PERFORMANCE WORK STATEMENT
FOR
COMPLIANCE TESTING, EVALUATION, AND
MINOR REPAIR OF STORAGE TANK SYSTEMS
AT
WRIGHT-PATTERSON AIR FORCE BASE, OHIO
17 December 2018
1.0 INTRODUCTION
Wright-Patterson AFB has an inventory of underground and aboveground storage tanks. The majority of these tanks store petroleum products and are equipped with tank gauging and release detection systems. In order to maintain compliance with Federal, State, and local tank regulations, it is necessary to periodically conduct integrity testing, containment testing, evaluation of tank gauging and release detection systems, compliance evaluations, and minor repair and adjustment of specific tank systems. The Contractor shall provide all personnel, equipment, tools, materials, supervision, and other items necessary to perform tasks under this performance work statement. Under the terms of this agreement, all work performed shall be accomplished in accordance with all applicable Federal, State, and local rules and regulations and industry standards.
2.0 LOCATION
Wright-Patterson Air Force Base (WPAFB) is located in southwestern Ohio, between the cities of Dayton and Fairborn. It is situated approximately 60 miles north of Cincinnati, Ohio and 50 miles west of Columbus, Ohio and occupies portions of Greene and Montgomery counties. The base is comprised of two areas, known as Areas A and B. Storage tanks containing petroleum products are located throughout the installation in both areas.
3.0 OBJECTIVES
The objective of this work is to conduct integrity testing, compliance evaluations, containment testing, evaluation of specific tank gauging and release detection systems, and minor repair and adjustment of specific tank systems as required by the Technical Representative and in accordance with all applicable tank regulations and accepted industry standards. A list of underground storage tanks which may require periodic integrity testing and containment testing in accordance with Ohio Administrative Code (OAC) 1301:7-9 requirements is attached. In addition, integrity testing, containment testing, and operational compliance inspection and documentation of other specific tank systems may be necessary periodically depending on condition of tanks, tank monitoring systems, and suspected problems or leaks. All operational compliance inspections on underground storage tanks will be conducted and documented in accordance with OAC 1301:7-9 and State of Ohio Bureau of Underground Storage Tank Regulations (BUSTR) requirements. All work on aboveground storage tanks will be documented on accepted industry standard forms or equivalent.
The need for emergency integrity testing of underground storage tanks suspected of leaking may occur infrequently and may require expedited services of the Contractor to perform the required testing.
4.0 GENERAL CONTRACTOR REQUIREMENTS
The Contractor shall furnish all labor, materials, equipment, and supplies necessary to complete the requirements of this PWS within the schedule specified below. All costs incurred in completion of this PWS will be borne by the Contractor.
4.1. GOVERNMENT PROPERTY. All documents, photographs, maps, graphics, mailing lists, computer files and the like developed by the Contractor in completing the requirements of this PWS are Government property and will be delivered to the 88 ABW/CEIEA point of contact (POC) upon completion of the task. All contractors taking photographs must be escorted and use a digital camera. The pictures must be reviewed and approved by the WPAFB CE OPSEC Manager before the Contractor leaves the base.
4.2. DATA SECURITY. The Contractor shall not release any data, reports, or materials collected or developed while completing this project without the expressed written consent of the 88 ABW/CEIEA POC.
4.3. DELIVERABLES. Documentation of work performed under this scope will follow OAC requirements for USTs and industry standards or equivalent for other tank work and will be approved by the 88 ABW/CEIEA Technical Representative prior to use.
Documentation of work performed will be submitted to the 88 ABW/CEIEA Technical Representative under separate cover letter containing the Contractor’s company name, address, and contact information. All cover letters will contain a description of the work performed, the date and location where the work was performed, and names of personnel involved. The results of work performed will be attached to the cover letter.
4.4. CONTRACTOR QUALIFICATIONS. All work under this scope must be performed by an experienced individual(s) with demonstrated knowledge of tank systems and holding a current certification from BUSTR as an Underground Storage Tank (UST) installer or supervised by an individual with current certification from BUSTR as a UST installer.
Evaluation of Veeder-Root and other Automatic Tank Gauging (ATG) and release detection systems shall be performed by an individual knowledgeable of these systems.
4.5. GENERAL PROVISIONS. All reports, maps, photographs, notes and other work developed in the performance of this contract shall be and remain the sole properties of the Government and may be used by the Government on any other work without additional compensation to the contractor. The contractor agrees not to assert any rights and not to establish any claim with respect thereto. The contractor agrees to furnish all retained materials on the request of the contracting officer. The contractor shall be responsible for the accuracy and validity of the material presented in their reports.
4.6. SCHEDULING. The Contractor shall schedule all work to be performed in advance with the Technical Representative to ensure regulatory compliance dates are met. The Contractor shall respond to emergency work within 24 hours of notification. The Technical Representative(s) will notify the contractor in advance of non-emergency work to be accomplished. The Contractor will obtain approval from the Technical Representative for all schedules of work to be performed.
5.0 GENERAL INSTRUCTIONS TO THE CONTRACTOR. The following conditions shall be observed by the Contractor:
5.1. Any work performed in/around the War Fighter Training Center must be coordinated with the organization that manages the site. The CEIEA POC will coordinate all activities on behalf of the contractor prior to mobilization.
5.2. The contractor must familiarize himself with the hours of gates that impact access to each work area, and coordinate access to all work areas through the 88 ABW/CEIEA
POC.
5.3. The contractor must abide by applicable state traffic laws.
5.4. Before starting any work whereby traffic would be impeded or hindered, especially fire trucks and police vehicles, notify the Base Fire Department at 257-6933 and Security Forces at 257-6959 or 257-6516 at least 72 hours prior to placing any barricades. The site work hours are Monday through Friday, daylight hours, however notify the program manager if planning to work past 5:00 p.m. Work may be accomplished on Saturdays, Sundays or government holidays with written concurrence from the program manager or contract administrator.
5.5. The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Air Force via a secure data collection site. The contractor is required to completely fill in all required data fields at http://www.ecmra.mil.
Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October* of each calendar year. Contractors may direct questions to the CMRA help desk.
*Reporting Period: Contractors are required to input data by 31 October of each year.
However, due to the delayed implementation, input of FY12 data is required by 31 Mar 13.
Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.
User Manuals: Data for Air Force service requirements must be input at the Air Force CMRA link. However, user manuals for government personnel and contractors are available at the Army CMRA link at http://www.ecmra.mil.

Voluntary Protection Program PWS Addendum
APPLICABILITY: This section of the PWS applies to “Applicable Contractors” as defined below.
DEFINITIONS:
Applicable Contractor. A contractor whose employees worked at least 1,000 hours at the site in any calendar quarter within the last 12 months and is NOT directly supervised by the applicant
(installation).
Days Away, Restricted, and or Transfer Case Incident Rate (DART). The number of recordable injuries and illness cases per 100 full-time employees resulting in days away from work, restricted work activity, and/or job transfer that a site has experienced in a given time frame.
Total Case Incidence Rate (TCIR) - Total number of recordable injuries and illness cases per
100 full-time employees that a site has experienced in a given time frame.
Voluntary Protection Program - The Voluntary Protection Program (VPP) promotes effective worksite-based safety and health. In the VPP, management, labor, and OSHA establish cooperative relationships at workplaces that have implemented a comprehensive safety and health management system. Approval into VPP is OSHA’s official recognition of the outstanding efforts of employers and employees who have achieved exemplary occupational safety and health.
REQUIREMENTS:
Wright-Patterson AFB is in the process of pursuing VPP recognition or has already been recognized under the OSHA Voluntary Protection Program (VPP). VPP impacts all “applicable contractors” operating on Air Force Installations. The contractor shall be responsible for ensuring their employees and managers have a comprehensive understanding of VPP as well as full compliance with OSHA requirements. The contractor shall follow the safety and health rules of the installation or VPP site. Detailed information on VPP is available on the OSHA website at http://www/osha.gov/dcsp/vpp/index.html.
The contractor shall submit a Safety and Health Plan and corresponding site safety checklist to the Contracting Officer not later than 10 calendar days after contract award. The contractor’s plan shall include appropriate measures to ensure the contractor reacts promptly to investigate, correct, and track alleged safety and health violations and/or uncontrolled hazards in contractor work areas. The plan shall:
Demonstrate a management commitment to employee safety and health;
Identify the application of the safety and health plan to subcontractors;
Identify the roles and responsibilities of the following individuals with respect to safety and VPP: Management, Supervisors, Employees, and Safety Coordinator;

Identify applicable safety rules and regulations;
Include a worksite hazard analysis to include base-line hazard identification and required control measures;
Include a job site analysis to include hazards of tasks required to control measures;
Identify employee safety and health training requirements and the documentation process;
Include a workplace inspection frequency, to include identifying the individual conducting the inspections;
Include employee hazard reporting procedures;
Identify individual(s) responsible for corrective action of hazards;
Identify first aid/injury procedures;
Identify procedures for accident investigation and reporting;
Identify emergency response procedures; and
Identify the process for tracking controlled hazards in contractor work areas.
The contractor shall submit their Total Case Incidence Rate (TCIR) and Days Away, Restricted, and or Transfer Case Incident Rate (DART) rates and OSHA Form 300A annually to the contracting office for consolidation and submission as part of the installation’s annual VPP
Safety and Health Management report. TCIR and DART rates are due by the 15th of January of each year.
The contractor’s Quality Control Plan shall identify the processes and procedures the contractor will use to track compliance with the Safety and Health Plan, and the process and procedures that will be used to correct violations.
The contractor shall establish these VPP requirements for all subcontractors who qualify as applicable contractors under the resulting contract.
CONTRACTOR MANPOWER REPORTING REQUIREMENTS
PWS ADDENDUM
The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Air Force via a secure data collection site. The contractor is required to completely fill in all required data fields at http://www.ecmra.mil.
Reporting inputs will be for the labor executed during the period of performance for each
Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October* of each calendar year. Contractors may direct questions to the CMRA help desk.
*Reporting Period: Contractors are required to input data by 31 October of each year.
However, due to the delayed implementation, input of FY12 data is required by 31 Mar 13.
Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.
User Manuals: Data for Air Force service requirements must be input at the Air Force
CMRA link. However, user manuals for government personnel and contractors are available at the Army CMRA link at http://www.ecmra.mil.
