PERFORMANCE WORK STATEMENT
351st Battlefield Airman Training Squadron, Mobile Latrine, Service Support Kirtland AFB, NM 87117
20 Dec 17
1. The 351st Battlefield Airman Training Squadron (351 BA TS) conducts training near Magdalena, New Mexico and Grants, New Mexico and requires portable latrines in order to meet Federal and State land use agreements. The training location requires suitable portable latrines that can be staged before training starts and pick-up when training is complete.
2. The contractor shall appoint a Point of Contact (POC) who will have responsibility for the performance of work. The Point of Contact will also have authority to act for the contractor on all matters related to the contract.
3. Training is performed eight times a year for duration of 7-8 days at a time. A service call will be place no later than five business days prior to need. The contractor will furnish all labor, tools, parts, materials, facilities and transportation to provide these services. The contractor will deliver the portable latrines to the training location no later than 0800hrs on the first day of training and pick-up/transport the portable latrines from the training location no earlier than 1600hrs on the last day of training. Servicing must be performed within OSHA regulations. Requirements for each training event includes the following:
• Transport and deliver eight (8) portable latrines to the training location on the first day of training.
• Pick-up and transport eight (8) portable latrines from the training location on the last day of training.
• Each latrines must have a hand sanitation station
• Each latrines must be fully stocked with four toilet paper rolls
Please note, access to the training location will require off-road, all weather type vehicles.
4. The contractor will be licensed and insured. No Security Clearance is required. The contractor shall perform all phases of the work from start to completion and coordinate with Maj Eric Hansen (505-853-4162) prior to beginning of work. Other 351st BA TS personnel authorized to place calls against services:
• 351 BA TS Director of Operations
• 351 BA TS Logistics OIC
• 351 BA TS Logistics NCOIC
• 351 BA TS OPS Superintendent
• 351 BA TS Tactics Block Lead
• 351 BA TS Land Navigation Block Lead
• 351 BS TS Resource Advisor
5. The contractor will protect all government and personnel property from damage. Any damage caused by the contractor will be replaced or repaired at contractors cost prior to payment. Contractor will abide by all OSHA Regulations, concerning proper wear of Personal Protective Equipment (PPE), and Material handling Equipment (MHE) and lifting techniques.
6. A final inspection of the completed work will be conducted between the contractor and an appointed representative of the 351st BA TS prior to payment. All discrepancies noted during the final inspection or during the final acceptance inspection shall be corrected prior to invoice and payment. The area must be completely cleaned and returned to its original state. All inspection/repair documentation will be provided to the 351st BA TS representative.
7. 351 BA TS FY 18 Port-a-John Schedules are subject to change due mission needs. If dates are determined to differ than the below set schedule, the contractor shall be notified 30 calendar days in advance for proper coordination by the Government. Future schedule dates will be determined prior to current period ends. Below are the scheduled periods for service:

6 Nov 2018 – 19 Nov 2018 (Service will only be required for 7 days during this period )
