SECTION I: DESCRIPTION OF SERVICES
1.0. Description of Services.
The Contractor shall provide all management, licenses, personnel, laboratory testing, tools, supplies, equipment, transportation and labor to perform specialized pest control services at the Muroc Lake Golf Course in accordance with (IAW) Air Force Manual (AFMAN), AFMAN 32-1053 Integrated Pest Management Program,” dated 06 Aug 2019 and Department of Defense (DoD), DoD Instruction 4150.07, “DoD Pest Management Program,” dated 29 May 2008 with incorporated changes dated 31 Aug 2018 that applies to contract pest management operations. The Contractor shall provide a copy of their current California Department of Pesticide Regulation Pest Control Business Main License with their proposal. The Contractor shall provide a copy of Employees State of California Pesticide Qualified Applicator License to the Installation Pest Management Coordinator (IPMC) prior to initial performance after contract award.
1.1. Basic Services.
The Contractor shall develop and submit a Golf Course Integrated Pest Management (IPM) Plan to the IPMC within 15 days of contract award. The Plan will be reviewed by IPMC and forwarded to the Command Pest Consultant for approval. All pesticides shall be registered for use in State of California, and approved by Base level and Major Command prior to use on Edwards AFB and be listed on the Armed Forces Pest Management Board approved pesticides. The Contracting Officer Representative (COR) will provide an electronic copy of the DoD Pesticide List to the contractor. The Contractor has the option to use pesticides not listed in the DoD Pesticide List, however they must be approved for use in the State of California, included in the IPMP and approved by the Command Pest Consultant. The Contractor’s plan shall establish the strategy and methods for conducting a safe, effective, and environmentally sound pest management program. The IPM Plan shall be updated whenever there are changes in the requirements. The plan shall include changes in chemicals and strategies for all pest control categories. The plan shall comply with all applicable Environmental Protection Agency (EPA), Occupational Safety and Health Administration (OSHA), and State/Local regulatory guidance on pesticide storage, disposal and applications. The plan shall adhere to all of AFMAN 32-1053 Integrated Pest Management Program,” dated 06 Aug 2019 and DoD Instruction 4150.07, “DoD Pest Management Program,” (Enclosure 5) dated 29 May 2008 with incorporated changes dated 31 Aug 2018 that applies to contract pest management operations. NOTE: Copies of both Instructions are available on the web. All pesticide applications shall be recorded IAW Government directed procedures. The Contractor shall use the outline format in Appendix B to develop the IPM Plan. The IPM plan requires approval by the Command Pest Consultant prior to implementation. The Contractor shall review and update the plan annually and as strategies or chemicals change for all pest control categories. The updated IPMP shall be submitted to the IPMC for approval and 30 days prior to the start of each contract year. 1.1.1. Golf Course Pest Control Schedule. The Contractor shall develop and submit a monthly specialized pest control schedule to the COR within 30 days of contract award and then 30 days prior to each following year. The Contractor shall contact the Muroc Lake Golf Course Maintenance Superintendent seven (7) business days prior to each scheduled service to determine the type of Golf Course specialized pest control required, and coordinate any required application of approved pesticides IAW the Golf Course IPM Plan.
1.1.2. Golf Course Turf Control. Specialized pest control includes but is not limited to fungi, cut worms, yellow patch, fairy ring and other golf course pests being present. Herbicides, Insecticides, Growth Regulators, and Fungicides will be applied as required to maintain the health of the golf course turfs. Specialized pest control includes but is not limited to pre-emergent, contact herbicide, and broadleaf weed control. Applications to Golf Course greens, collars and other areas shall include a marker dye that will be visible for a minimum for two (2) days that clearly identifies the areas that have been treated. Measurements for the Golf Course Greens and collars are listed in Appendix C. The Golf Course Maintenance Superintendent will identify areas on the Golf Course that require specialized pest control such as cart paths, clubhouse area, Golf Course pond etc.
1.2. Pesticide Mixing and Storing.
All pesticides shall be stored off base. Only CA State Certified applicators shall mix or apply pesticides. All unused pesticides, empty pesticide containers, and residue shall be disposed of properly at an approved off base disposal area. In the event the Contractor spills or releases any hazardous substances (example, substances listed in 40 CFR 302), the Contractor shall immediately notify the COR, IPMC, and base environmental coordinator. Minimal chemical mixings may be accomplished at the site of application/treatment only. Contractor shall provide appropriate spill containment devices at mixing areas during mixing to insure that no chemicals impact an area that is not being treated. For large quantity mixing, Contractor shall have an operational emergency eyewash station available within 25 feet.
1.3. Pesticide Application Equipment.
The Contractor shall use a boom type sprayer unit to evenly apply pesticides at the proper application rate to the Golf Course greens and collars. Sprayer shall be equipped with proper turf tires as to not damage greens or putting surfaces. Manual, back-pack or hydraulic sprayers with hand applicators may be used to perform small area and spot weed control.
1.4. Specialized Pest Control Records and Reports.
1.4.1. Pesticide Usage Daily Report. The Contractor shall forward an electronic report of pesticide usage and pest control services provided to the COR within 3 business days following the day services are provided. The report shall include the type of service provided and any other comments (See Appendix G).
1.4.2. Pesticide Usage Monthly Report. The Contractor shall forward an electronic report of pesticide usage and pest control services provided for the entire month to the COR electronically within the first five business days of the following month (See Appendix G).
SECTION II: SERVICES SUMMARY
2.0. Services Summary.
The contract service requirements are summarized into performance objectives that relate directly to essential services. The performance threshold briefly describes the minimally acceptable levels of service required for each requirement. The Services Summary (SS) and the Contractor’s Quality Control Plan (QCP) provide information on contract requirements, the expected level of Contractor performance and the expected method of Government surveillance and confirmation of services provided. These thresholds are critical to mission success. Procedures as set forth in the FAR 52.212-4 (a), Contract Terms and Conditions - Commercial Items, Inspection/Acceptance, will be used to remedy all deficiencies.

2.1. Quality Control Plan (QCP).
The Contractor shall develop and maintain a QCP to provide a systematic approach to perform effective specialized pest control at the Muroc Lake Golf Course. The plan shall be written and submitted to the COR for approval within 30 days of contract award, prior to any changes, and 30 days prior to the start of the following contract year. Contractor shall develop and implement procedures to identify and prevent defective services from recurring. As a minimum, the Contractor shall develop quality control procedures that address the areas identified in Section 2, Services Summary. The Contractor shall identify in writing their Quality Control Inspector (QCI) to the Government COR for notification and resolution of valid customer complaints or Quality Control issues.
2.2. Quality Assurance.
The Government will evaluate Contractor’s performance IAW the Quality Assurance Surveillance Plan (QASP), through periodic on-site inspections of Contractor’s performance or receipt of complaints from base personnel. The Government may inspect each task as completed or increase the number of inspections if needed due to repeated failures discovered during inspections or because of repeated customer complaints. Likewise, the Government may decrease the number of inspections if performance dictates. The Government will also receive and investigate complaints from various customers located on the installation.
2.3. Customer Complaints.
The COR will receive and investigate any complaints. The COR shall be responsible for initially validating customer complaints. The CO shall make final determination of the validity of customer complaint(s) in cases of disagreement with the COR and the Contractor. Validated customer complaints shall be re-performed after notification by the COR. Exceeding the performance threshold for performance objectives in any one month period may result in a warning or letter of concern from the CO. Exceeding the performance threshold for performance objectives for any two, or more consecutive or non-consecutive months during a contract period may constitute an immediate Progress Meeting with the Multi-functional Team. All remedies shall be IAW the FAR 52.212-4 (a), Contract Terms and Conditions - Commercial Items, Inspection/Acceptance.
SECTION III: GOVERNMENT PROVIDED FACILITY AND SERVICES
3.0. Government Provided Facility and Services.
The Government shall provide the following services listed here.
3.1. Utilities.
Electricity, gas, water and sewage service as necessary for accomplishment of work IAW this contract. Additionally, the Contractor shall adhere to all base level utility conservation practices or requirements. The Contractor shall be responsible for operating under conditions that prevent waste of utilities.
3.2. Fire Prevention and Protection.
The Government will provide fire prevention and protection, inspection and maintenance of Government furnished fire extinguishers and systems. The Fire Department telephone extension is 911 for emergencies when calling from a base phone and (661) 277-4540 when calling from a cell phone.
3.3. Security Forces.
The Government will provide general on base Security Forces services. Edwards AFB Security Forces telephone extension is 911 for emergencies and (661) 277-3340 for routine calls.
SECTION IV: GENERAL INFORMATION
4.0. General Information.
4.1. Hours of Operation.
The Contractor shall perform specialized pest control services during normal duty hours which are 0700-1700, Mondays through Fridays, excluding Federal Holidays. Specialized pest control applications to the Golf Course are to be scheduled as not to interfere with course play whenever possible.
4.2. Contract Manager.
Contractor shall provide a Contract Manager responsible for the performance of work under this contract and shall provide a contact phone number for emergencies. The name of this individual and alternate shall be designated in writing to the CO and COR within 10 calendar days of award of the contract. The Contract Manager or alternate must be able to read, write, speak, and understand the English language, both spoken and written. The Contract Manager shall ensure employees have current personal identification and valid certifications required for the performance of work to be accomplished.
4.3. Special Qualifications.
The Contractor shall have a State of California certified and licensed pest control manager specifically assigned to support this contract. All Contractor employees shall comply with California Code of Regulations Title 3, Division 6. All Contractor employees that apply pesticides shall have a California pesticide application certification. On DOD land and property, direct supervision is not permitted for Contractor’s applicators. The Contractor shall be responsible to ensure that the licenses, certificates and training of Contractor personnel are kept current. Contractor shall provide a copy of each applicator’s State of California certification that will be applying chemicals on Edwards AFB to the IPMC at contract award. An updated copy will be supplied each time there is a change to the license/certification. The Contractor and employees shall have completed required hazard communication training. The applicators shall have in their possession and utilize Personnel Protective Equipment as required by the Safety Data Sheet (SDS) and Label.
4.4. Contractor’s Personnel Appearance.
Contractor’s personnel shall present a neat appearance and be easily recognized. This may be accomplished by wearing distinctive clothing bearing the name of the company or wearing badges that contain the company name, employee name and photo.
4.5. Contractor’s Vehicles.
Contractor’s vehicles must clearly display the name of the company on them. All Contractor vehicles must be maintained and in working order IAW all EPA guidelines and used IAW all manufacturer’s vehicle safety recommendations. All vehicles must be licensed and insured.
4.6. Security Requirements.
All Contractor employees will be subject to a criminal history background check IAW AFMAN 31-113, Installation Perimeter Access Control, and 412th Test Wing Integrated Defense Plan (IDP) 31, Volume I (the AFMAN and IDP are both limited access documents but more details on the requirements/process in terms of background checks can be provided by the Contracting Office upon request). Vehicle registration, proof of insurance, and a valid driver’s license must be presented for vehicles. Installation Commanders may deny access and credentials based on information obtained during identity vetting that indicates the individual may present a threat to the good order, discipline, and morale of the installation.
Within five (5) days after contract award, the Contractor Base Access for Contractor personnel who wish to obtain long-term Base access via an Air Force Materiel Command (AFMC) Form 496, Application for AFMC Identification Card, IAW Air Force Federal Acquisition Regulations (AFFARS) Clause 5352.242-9000, Contractor Access to Air Force Installations, the Prime Contractor shall submit a written request on company letterhead to the CO with the following information (prior to submitting a request for Base access via an AFMC Form 496): 1) contract number, 2) location of work, 3) expiration date (must be on or before contract’s expiration date), and 4) names of employees/ subcontractor employees needing access to the base, to include each employee’s company name.
The Contractor shall meet with the COR at least 30 days prior to Base pass expiration in order to insure ample time for the Government to process appropriate documents.
It is recommended that the Prime Contractor provide a copy of the request letter to each employee for verification purposes when the employee comes to Building 3500 to get their AFMC Form 496 signed from the 412 CEG Security Manager. Employees for which no request letter can be found or provided will result in the employee being asked to return on another date/time so verification can be made. Comment by PROTHRO, THOMAS A GS-12 USAF AFMC AFTC/PZIOB: It’s my understanding that Contracting is moving away from this process and does not want to sign these any more. Contracting would like the mission partner to accomplish this step.
Base passes will be limited to the length of the contract not to exceed 3 years from the date the AFMC Form 496 is submitted to the Contracting Office for signature.
The AFMC Form 496 will be reviewed by the COR and approved and signed by the 412 CEG Security Manager, then taken by the Contractor employee to the Edwards AFB Pass & Registration Office in Building 3000 for the security portion and issuance of the Base pass. The Contractor shall comply with all base security regulations. Additionally, the Contractor shall comply with actions dictated by force protection condition changes/measures. For real-world situations/exercises, changes to force protection conditions could impact Contractor performance of work and can include denial of access to the installation to perform work
Only employees who will be working on the contract consistently/frequently should be submitted for a long term Base pass.
The Contractor shall comply with all security requirements imposed by the Installation. Specific security considerations will be handled on an individual basis as required.
The contractor shall report suspicious activity to Security Forces at 661-277-3340 in a timely manner.
The Contractor and its employees shall comply with all base traffic regulations.
No States are subject to Real ID Act enforcement at this time. The Department of Defense will accept standard State-issued driver's licenses and identification cards from all U.S. States and territories as proof of identity. Please visit the Department of Homeland Security's (DHS) website to get the latest information, https://www.dhs.gov/current-status-states-territories. Once Real ID Act is enforced, the Contractor shall comply fully.
Contractor personnel who require routine physical access to the installation shall comply with Level I - Antiterrorism Awareness training requirements. Routine physical access applies to Contractor personnel whose contract, performance work statement/statement of work has a 45 day or more period of performance. AT Level I training provides contractor employees with the requisite knowledge necessary to remain vigilant for possible terrorist threats. AT Level I training can be taken at https://jkodirect.jten.mil/Atlas2/page/login/Login.jsf for non-Common Access Card (CAC) holders. CAC holders may complete AT Level I training (Force Protection course) via the Advanced Distributed Learning Service at https://golearn.adls.af.mil/login.aspx. Tracking for each contractor or subcontractor employee is the responsibility of the COR or unit Antiterrorism Representative. The Contractor shall be responsible for safeguarding all Government property, equipment and materials provided for Contractor use. At the end of each work period, all Government facilities, equipment, and materials shall be secured.
During performance of the contract, the Contractor shall be responsible for obtaining required identification for newly assigned personnel.
The Contractor shall be responsible for prompt return (within three (3) calendar days) of credentials and vehicle passes for: 1) any employee who no longer requires access to the work site; 2) upon request by a Contracting Officer (CO); and 3) upon completion or termination of the contract. The Prime Contractor shall ensure that all Base identification passes issued to employees and subcontractor employees are returned to the issuing office. Failure to comply with these requirements may result in withholding of payment.
Contractor employees are prohibited from possessing weapons, firearms or ammunition, on themselves or within Contractor-owned or privately owned vehicle while on Edwards AFB.
The Contractor shall request base access through the COR for each employee via phone call, three (3) business days prior to (1) Site Visit (2) Performance of Work.
4.7. Environmental Controls.
The Contractor shall ensure all hazardous materials are authorized and managed in compliance with applicable sections of AFI 32-7086, Hazardous Materials Management (04 Feb 15) and Edwards AFB Hazardous Materials (HM) guidance to ensure compliance with Emergency Planning and Community Right-to-Know Act of 1986 (42 U.S.C. 11001–11050), and the Pollution Prevention Act of 1990 (42 U.S.C. 13101–13109). The Contractor shall complete and provide the Contractor Hazardous Materials Usage Tracking Form (Appendix F) to the COR at least 30 days prior to the start of the project for all HM brought onto Edwards AFB (i.e., any chemicals/materials procured for this project by any means other than an installation HAZMART and not previously authorized and tracked in the Enterprise Environmental Safety Occupational Health Management Information System [EESOH-MIS]). The Contractor shall ensure that solid and Hazardous Waste (HW) management on this contract complies with all applicable sections of AFI 32-7042, Waste Management (07 Nov 14) the Edwards AFB Hazardous Waste Management Plan (HWMP) and the Edwards AFB Integrated Solid Waste Management Plan. Hazardous/regulated waste shall be disposed of under the Edwards AFB United States Environmental Protection Agency (USEPA) Identification (ID) number, with Edwards AFB as the Generator unless contractor is considered other “persons” under the Resource Conservation and Recovery Act and operate their HW activities independent of the AF installation using their own USEPA ID number. Signed hazardous waste disposal manifests shall be required for hazardous waste that may be generated on this project prior to transportation for off-site disposal to a California Department of Toxic Substance Control and/or an USEPA-approved landfill or treatment facility. The Contractor shall submit draft manifests via e-mail at 412CEG.CEVC.HWWorkflow@us.af.mil for review. After the draft manifests, the waste shipment must be scheduled with 412 CEG/CEVC to arrange for manifest signature by 412 CEG/CEVC or a properly trained person with Delegation of Signature Authority. 4.8. Safety Requirements and Reports.
The contractor shall comply with all applicable parts of Title 29, Code of Federal Regulation (CFR), OSHA, Part 1910; Title 29, CFR, Safety and Health Standards for Federal Service Contracts, Part 1925; Title 40, CFR, Parts 150-189, and Title 49, CFR, Hazardous Materials Regulations, Part 171, while on an Air Force installation, to ensure safe working conditions for contract personnel and a safe environment for the occupants of Air Force facilities.
4.8.1. General Safety and Health Responsibilities and Accountability. The contractor shall comply with all federal, state and local laws, and Air Force Instructions for the protection of their employees while working on an Air Force Installation or property. The protection of contractor employees is exclusively the obligation of the contractor. The Contractor shall furnish personal safety equipment for its employees in performance of this contract. Safety equipment includes shoes or boots, eyewear, respirators, protective chemical aprons, rubber chemical gloves and regular work gloves. Contractor shall manage all work areas to ensure the safety of building occupants, base personnel, or visitors in or near the areas. Contractor must ensure that personnel are properly trained on the equipment they are tasked to operate. The contractor shall develop site specific mishap reporting procedures which result in damage to government property or injury to Air Force personnel. These procedures are to be included in the safety and health plan for review. Hazardous or noncompliant conditions identified by the CO shall be promptly corrected, and corrective action(s) taken shall be reported in writing to the CO within five (5) days of the CO’s notification to the contractor. In cases of serious and/or recurring hazards/noncompliance, the contracting officer may stop the work in question pending correction of the hazardous/noncompliant condition, and/or order the removal of the involved Contractor/subcontractor employees from the government installation/ facility. In such cases, the Contractor will not be granted a cost adjustment nor an extension of time due to the work stoppage or employee removal action. Failure of the Contractor to effectively resolve serious and/or recurring hazards or noncompliance may result in termination of the contract.
4.8.2. Safety and Health Plan. IAW AFI 91-202, AFMC Supplement, Safety Program, the contractor shall submit a Safety and Health Plan to Contracting for review and acceptance within 15 business days of contract award. The Contractor’s plan shall include appropriate measures to ensure the Contractor reacts promptly to investigate, correct and track alleged safety and health violations and/or uncontrolled hazards in Contractor work areas. Additional, installation specific references and policies may be included or attached to this section.
The plan shall include:
a. Demonstrate a management commitment to employee safety and health;
b. Identify the application of the safety and health plan to sub-Contractors;
c. Identify the roles and responsibilities of the following individuals: management; supervisors; employees; safety coordinator
d. Identify applicable safety rules and regulations’
e. Include a worksite hazard analysis to include base-line hazard identification and required control measures;
f. Include a job site analysis to include hazards of tasks required to control measures
g. Identify employee safety and health training requirements and the documentation process;
h. Include a workplace inspection frequency, to include identifying the individual conducting the inspections;
i. Include employee hazard reporting procedures;
j. Identify individual(s) responsible for corrective action hazards;
k. Identify first aid/injury procedures;
l. Identify procedures for accident investigation and reporting;
m. Identify emergency response procedures; and
n. Identify the process for tracking controlled hazards in Contractor work areas.
4.8.3. Accident or Mishap. The Contractor shall record and report to the CO and COR within one hour of occurrence all available facts relating to each instance of injury to Contractor or Government personnel or damage to Government property. In the event of an accident or mishap, the Contractor shall take prudent and reasonable action to establish control of the accident or mishap scene until relieved by the CO or COR. If the Government elects to conduct an investigation of the accident or mishap, the Contractor shall cooperate fully and assist Government personnel in the investigation until such is complete.
4.9. Contractor Full-Time Equivalent Reporting.
This requirement incorporates Air Force Implementation of FY11 NDAA Section 8108 of Public Law 112-10 of the Department of Defense and full-year Continuing Appropriations Act, 2011. As such, the Contractor shall adhere to the follow:
4.9.1. The Contractor shall report ALL Contractor labor hours (including sub-Contractor labor hours) required for performance of services provided under this contract for the Air Force Test Center via a secure data collection site. The Contractor is required to completely fill in all required data fields at http://www.ecmra.mil.
4.9.2. Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October* of each calendar year. Contractors may direct questions to the CMRA help desk.
4.9.3. Reporting Period: Contractors are required to input data by 31 October of each year.
4.9.4. Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and Contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the Contractor name and contract number associated with the data.
4.9.5. User Manuals: Data for Air Force service requirements must be input at the Air Force CMRA link. However, user manuals for Government personnel and Contractors are available at the Army CMRA link at http://www.ecmra.mil.
