About this file

This is a Master Sub-Grantee Services Agreement between the City of Bridgeport, Connecticut, through its Department of Health and Social Services, and a consultant or sub-grantee to provide services under grant-funded projects. The agreement establishes a five-year master contract framework under which the City will issue individual Task Orders to the consultant for specific services related to grant-funded initiatives. Services will be assigned through written Task Orders that specify scope, schedule, and compensation for each assignment, with the consultant required to commence services within five business days of receiving a Notice to Proceed. The consultant must maintain contemporaneous daily time records submitted bi-weekly to the City and is expected to perform services in a timely manner according to City-approved schedules. This particular solicitation seeks a Youth Council Coordinator under the Best Practice Tobacco Control Grant.

Compensation will be determined on a task-by-task basis as specified in individual Task Orders, with the consultant billing bi-weekly by itemized invoice for services rendered, payable by the City within 45 days of receipt. All funding is contingent upon grant availability and appropriations from the funder, with the City retaining the right to terminate immediately if grant funding is revoked or terminated. The consultant must maintain professional liability insurance of at least $1,000,000 per claim, workers' compensation insurance per Connecticut law with $100,000 employer's liability limits, commercial general liability of $1,000,000 per occurrence, and automobile liability of $1,000,000 per accident, with all policies carrying thirty-day cancellation notice provisions. The consultant is classified as an independent contractor responsible for all payroll taxes and employee benefits, must obtain prior written City approval before subcontracting any work, and is required to comply with all grant terms, conditions, and Connecticut non-discrimination laws.
