DEPARTMENT OF THE AIR FORCE

BUSINESS AND ENTERPRISE SYSTEMS DIRECTORATE

MAXWELL AIR FORCE BASE GUNTER ANNEX ALABAMA

5 October 2012

MEMORANDUM FOR ALL INTERESTED PARTIES

FROM: AFLCMC/HIK

501 East Moore Drive, Bldg 884, Room 1400

Maxwell AFB-Gunter Annex, AL 36114

SUBJECT: Past Performance Questionnaire – NetOps and Infrastructure Solutions (Small Business), RFP FA8771-09-R-0019

1. AF Program Executive Office for Enterprise Information Systems of the Air Force Materiel Command (AFMC) is in the process of selecting contractors for Network Centric Solutions-2 (NETCENTS-2) NetOps and Infrastructure Solutions. The goal of this NETCENTS-2 acquisition is to provide a full range of innovative, reasonably priced, world-class NetOps solutions to support the full spectrum of netcentric operations and missions.

2. One of the considerations in proposal evaluation is the verification of the Offerors' past performance on contracts, which reflect the offeror's ability to perform on the proposed effort. We depend on information received from companies and Government agencies such as yours, which have had firsthand experience with an offeror, for the evaluation of the offeror's performance on those contracts.

3. Our areas of interest in the offeror are summarized in the enclosed questionnaire. Our schedule is extremely tight and we would appreciate your written response no later than 12:00 PM (CDT) 31 Oct 2012. This schedule will allow us sufficient time to analyze the data before the start of evaluations.

4. DO NOT RETURN QUESTIONNAIRES TO THE OFFEROR/CONTRACTOR. Although this questionnaire was distributed to you by the offeror/contractor, it is important you return the completed questionnaire directly to the Government. To assist you in preparing your response and expediting your reply, you may complete the questionnaire and email it to NETCENTS2PCAG@gunter.af.mil. If you prefer, complete the questionnaire by hand and "fax" to Commercial phone number 334-416-2478 or DSN 596-2478 (Attention: PCAG). Please call the Contracting Officer, Randi Williams, at 334-416-4555 if you have any questions. Your completed questionnaire will become a part of the official Source Selection records and needs to be handled accordingly.

5. Your help is greatly appreciated and your prompt response will be one of the keys to the successful and timely completion of this Source Selection.
