About this file
This is a FAQ document for grant opportunity HHS-2025-ACL-CIP-NWGV-0029 "Enhancing State No Wrong Door Systems for Efficient Access to Long-Term Services and Supports" issued by the Department of Health and Human Services Administration for Community Living (ACL) in February 2025.
The grant will fund up to 10 awards ranging from $350,000 to $450,000 for a two-year project period. Eligible applicants include domestic public/private non-profits, state/local governments, tribal organizations, faith-based organizations, community organizations, hospitals, and higher education institutions. These state agencies should be involved in both the application and grant process. Refer to the FOA, pg. 7, under “Other Requirements” for the agencies that should be represented.
15. Question: At the end of the grant’s two years, is it acceptable if the proposed work is not completed, but a “roadmap” is in place?
