FDA-SOL-1225695 Attachment 1 Amendment 1

Question 1: Do we use Standard Form 330 revision 3/2013 or 8/2016? (Revision 3/2013 does not allow you to edit all fields.)

Answer: Use the most current revision.

Question 2: a) In part 1, paragraph C. (Proposed Team), do we just list the disciplines that will be provided by the contractor or do we list the subcontractors we would use in the event that there is more than MEP work on a given project. b) Referring to page 36 from Attachment J-1, paragraph 15.0 states that ‘the MEP engineering firm, as a minimum, shall provide qualified personnel for each of the following disciplines: Architect, Mechanical Engineer, Electrical Engineer, Structural Engineer’, etc. Is this for the RFQ or for specific projects when they would occur?

Answer: a) In part 1 paragraph C there is a check box to identify Prime/J-V Partner/Subcontractor. The proposed team section should be completed for the prime contractor and subcontractors.

b) Yes, all labor rates for each of the disciplines identified in section B-3 of the solicitation.

Question 3: I assume we fill in the ‘Schedule of Negotiated Contract Labor Rates’ in our response to the RFQ. Is this correct? Would it just be our labor rates or also those of our sub-consultants?

Answer: Yes, all labor rates for each of the disciplines identified in section B-3 of the solicitation.

Question 4: In part 1, paragraph F.23 (Project Owner Information), does this need to be the owner of the given facility or our client for the example project? (Typically our client is an architect and we don’t always have access to, or interaction, with the facility owner.)

Answer: Owner, the client may be provided in the description.

Question 5: In part 1, paragraphs E. and G. for ‘Key Personnel’, do we just list the individuals filling the three roles listed under paragraph G-3 HHSAR 352.237-75 Key Personnel in Standard Form 330 (Project Manager/Engineer, Lead Electrical Engineer, Lead Mechanical Engineer), or is it OK to list more than those three? Is there a limit?

Answer: No, Only the three identified in the solicitation shall be provided.

Question 6: One of the projects we intend to use for our example projects is currently in construction and is due to be completed in the next few months. And, we understand that this is acceptable. However, we would also like to use this same project for Client Past Performance Evaluation. Is this acceptable as well?

Answer: Section L-4, under A/E Firm Qualifications Proposal, Paragraph 4 states, “Offerors shall identify up to five (5) recent and relevant contracts which have been 100% or substantially completed within the last three (3) years…” Past performance evaluation questionnaire evaluators shall be able to answer all questions and confirm that the contract is substantially complete.

Question 7: I was reading document FDA-SOL-1225695.pdf and it states under the title A/E Firm Qualifications Proposal point 5., “Location in the general geographical area of the project and know of the locality of the project in Jefferson, AR (within 100 miles).” Does this mean that we need to located within 100 miles of Jefferson, Arkansas?

FDA-SOL-1225695 Attachment 1 Amendment 1

Answer: Yes. Distance measurements will be determined using Google Map directions by public traveled roadway one-way from contractor’s facility to NCTR facility (3900 NCTR Rd, Jefferson, AR 72079). The Government will use the address associated with prime contractor’s SAM.gov registration with the DUNS number provided in the proposal to verify the address.
