SECTION 1
Contractor Services Overview
1.1. Description of Services
The Contractor shall perform Aircraft Wash, Corrosion Control, for the 445th Maintenance Group (445 MXG) at Wright-Patterson Air Force Base (AFB), Ohio, as defined in all sections of this Performance Work Statement (PWS). The Contractor shall provide all personnel, equipment, tools, materials, supervision, and other items necessary to perform contracted services, unless specified as provided by the Government.
Contractor tasks include:
• Aircraft Wash Services Management Functions
• Aircraft Wash Services for the C-17
• Emergency Services and Contingency Services
• Environmental Compliance
• Facility Cleanliness
1.2. Regulations and Guidance
Federal, state, and local directives prescribe specific procedures for aircraft corrosion prevention and cleaning services. The guidance listed below, includes but is not limited to, Federal Aviation Administration (FAA) guidance, Occupational Safety and Health Administration (OSHA) guidance, Federal Acquisition Regulations (FARs) and Technical Orders (TOs). The Government will provide the Contractor with all TOs and local policies. General tasks are to be performed in accordance with general TOs and applicable Air Force Instructions, Command Instructions, 88th Air Base Wing and 445th Airlift Wing Instructions, and the 445th Maintenance Group’s Maintenance Operating Instructions (MOIs). Frequently used TOs and instructions are available. All other TOs and instructions for the performance of this PWS are available in the 445th Maintenance Group Quality Assurance Office.

1.3 Aircraft Wash Services Management Functions: The Contractor shall establish Aircraft Wash
Services Management Functions to perform the following:
1.3.1. Ensure an effective Foreign Object Debris (FOD) control and prevention program is written and in place. This program includes performing FOD inspections on all aircraft wash facilities, employee work areas, and Contractor vehicles.
1.3.2. Respond in writing to deficiencies identified on the Corrective Action Report within 24 hours. The response shall identify the corrective action taken and what the Contractor will do to prevent recurrence of the deficiency.
1.3.3 Contractor responsible for washing large commercial or military cargo aircraft (e.g., C-5, C-17, C-
130, KC-135, A320, A330, A340, B737, B777, B787).
1.3.4. Contractor Employees:
1.3.4.1. Contractor employees shall strictly comply with the Contractor’s quality control plan and all applicable regulations and guidance discussed in Paragraph 1.2 of this PWS.
1.3.4.2. Aircraft wash management personnel shall be available twenty-four hours a day, seven days a week by phone or electronic media for coordination with the 445th Maintenance Group’s Maintenance Operations Center (MOC), Plans, Scheduling and Documentation (PS&D), or the COR to perform aircraft wash services outlined in this PWS.
1.3.5. Provide all motor vehicles needed to accomplish the requirements of this PWS. All Contractor supplied motor vehicles shall be identified with the Contractor’s company name. The contractor will ensure motor vehicles are operational at all times.
1.3.6. Provide all high reach equipment needed to accomplish the requirements of this PWS.
1.3.7. Notify the COR within one hour of any damaged or malfunctioning Contractor’s motor vehicles/equipment used to provide services if such motor vehicles/equipment will cause a delay in completion of services. Repair or replacement, to include rental of motor vehicles/equipment must be made within 24 hours, in order to preclude a lapse in service.
1.3.8. Notify the MOC and COR within one hour of any mishap that results in any damage to Government assets or any mishap that results in injury to Contractor personnel.
1.3.8.1 Provide written response for mishap, customer, or Contractor complaint is provided to COR NLT 24 hours of incident occurrence.
1.3.9. Ensure the potential abrupt absence of Contractor-hired off-duty active military and/or reserve/national guard members does not adversely affect the Contractor’s ability to accomplish the requirements of this PWS. Their absence does not constitute an excusal for nonperformance under this contract.
1.4 Aircraft Wash Services for the C-17
This section defines the types of services and specifications required and the maximum allowable time for service. The Contractor shall not exceed the maximum allowable time indicated for a service. All services will be accomplished in accordance with regulations and guidance discussed in Paragraph 1.2 of this PWS.
All aircraft wash services will be accomplished inside the designated 445 MXG wash facility.
NOTE 1: Government personnel will deliver aircraft and maintenance stands, prepare wash equipment, and configure aircraft. Configuration of aircraft is defined as: flaps lowered to down position, nose and main landing gear doors positioned for wash, cargo door and ramp positioned for wash, drain plugs removed from receptacles prior to wash, and battery disconnected. Government personnel will install protective covers to include water barrier materials, masking materials, pressure sensitive tape, and wheel coverings prior to wash.
Prior to aircraft delivery to the Contractor, the Government will insert the applicable pre-printed wash and lube package into the aircraft AFTO Form 781 series forms binder. Wash service will NOT begin until AFTO Form 781A packages have been properly entered. After the wash is complete, the Government personnel will clear all applicable wash preparation and wash due discrepancies in the aircraft AFTO Form 781 series forms IAW applicable technical guidance.
NOTE - 2: If any maintenance discrepancies are discovered that prevent completion of service, the contractor shall notify the COR within 1 hour of occurrence.
NOTE - 3: After completion of Wash Service in the designated wash facility, the entire facility floor shall be free of all dirt, grime and grease. All free standing water shall also be removed.
NOTE - 4: The start time (i.e., calendar date and clock time) for all wash services will be a start time agreed upon by the COR and the Contractor. The start time will be used to determine if the Contractor completed the services described below within the maximum allowable time limits.
NOTE 5: In the event of adverse conditions (weather etc…) resulting in a delay of the wash process, the maximum allowable time shall be coordinated with the COR and extended to cover the delay.
1.4.1. COMPLETE AIRCRAFT WASH: Maximum Allowable Time – 29.0 Hours This service will include items 1.4.2., 1.4.3., and 1.4.4.
1.4.2. COMPLETE EXTERIOR WASH: Maximum Allowable Time – 20.0 Hours This service applies to the entire exterior of the aircraft and includes, but is not limited to, engines, engine pylons and thrust reversers, flap wells, spoilers, wheel wells, landing gear doors, main landing gear pods, T-tail, and service compartments accessible by quick-disconnect fasteners on the outside of the aircraft.
This service applies to all landing gear assemblies, wheels, tires, brakes, and any components within the specified work areas, not specifically identified herein.
NOTE 1: Wheels, brakes, axles, and landing gear strut components shall not be cleaned using high pressure cleaning equipment.
NOTE 2: Water directed at close range may remove lubricants from joints. Do not direct water at openings normally masked during regular washes. Avoid direct spray into openings and surfaces immediately adjacent to openings. Keep water nozzle at least 10 feet away from the surface being rinsed. Position the water stream at a direct contact angle of 45 degrees or less to minimize the impact pressure.
NOTE 3: Where Carbon buildup occurs common to the exhaust paths and/or adjacent to the engine exhaust paths, enhanced cleaning methods will be performed utilizing Melamine Sponges (AMMS 5101) to meet consistent levels of cleanliness similar to the remaining surface(s) appearance of the aircraft.
1.4.3. COMPLETE INTERIOR CLEANING OF CARGO COMPARTMENT: Maximum Allowable Time – 3.0 Hours
The complete interior cleaning of the cargo compartment shall include, but is not limited to, the cargo floor, D-ring cups, rails, catwalks, and aft ramps. This cleaning includes vacuuming, mopping, and scrubbing the cargo floor, D-ring cups, and rails as required. Care will be taken to prevent moving or damaging aircraft controls, switches, instruments, and other installed equipment. Do not permit cleaning solution to drip below the intended cleaning surfaces. Do not use high-pressure water for rinsing interior surfaces.
1.4.4. LANDING GEAR WASH: Maximum Allowable Time – 6.0 Hours
This service consists of cleaning the nose and main landing gear assemblies, to include the wheels, brakes, struts, interior of inboard and outboard doors, and wheel well area.
NOTE 1: Wheels, brakes, axles, and landing gear strut components shall not be cleaned using high pressure cleaning equipment.
NOTE 2: Water directed at close range may remove lubricants from joints. Do not direct water at openings normally masked during regular washes. Avoid direct spray into openings and surfaces
1.5 Weather
The contractor may be required to provide services IAW this PWS during inclement weather.
1.6 Environmental Compliance
The Contractor shall comply with all Federal, State, Local and Wright-Patterson AFB environmental laws, regulations and directives.
1.6.1. The Contractor shall monitor and inspect all Contractor and Government provided equipment to ensure all equipment is serviceable and does not leak or release fluids into the environment. The Contractor shall ensure drip pans are fully utilized, if necessary. The Contractor will clean drip pans as necessary.
1.6.2. The Contractor shall clean up all hazardous fluid spills within one hour. The Contractor shall notify appropriate agencies immediately, including the Fire Department and COR, as spelled out in 445 AW Instructions available from the 445 MXG Quality Assurance office.
1.6.3. The contractor shall cleanup spills that occur during the course of performance. The contractor is responsible for any fines, penalty assessments, and/or natural resources damage claims that may be brought against Wright-Patterson AFB by any Government entity due to the Contractor’s or the Contractor personnel’s performance or failure to perform during the execution and performance of the contract.
NOTE: The Contractor shall comply with Federal, State, Local, Air Force, and Wright-Patterson AFB regulations pertaining to the purchase, storage, handling, and reporting of Hazardous Materials (HAZMAT) as well as proper disposal of all Solid Wastes and potential Hazardous Waste.
1.7. Facility Cleanliness
1.7.1. The Contractor shall be responsible for the overall cleanliness and orderly appearance of the work area. All areas will be free of Foreign Objects. After completion of Wash Services, the Contractor shall clean the facility floor area and shall ensure no standing water is left on the floor. The Contractor shall also remove and prepare equipment for storage and reinstall all Contractor equipment and any Government furnished equipment when requested by the COR.
SECTION 2
Services Summary (SS)
2.1. SERVICE SUMMARY (SS). The purpose is to define performance evaluation and payment procedures. The contract service requirements are summarized in performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimally acceptable levels of service required for each requirement. These thresholds are critical to mission success. Where re-performance is not possible, the CO, with coordination with the COR, will determine the remedy in accordance with Federal Acquisition Regulation 52.212-4(a)—Inspection/Acceptance.

The procedures set forth in the clause at FAR 52.212-4(a) Inspection/Acceptance will be used to remedy all deficiencies. Depending upon the sensitivity of the issue, the
Government may direct the contractor to re-perform defective tasks or services by a specific time within the maximum 1-hour window allowed for re-performance.
SECTION 3
Government and Contractor Furnished Items
3.1. Government Services
The Government shall provide the following services to the Contractor at no cost to the Contractor.
3.1.1. Utilities: Electricity, water, sewage.
3.1.2. Initial training: The Government shall schedule and provide initial training and certify Contractor personnel who require operational training to perform this contract. Up to five (5) Contractor personnel identified as trainers will be provided initial operational training by the Government for a period of up to 30 days after award of contract. Thereafter, the Contractor shall train their personnel. This training, discussed in Paragraph 3.3 of this PWS, is not all inclusive and additional training will be conducted as necessary to fulfill the scope of the contract.
3.2. Government Property
The Government shall provide use of the following property to the Contractor at no cost to the Contractor.
3.2.1. Non-Powered Aerospace Ground Equipment (AGE): The Government will make the following AGE available to aid in the performance of aircraft wash services.

The Contractor shall inspect equipment for serviceability and condition prior to use. The Contractor shall notify in writing to the COR of any unserviceable or damaged equipment. The government will annotate any discrepancies on the attached AFTO Form 244, Industrial/Support equipment Record.
3.3. Government Training
Prior to a Contractor Employee performing any service on this contract, the training below shall be completed. The following is a list of mandatory Government furnished training, approximate time to complete, and frequency. All Government-furnished training will be arranged and documented by Contracting Officer Representative (COR). While the cost of training will be furnished at no cost to the Contractor, the Contractor shall pay for the employee’s time while in training.

NOTE: The mandatory training hours listed are for the initial training only. The training hours are an average. It may take longer or shorter to complete the required items.
3.4. Government Materials
The 445th Maintenance Squadron shall provide all soaps, cleaning agents, wash carts, foamers, pressure wash system(s), and cleaning pads required to accomplish the wash.
NOTE: In the event of unavailability of AF provided materials/equipment resulting in a delay of the wash process, the maximum allowable time will be extended to cover the delay
3.5. Contractor Furnished Items
Except for those items and services specifically stated as Government furnished, the Contractor shall provide everything required to perform this PWS according to all terms and conditions. This includes rain suits, boots, face shields, goggles, and any other Personal Protective Equipment (PPE) required.
SECTION 4
General Information
4.1. Contractor Employees: The contractor shall provide, in writing to the Contracting Officer, the names and positions of all personnel that will be employed under this contract within two workdays of start of their employment. The Contractor shall not employ any person who is an employee of the U.S. Government if employing that person would create a conflict of interest. Additionally, the Contractor shall not employ any person who is an employee of the United States Air Force, either military or civilian, unless such person seeks and receives written approval prior to employment through appropriate channels and provides an authorization AF Form 3902, Application and Approval for Off-Duty Employment, to the COR office.
The contractor shall provide documentation to validate employees have watched the Aircraft Wash Procedures video and the printed certificate is on file. The Government has the authority to restrict the employment of any employee, or prospective employee, who is identified as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population. All employees must be able to fluently read, write, speak, and understand English. The contractor shall act on all contract matters relating to the daily operation of this contract.
4.2. Off-Duty Employment of 445 Air Wing Federal Civilian Employees: The Contractor will not hire or attempt to hire any 445 Air Wing federal civilian employee as this represents a conflict of interest and violates 18 USC Section 203 and 18 USC Section 205. This includes all Air Reserve Technicians (ARTs) who work in the wing.
4.3. Hours of Operation: Normal hours of operation will be Monday-Sunday 0700-1600 (Excluding any Federal Holidays). For scheduled aircraft wash services the Contractor personnel shall be on-site and prepared to perform all scheduled services within 1 hour prior to start of services.
4.4. Employee Uniform and Appearance: All Contractor employees shall wear a distinctive work uniform containing the company name and the employee name. Employees shall present a neat, clean, safe and professional appearance at all times. Safety toed shoes shall comply with current American National Standards Institute (ANSI) guidance and shall be worn at all times. Hair shall conform to all OSHA standards for working around machinery. All clothing shall comply with OSHA standards as well.
4.5. Security Requirements: All on-site contractor personnel shall apply for security clearances within five days after employment and obtain an AF Form 1199, Entry Control Card, for each on-site employee through the 445 MXG Security Manager. All other employees must complete a DD Form 398-2, Department of Defense Personnel Security Questionnaire, and undergo a local files check through the AF Law Enforcement Terminal System (AFLETS). Qualified Contractor on-site employees must, at all times, escort all employees that do not possess a valid AF Form 1199, Entry Control Card. The Contractor may not employ any personnel on this installation that are not able to obtain a favorable National Agency Check.
The Contractor shall obtain and pay for all security clearance investigations required to provide services under this PWS. The Contractor shall obtain a valid AF Form 75, Visitor/Vehicle Pass, for all on-site employees and vehicles used for daily operations from the 88th Security Forces Squadron.
4.6. Training and Qualifications: The Contractor shall provide employees who are trained, qualified, and certified for specific job tasks to be performed. No employee shall engage in any job task unsupervised, without training record documentation certifying qualifications to perform said task. Records of training, qualification and certification shall be maintained by the Contractor on file and made available for Government review 24 hours a day upon request.
4.7. Quality Control: The Contractor shall provide a Quality Control Program to ensure that aircraft wash and corrosion control services are performed in accordance with commonly accepted commercial practices and services identified in this PWS. The contractor shall develop and implement procedures to identify, prevent, and ensure non-performance and continual repeat of defective service does not occur. As a minimum the contractor shall develop quality control procedures addressing the areas identified in paragraph 2, Services Summary. It shall also identify how the Contractor will ensure non-recurrence of defective services shall be identified and prevented from happening again. A record of completed inspections shall be kept and made available to the Government throughout the contract period.
Provide a final copy of the Quality Control Plan to the Contracting Officer and COR within 10 days after contract award.
4.8. Contractor Liability: The Contractor shall assume strict liability for any damages to aircraft, aircraft parts, support equipment, facilities, or other Government property caused by any activities under, or in violation of, this contract. The Contractor shall be assessed the full cost (parts, labor, and administrative activities) of any repairs required to correct damages. The Contractor may be given the option to repair any non-aircraft related damage at the Government’s discretion, in lieu of payment. Any repairs shall be pre-approved by the Government. The final product shall meet Government specifications and be certified as acceptable by the COR and/or Government. Reasonable wear and tear is excluded.
4.9. Physical Security: The Contractor shall safeguard all Government furnished property. At the close of each work period, Government facilities, equipment, and materials shall be secured. Any damage to Government furnished property shall be reported to the COR within 1 hour of discovery. The Contractor shall establish and implement a written methods for ensuring that all keys issued to the Contractor are not lost or misused by unauthorized persons.
4.10. Records: The Contractor is responsible for meeting the requirements established in the AFRIMS Records Disposition Schedule (RDS), AFI 33-322, Records Management Program, AFI 33-364, Records Disposition Procedures and Responsibilities, and AFMAN 33-363, Management of Records when creating, handling and maintaining records for the Air Force, either electronic or paper. In order to meet the requirement established IAW the above mentioned regulations, contact the Base Records Management Office (445 CF/SCB, Bldg. 4010) at extension 7-1672 for guidance and training.
4.11. Reports: The Contractor shall submit reports IAW instructions outlined in the PWS and as required by the COR. The Contractor shall retain all logs and reports for the duration of the contract.
4.11.1. The Contractor shall report ALL Contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract via a secure data collection site.
The Contractor is required to completely fill in all required data fields at http://www.ecmra.mil.
4.11.1.1. Reporting inputs will be for the labor executed during the period of performance for each Government Fiscal Year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year. Contractors may direct questions to the Contractor Manpower Reporting Application (CMRA) help desk.
4.11.1.2. Uses and Safeguarding of Information: Information from the secure web site is considered proprietary in nature when the contract number and Contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the Contractor name and contract number associated with the data.

4.11.1.3. User Manuals: Data for Air Force service requirements must be input at the Air Force CMRA link. However, user manuals for Government personnel and Contractors are available at the Army CMRA link at http://www.ecmra.mil.
4.12. Technical Orders, Publications, and Forms: The Contractor shall utilize appropriate technical orders, publications and forms in effect at the time of performance. These documents are subject to modification, addition and deletion during the course of performance. If additional technical orders, publications, or forms are required for executing this PWS, arrangements to acquire them can be made through the 445 MXG QA Technical Order Distribution Office or the COR.
4.13. Safety: The Contractor shall follow all Occupational Safety and Health Administration (OSHA) standards as outlined in OSHA 29 CFR 1910, 1926 and 1926.56. The Contractor shall adhere to all aircraft safety directives. Contract personnel shall comply with the of State of Ohio and Wright-Patterson AFB driving regulations and all AF regulations governing fire prevention and fire reporting procedures.
4.14. Conservation of Utilities: The Contractor shall instruct employees in utilities conservation practices.
The Contractor will ensure operations preclude the waste of utilities. Water faucets or valves shall be turned off after the required usage has been accomplished. All water hoses and nozzles must be maintained in good working order to prevent any leakage.
4.15. Housekeeping: The Contractor shall maintain a clean and professional appearance in all work areas during operations and perform detailed cleaning at the end of each work shift. When the designated wash facility is utilized for wash services, the Contractor will clean the entire hangar floor and will ensure that no free standing water is left on the floor after completion of the wash. The Contractor will provide all supplies for daily administrative operations and daily housekeeping.
4.16. Industrial Safety and Health Requirements: The Contractor shall be responsible for the following:
4.16.1. Environmental, Safety, and Occupational Health (ESOH). The Contractor shall be solely responsible for compliance with all federal, state and local laws, the Occupational, Safety and Health Act (OSHA) (Public Law 91-596) and the resulting standards, OSHA Standards 29 CFR 1910 and 1926, as applicable, and for the protection of their employees. Additionally, the Contractor shall be responsible for the safety and health of all subcontractor employees. The Contractor shall provide protection to Government property to prevent damage during the period of time the property is under the control or in possession of the Contractor.
4.16.1.2. An applicable Contractor Quality Control Plan shall identify the processes and procedures the Contractor will use to track compliance with the Safety and Health Plan, and the process and procedures that will be used to correct violations.
4.16.1.3. The Contractor shall submit a Safety and Health Plan and corresponding site safety checklist to the contracting officer 10 days after contract award. The Contractor’s plan shall include appropriate measures to ensure the Contractor reacts promptly to investigate, correct, and track alleged safety and health violations and/or uncontrolled hazards in Contractor work areas. The plan shall:
A. Demonstrate a management commitment to employee safety and health;
B. Identify the application of the safety and health plan to subcontractors;
C. Identify the roles and responsibilities of the following individuals with respect to safety:
Management, Supervisors, Employees, and Safety Coordinator;
D. Identify applicable safety rules and regulations;
E. Include a worksite hazard analysis to include base-line hazard identification and required http://www.ecmra.mil/ control measures;
F. Include a job site analysis to include hazards of tasks required to control measures;
G. Identify employee safety and health training requirements and the documentation process;
H. Include a workplace inspection frequency, to include identifying the individual conducting the inspections;
I. Include employee hazard reporting procedures;
J. Identify individual(s) responsible for corrective action of hazards;
K. Identify first aid/injury procedures;
L. Identify procedures for accident investigation and reporting;
M. Identify emergency response procedures; and N. Identify the process for tracking controlled hazards in Contractor work areas.
4.16.2. Contractor Employees. The Contractor shall be exclusively obligated to comply with OSHA and other applicable laws and regulations for the protection of their employees. Air Force Occupational Safety and Health Standards (AFOSH STDs) are annotated because many of the Air Force Standards exceed the OSHA standard criteria. If a conflict is noted, the most stringent requirement takes precedence. The Government will assume no liability or responsibility for the Contractor’s compliance or non-compliance with such requirements. The Contractor shall furnish to each of his/her employees a place of employment which is free from recognized hazards. The Contractor shall brief his/her employees on the safety requirements of this contract and on hazards associated with prescribed tasks. In addition, the Contractor is required to flow down the safety requirements/specification to all subcontractors. This contract shall in no way require persons to work in surroundings or under working conditions which are unsafe or dangerous to their health. The Contractor shall coordinate and perform work so as not to impact the safety of Government employees or cause damage to Government property. This requires providing personnel with protective equipment and associated safety equipment as may be necessary. The Contractor must also protect personnel from hazards generated by the work.
4.16.3. Inspections. Air Force Environmental, Safety, Occupational Health and Fire protection officials or responsible COR may periodically enter the Contractor’s workplace to verify working conditions. If an Air Force employee observes improper procedures or validates unsafe conditions exist, which place Air Force personnel or equipment/facilities in jeopardy, the Contractor shall remove personnel and/or equipment from the unsafe condition and immediately notify the Contracting Officer (CO) and the Installation Safety Office. Department of Labor, OSHA inspectors are authorized right of entry to inspect any place of employment operated by an Air Force Contractor. They are, for the most part, “NO NOTICE” inspections. The Contractor shall notify the Installation Safety Office and COR if an OSHA inspector visits unescorted by an Air Force Safety Technician and/or COR. Noncompliance with safety requirements can result in work stoppage, inexcusable delays, and/or costly fines issued by the Department of Labor (DOL).
The Government will assume no liability or responsibility for the Contractor's compliance or non-compliance with such requirements. Fines and additional costs for violations levied against the Contractor as a result of OSHA findings, and/or installation safety, fire, health or environmental are the sole responsibility of the Contractor and shall not be passed through to the Government.
4.16.4. Mishap Notification. The Contractor shall notify the COR office and the Installation Safety Office within one (1) hour after initial notification of all mishaps or incidents of damage to Department of Defense (DoD) property (material plus labor) entrusted by this contract and employee injuries as a result of the contract operation. A follow-up (written or e-mail) of this mishap/incident notification shall be sent within three (3) calendar days to the Installation Safety Office, COR and CO. The information that is not available at the time of the initial notification shall be provided within 20 calendar days after the mishap. Mishap notification shall contain, as a minimum, the following information:
A. Contract, Contract Number, Name and Title of Person(s) Reporting B. Date, Time and exact location of accident/incident C. Brief Narrative of accident/incident (including events leading to accident/incident) D. Cause of accident/incident, if known E. Estimated cost of accident/incident (material and labor to repair/replace) F. Nomenclature of equipment and personnel involved in accident/incident G. Corrective actions (taken or proposed) H. Other pertinent information
If requested by COR or CO, the Contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenance and training records, until released by the investigating safety office.
If the Government investigates the mishap, the Contractor and the subcontractors shall cooperate fully and assist Government personnel until the investigation is finalized and closed out.
Note: *A CLASS A mishap is: A fatality or property damage in excess of 1 million dollars
*A CLASS B mishap is: An injury resulting in permanent or partial disability or property damage in excess of 200,000 dollars *A CLASS C mishap is: An injury or occupational illness resulting in lost workdays or property damage in excess of 20,000 dollars up to 199,999 dollars.
