PERFORMANCE WORK STATEMENT

FOR

412th LRS LINEN CLEANING CONTRACT

SECTION I

1.0 DESCRIPTION OF SERVICES/GENERAL

1.1. SCOPE OF WORK: The contractor shall furnish all management, labor, uniforms/coveralls, rags, facilities, materials, equipment and transportation necessary to perform the following service to the 412LRS/Vehicle Maintenance at Edwards Air Force Base, CA. The Contractor is responsible for the cleaning or laundering of uniforms and rags and repairing of them when damage results from “normal wear and tear”. Contractor shall inspect all garments for minor tears, seam flaws, missing buttons, etc., prior to uniforms being returned. (Note: Normal wear and tear is defined as “the physical deterioration which occurs in the normal use for which the garment is intended, without negligence, carelessness, accident or abuse of the garment by the employee). All items shall be picked up and delivered in accordance with Technical Exhibit I. The contractor will pick up, load, count, process and unload linen and uniforms and return all items tendered from cleaning in accordance with Technical Exhibit I. The service to be performed shall be non-personal services and are not inherently governmental functions.

1.2. PERSONNEL

1.2.1 Contract Manager: The contractor shall ensure a full-time contract manager or designated representative is available to ensure performance of the work is conducted in accordance with this statement of work. The contractor shall ensure that the contract manager or a representative, other than the driver, will visit vehicle maintenance on a quarterly basis. The name of the contractor or representative shall be designated in writing and provided to the contracting officer.

1.2.1.1. Contractor Representative: The contractor will be available by cell phone, phone, e-mail, and fax for routine matters in lieu of the contractor representative being available during normal duty hours to meet on the installation. However, when necessary the contractor or his designated representative will be available to meet on the installation within 72 hours.

1.3. Pick-up and Delivery:

1.3.1 Schedule: The contractor shall conduct pickup and delivery Wednesday mornings after 7:00am and NLT 3:00 pm local time. Any deviation to this day, due to holiday, weather, etc. shall be coordinated and agreed upon with the Program Manager or Government representative with at least 24 hours advanced notice prior to delivery of uniforms.

1.3.2. Locations: The contractor shall conduct pick-up and delivery at Vehicle Maintenance located at 115 N. Rosamond Blvd, Building 3510, at Edwards AFB, CA. Clean uniforms, will be dropped off in the locker room and shop rags will be dropped off in the clean rag red bins. Used rags will be picked up from dirty rag red bins. Dirty uniforms will be picked up from the locker room.

1.3.3 Accountability and Inspection: The contractor shall conduct 100% count of shop towels and uniforms with Vehicle Management representative during pick-up and delivery to ensure accurate accountability. Inspection for stains, discoloration and torn or ripped uniforms/rags shall be conducted by contractor and Vehicle Management representative before pick-up; accurate counts shall be conducted upon return of linens the following scheduled delivery day.

1.3.4 Holiday Procedures: During official scheduled holidays, services required by this contract shall be performed on the immediate following workday and continue the regular schedule thereafter.

1.4. Damaged Items: All laundered items returned from the laundry plant damaged, to include but not limited to items that are stained, discolored, torn or ripped, will be the responsibility of the contractor to replace at their own cost. Notification will be giving to Material Control personnel or the Vehicle Management Flight Chief/Superintendent prior to replacement.

1.5. Contractor Quality Control Plan: Contractor shall submit a quality control plan (QCP) to the Program Manager (PM) and CO within 10 calendar days of award and changes to the plan. The QCP shall ensure that all linen and uniform services are performed in accordance with applicable commercial standards, regulations, laws and codes. The QCP must describe the overall quality approach and quality control inspection system tasks to identify and prevent defective services from occurring and recurring. As a minimum, the contractor shall develop quality control procedures that address the areas identified in the Services Summary and an internal inspection/surveillance system. The contractor shall maintain all quality control records for the duration of the contract and will make them available to the government PM and CO within 24 hours upon request.

1.6. Quality Assurance: The government will periodically evaluate the contractor’s performance by appointing a representative(s) to monitor performance to ensure services are received. The government may increase or decrease the number of quality assurance inspections as performance dictates. The PM/representative will evaluate the contractor’s performance through intermittent on-site inspections. Any discrepancies will be investigated by the PM or representative and a final determination will be made of the validity of the discrepancies.

1.7 Uniform Inventory Listing: The Contractor shall establish a uniform inventory listing by individual employee along with type, size and pieces issued. Contractor shall adjust uniform allocations based on employee turnover by removing uniforms when an employee leaves and outfitting new employees as they are hired. The outfitting of any new employees must be coordniated through the Program Manager or Government representative.The government will provide the contractor with the uniform size, employee name, and quantity prior to the work start date of the new employee. The contractor will provide the requested uniforms with proper sizes, quantities, and name tags for new employees within 10 duty days of notification.

1.8 Change in Employees: The number of employees covered by this contract may increase or decrease at any time (i.e., new / temporary hires, dismissals, retirements, military members receiving Permanent Change of Station, etc.). If employees are added to or deleted from the contract, the unit cost per employee shall remain the same. Accordingly, weekly costs shall be adjusted based on the number of uniforms provided at the unit cost per employee indicated in the Price Schedule. Except as this contract may otherwise provide, if the Government's requirements result in changes to the estimated quantities stated in the contract, that fact shall not constitute the basis for an equitable price adjustment.

SECTION II

2.0 SERVICES SUMMARY: The contractor service requirements are summarized into performance objectives that relate to mission essential items. The performance thresholds briefly describe the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success.

2. The Contractor will provide new basic uniforms in the following color: Short Sleeve Shirts/ Long Sleeve Shirts – Navy Blue Pants – Navy Blue

2.1 Items being replaced will fall in line with 1.4 for damaged and worn items. Items being replaced will be new or like new.

2.2 Shop Rags will be of cotton material and any color.

2.3 Shop rags will be serviceable, residue free and any color.

VEHICLE MAINTENANCE LINEN CLEANING CONTRACT

1. The Contractor shall pick up all dirty items every Wednesday, clean all the dirty items and shall bring back those dirty items cleaned the following week (For example, if the Contractor picks up 100 dirty items, the Contractor will clean those 100 dirty items and return those 100 items cleaned the following week, etc.). Every item that is picked up dirty must be cleaned and returned the following week.
