About this file
This is Addendum #1 to an Invitation to Bid (ITB) issued by the Florida Department of Law Enforcement (FDLE) for oral swab collection kits to support the DNA Investigative Support Database. The addendum, issued April 6, 2026, provides written responses to 15 vendor questions and includes amended pages 17-18 with updated artwork specifications for the collection kit form. The procurement seeks an estimated 120,000 to 180,000 oral swab collection kits annually, with each kit containing a sample collection card (either Qiagen QIAcard FTA Indicating Micro catalog WB120211 or Thermo Scientific Nucleic Card Color Matrix catalog 4473974), a sterile sponge-tipped swab (Puritan Medical Products catalog 25-1805 1PF RND or approved equivalent), and related packaging and forms. Bid responses were due April 15, 2026 at 2:00 PM, with physical kit samples submitted to 813B Lake Bradford Road. Public bid opening is scheduled for April 17, 2026 at 10:00 AM via conference call. The resulting contract will have a one-year initial term with three optional one-year renewal periods at FDLE's discretion, with monthly deliveries of a minimum of 10,000 kits required to FDLE Headquarters at 2331 Phillips Road, Tallahassee, Florida 32308, with supplemental orders up to 5,000 kits accommodated at no additional charge within fifteen calendar days.
The addendum clarifies that kit assembly facilities do not require FDA registration or ISO 13485 certification but must follow documented FDA Good Manufacturing Practices and contamination control procedures including use of sterile gloves and masks. Bidders must submit separate unit pricing for two sample collection card options (with and without cards provided by FDLE) and two packaging configurations with no additional tooling or administrative fees. FDLE reserves the right to change outer packaging configuration upon 90 days' advance written notice. Renewal year pricing shall reflect the initial per-kit price increased by the maximum allowable 3 percent annually. If FDLE provides sample collection cards, FDLE covers all card and shipping costs and will coordinate delivery timing with the contractor's production schedule. The previous contract experienced supply chain shortages, and anticipated annual kit orders range from 120,000 to 180,000 units. The solicitation incorporates Florida's standard General Instructions to Bidders (PUR 1001, 2006 version) and General Contract Conditions (PUR 1000), with all communications directed to Angela Githens, Procurement Officer at FDLEOGSContracts@fdle.state.fl.us.

WRITTEN ANSWERS TO QUESTIONS
FDLE ITB 2610
Oral Swab Collection Kits
All written questions are reproduced in the same format as submitted by the Vendor.
Question #1 Section 2.1(F) specifies the Sample Collection Card shall be either an FTA Indicator Card or a Nucleic Card Color Matrix — 1 spot. Can FDLE confirm the specific manufacturer name and catalog number(s) for each acceptable card type currently in use in the FDLE DNA Investigative Support Database program, so that bidders can confirm compliance with the specification?
Answer #1 Qiagen - QIAcard FTA Indicating Micro (100), 1 sample area per card https://www.qiagen.com/us/products/human-id-and-forensics/sample-collection/indicating-fta-cards?catno=WB120211 Cat. No. WB120211 Indicating FTA Cards 100 cards Or Thermo Scientific™ NUCLEIC-CARD™ COLOR matrix, 1 spot Catalog number 4473974
Question #2 Secon 2.1(E) specifies the sponge-pped sterile swab must be idencal to Hardwood Products Company Product/Catalog #25-1805 1PF RND or an approved equivalent. Can FDLE confirm whether this product is currently manufactured and sold under the Puritan Medical Products brand, and whether Puritan Medical Products catalog number 25-1805 1PF RND is considered idencal and therefore acceptable without requiring addional equivalency documentaon?
Answer #2 Yes, this is considered identical to the Puritan Medical Products Brand
Question #3 Section 2.3 requires kit assembly using sterile gloves and mask, and Section 2.0 references FDA Good Manufacturing Practices for the manufacturer of sterile components. Can FDLE clarify whether the kit assembly facility itself must be FDA-registered and/or ISO 13485 certified, or whether documented GMP-compliant assembly procedures are sufficient to meet this requirement?
Answer #3 The FDA Good Manufacturing Practices (GMP) requirements referenced in Section 2.0 apply to the manufacturer of the sterile components for the Oral Swam Collection Kits.
FDLE does not require the kit assembly facility to be FDA-registered or ISO 13485 certified.
The Contractor shall ensure that the kit assembly is performed in accordance with Section 2.3, including the use of appropriate contamination control practices such as sterile gloves and masks to prevent contamination of the sample collection components.

Question #4 Section 2.2.1 states FDLE may elect to provide the Sample Collection Cards to the Contractor. If FDLE exercises this option, can FDLE describe the logistics by which cards would be delivered to the Contractor — specifically, who initiates delivery, how frequently cards would be shipped, and whether delivery would align with the monthly kit production and delivery schedule?
Answer #4 If FDLE provided the FTA cards, FDLE would order the set amount of FTA cards and have direct delivery of cards to the vendor. FDLE would cover the cost of the cards and the shipping of the FTA cards to vendor. The winning vendor would provide a schedule for delivery and FDLE would attempt to meet the schedule.
Question #5 Section 2.4 requires physical kit samples to be submitted with the Bid by April 15, 2026 at 2:00 PM. If kit samples are shipped via a separate package from the bid documents but arrive at the same delivery location on the same date and prior to the 2:00 PM deadline, would the samples be considered timely submitted and compliant with this requirement?
Answer #5 Bid submissions may be shipped separately, however please be sure to label each package clearly as detailed on page 23, section 4.9 Bid Packaging and Submission Requirements.
Question #6 Section 2.2.2 states FDLE reserves the right to change the selected outer packaging configuration during the contract term upon written notice. Can FDLE clarify what the minimum advance written notice period would be before a packaging configuration change takes effect, so that the Contractor has adequate time to adjust production planning and inventory accordingly?
Answer #6 FDLE will give an advance notice of 90 days prior.
Question #7 Section 2.5 establishes an automatic monthly delivery schedule but does not specify a delivery address. Can FDLE confirm the complete facility name and physical address where monthly kit deliveries are to be made by the Contractor?
Answer #7 FDLE Headquarters location Attn: DNA Investigative Support Database 2331 Phillips Road Tallahassee, FL 32308
Question #8 Appendix B — Bid Price Sheet includes columns for per-kit pricing for the Initial 12-Month Term and each of the three optional renewal years. Should bidders enter their actual proposed per-kit price for each renewal year based on their individual cost projections, or should the renewal year columns simply reflect the prior year’s price increased by the maximum allowable 3% for each renewal year?
Answer #8 The Initial per-kit price increased by the maximum allowable 3% for each renewal year.
Example:
Initial per-kit cost $3.48 x 120,000 = $417,600 3% increase each renewal year:
1st Renewal Year: $3.58 x 120,000 = $429,000 2nd Renewal Year $3.69 x 120,000 = $442,800 3rd Renewal Year $3.79 x 120,000 = $454,800
Question #9 Can FDLE provide the full contract price from the previous contract?
Answer #9 The FDLE contract referenced can be found here by searching FDLE-020-23 https://facts.fldfs.com/Search/ContractSearch.aspx
Question #10 What was the actual annual usage of kits under the previous contract?
Answer #10 The actual kits ordered per year varies. The anticipated orders will range from 120,000 to 180,000 annually.
Question #11 What challenges did FDLE experience with the previous contractor’s performance?
Answer #11 Supply chain shortages
Question #12 Are there any approved manufacturers or previously accepted suppliers for kit components?
Answer #12 Bidders must select which manufacturers they believe will provide the best value and supply chain requirements. Bidders proposing an equivalent product shall submit manufacturer specifications and supporting documentation with their Bid demonstrating that the proposed product meets or exceeds all material specifications identified in this Solicitation.
Question #13 Do the bid docs need to be physically delivered at the address specified or can they be emailed to FDLEOGSContracts@fdle.state.fl.us?
Answer #13 Electronic submission or facsimile transmission of Replies are not authorized and will not be accepted. Please see Section 4.9 for Bid Packaging and Submission Requirements.
Questions #14 The physical sample kit is to be sent to 813B Lake Bradford Road, correct?
Answer #14 Correct. Please see Section 4.9 for Bid Packaging and Submission Requirements.
Question #15 The artwork provided in the bid specs appears to be from an older version. Are we ok to provide our sample with the artwork that is currently being used in the kits?
Answer #15 Artwork samples have been updated as part of Addendum #1. Bidders shall base their submissions, including sample kits, on the updated artwork provided in Addendum #1.
Samples should reflect the specifications included in the bid document.
