Air Force

Hazardous Communication and hazardous waste training. The contractor shall appoint a primary and alternate HAZMAT and Hazardous Waste monitor. Monitors are responsible for coordinating training of all contractor personnel regarding hazardous material containers maintained on base and complete SDSs immediately upon receipt of new chemicals, products, or materials through

412 CEG/CEVC.

4.7.5. The Contractor shall comply with Pollution Prevention and Right-to-Know Information requirements. Control of environmental pollution requires consideration of air, water, and land uses, and involves the management of noise, solid waste, thermal energy, and radioactive materials, as well as other pollutants. Protective measures shall be incorporated to control pollution and/or correct environmentally impacting conditions that may develop resulting from this contract.

Environmental protection shall be provided during the life of the contract.

4.7.6. The Contractor shall comply with Environmental Management Systems requirements. The

Contractor’s work under this contract shall conform with all operational controls identified in the applicable agency or facility Environmental Management Systems and provide monitoring and measurement information necessary for the Government to address environmental performance relative to the goals of the Environmental Management Systems.

4.7.7. The Contractor shall comply with Aerosols requirements. Unless otherwise specified in the contract, the Contractor shall reduce its use, release, or emissions of high global warming potential hydrofluorocarbons, when feasible, from aerosol propellants or solvents under this contract. When

4.7.8. The Contractor shall comply with Health and Safety on Government Installations requirements. Any violation of these health and safety rules and requirements, unless promptly corrected as directed by the contracting officer, shall be grounds for termination of this contract in accordance with the Default clause of this contract.

4.7.9. The Contractor shall be solely responsible for obtaining at its cost and expense any environmental permits required for its operations under the Contract, independent of any existing permits held by the Government. Any and all environmental permits required for any of the

Contractor’s operations or activities would be subject to prior concurrence of Government. The

Contractor acknowledges that the Government will not consent to being named a secondary discharger or co-permittee for any operations or activities of the Contractor under the Contract. In the event the Government is named as a secondary discharger or co-permittee for any activity or operation of the Contract, the Government shall have the right to take reasonable actions necessary to prevent, suspend, or terminate such activity or operation, including terminating this Contract, without liability or penalty.

4.7.10. Right to Inspect. The Government’s rights under this Contract…
